ORGANIZATIONS
• List of activities accomplished (See the template for List of
Accomplished Activities) in the preceding year with ALL ACTIVITY
REPORTS FOR EACH ACTIVITY proofs attached.
• Community project/s accomplished in the preceding school year
with ALL ACTIVITY REPORTS FOR EACH ACTIVITY proofs attached.
• Audited financial statement/unaudited FS with official receipts.
• Information Brochure about the organization (1 page, 8.5x11 size;
the brochure may contain but not limited to the following: about

General procedure
STEP 1: Attend the Orientation
on Accreditation of Student
Organizations on August 9,
1:30pm at COSG Office.
STEP 4: Submit the completed
forms and requirements at the
OSAS Main Campus office not
later than September 9,
6:00pm. No late submissions
will be accepted.
STEP 2: Get copies of accreditation
forms and other requirements from
OSAS Main Campus Office (Rm113c).

STEP 5: Presidents and Secretaries should attend
the Accredited Student Organizations General
Assembly, General Planning, and Synchronization
of Student Activities (including CSG and COSG) on
September 13 (venue and time: TBA). The
purpose of the assembly is to synchronize and
adjust the proposed program of activities to avoid
overlapping campus activities and to manage our
limited venues for activities as well as to prepare
for activities that will be needing parental
consents and for activities to be conducted off-
campus.
STEP 3: Accomplish the required forms and
other requirements completely. Please
make sure that all information in all the
forms are completely and correctly filled
out. Each members and officers must
provide photos where needed. Print out
photos for as long as clear and decent are
acceptable. Make sure also that all
signatories have signed all sections that
need signing.
STEP 6: Wait for the
announcement of approval of
your application. The documents
will be forwarded to higher ups
for approval and eventual release
of university memorandum.

reminder
Student Organizations, once they have submitted their
completed forms and other requirements even without the
university memorandum (pre-cognition), can start
conducting their activities provided that they will follow
protocol on conducting student activities as indicated in the
2012 Student Handbook. However, student orgs with
pending accreditation that will be excluded will have to stop
their operation immediately.
