From the daily attendance records it is found that you are absent from without intimation for two meetiing
dated 2 january,2018 and 8 january 2018 without prior approval of leave. Keeping absent yourself without
intimation is a misconduct under model standing order act, your absentee is hampering the work. You are
hereby called upon to give your explanation within 48 hours in writing, as why disciplinary action should not
be taken against you.
