Anamnesis

Early
• The occurrence of a recently concave eye.
• The duration and frequency of vomiting or diarrhea, as well as the old appearance of vomiting or diarrhea.
• The last time urinating
• Since when the hands and feet are cold.

Advanced
• Eating habits before sickness
• Eat / drink / breastfeed during sickness
• The amount of food and fluids in the last few days
• Contact with people with measles or pulmonary tuberculosis
• Have had measles in the last 3 months
• Occurrence and causes of death from brother or sister
• Birth weight
• Growing flowers, for example: sitting, standing, and others
• History of immunization
• Was weighed every month at posyandu
• Have been fully immunized

 Physical examination
• Does the child look very thin / edema / swelling of both feet

• Signs of shock (cooling): cold hands and feet, weak pulse, and decreased consciousness

• Body temperature: hypothermia or fever

• Thirst

• Respiratory frequency and respiratory type: Pneumonia or symptoms of heart failure

• Weight, height or length

• Enlarged liver and yellowishness (jaundice) in the whites of the eyes (conjunctiva)

• Flatulence, intestinal sound, and the presence of sound like a blow to the water surface (abdominal splash)

• Pale very heavy especially on the palm of the hand (compare with the mother's palm)

• Symptoms of the eye: abnormalities of the cornea and conjunctiva as signs of vitamin A deficiency

• Ear, mouth, and throat: signs of infection

• Skin: signs of infection or the presence of purpura

• The appearance (consistency) of the stool
