Purposes

 To removed soiled dressing to prevent spread


of infection.
 To observe the nature of wound for
information.
 To control bleeding from the wound.
 To protect the wound from mechanical injuries
and contamination with micro-organisms.
 To ihhit or kill organisms by using dressing
that contains antiseptic solutions.

Principles
• Maintain strict aseptic techniques while changing
dressing,examining,and closing the wound.
• Everything that comes contact with the wound
must be sterile.
• Hand should be throughly washed before and
after each dressing to avoid cross infection.
• All articles should be disinfected so that they will
be free from pathogens
• Use masks,sterile gloves and gown for large
dressing.

• While cleaning a wound,clean from the lwast
contaminated area to the more contaminated
area.
• Dressing is changed at least 15 minutes after
the room has been cleaned and meal timing.
• Swith off the fan.
• Avoid coughing sneezing and talking while the
wound is opened.
• Use single stroke for cleansing the wound.

Procedures
• Check the physician order for dressing change
and any specific instructions
• Identify the patient who require wound
dressing and the types of dressing to be
performed.
• Inform patient of dressing change and explain
the procedure to the patient to win his
cofidence and co-opreation.
• Wash hands

• Gather the equipments and arrange the bed
side.
• Maintain privacy by closing the door or using
screen.
• Close doors,windows and fans to prevent drfts
and dust.
• Assist the patient to a comfortable position that
provides easy access to wound area.place a
rubber sheet or water proof pad on bed beneath
the area of dressing.

• Place opened,cuffed plastic bag or bucket near
working area.
• Loosen tape on dressing by holding down the
skin and pulling the tape gently but firmly
towardst wound.if the tape is soiled, wear
clean gloves before loosening the tape.
• If the dressing is ahderent to the skin,mosten it
by pouring small amount of saline.
• Remove soiled dressing carefully from more
clean to less clean to less clean area and
discard it in a proper receptacle.

• Keep the soiled site of dressings away from
from the patient`s view to reduce anxiety of
the patients.
• Assist the color,amount and odor of drainage,it
helps the identifying the wound healing
process.
• Pull off gloves inside out and discard if
wearing before.
• Wash hands throughly with soap and water.
• Using sterile technique,open sterile dressing
tray and arrange supplies on work area.

• Open cleaning solution and pour into the
sterile gallpot over the cotton balls.
• Don sterile gloves and gown if needed.
• Picked up soaked cotton using artery forceps.
• Clan the wound with cotton swab.
o For surgical wound clean from the top to
bottom or from center to periphery.
o For infected wound clean from the center to
periphery.

o Circular motion for cleaning circular wound.
o If culture swab is required,obtained it before
cleaning the wound.
o Moving from least to most contaminated area
prevents spread of micro organism to less
contaminated area.
• Use one cotton swab or gauze for each
wipe.discard each by dropping into the plastic
bag after wiping.Do not touch the plastic bag
with forceps.

• Use more swabs if the wound is dirty,otherwise 1-
2 swabs are enough for cleaning the wound.
• Avoid touching the wound directly.
• If drain is present,clean around it,moving from
center to outward in a circular motion.
• Dry the wound using sponge in the same
motion.Moisture provid medium for growth of
micro organism and drying the wound may retard
the growth of organisms and improve the healing
process.

• Apply medication ordered to the wound on dry
sterile gauze and apply a layer of sterile
dressing over the wound or apply medication
with a cotton applicator(additional dressing
serves as a quick for drainage).
• Place sterile gauze slit on side under and
ground the drain.Use precut gauze or cut one
using sterile scissors.
• Apply a second layer of gauze to wound site
and a surgical pad as the outer most layers.

• Remove gloves from inside out and discard in
a plastic waste bag.
• Apply adhesive tape to secure the dressing or
fixed with bandage.
• Discard the soiled dressing in a proper place.
• Explain the condition of the wound to the
patient and relatives and provide instruction
regarding care,rest,exercise and diet.
• Make the patient comfort.

• Replace articles.
• Decontaminate reuseable articles to be sent for
sterilization.
• Wash hands.
• Record the procedure with date and time and
condition of the wound and medication used.
