 Assessment
 Dietary Intake Data -Dietary intake refers to the daily eating patterns
of an individual, including specific foods and calories consumed
and relative quantities.
 Daily Food and Activity Diary- Keeping a record of your
daily food intake will help you stay on track when trying to lose
weight or maintain a healthy weight and activity levels
 A food diary is a record of foods that you eat on a meal-by-meal,
daily, or weekly basis. You can include nutritional or other dietary
information, organize and track foods by food group, and so on.

Food frequency checklist Food
Frequency Questionnaire (FFQ) is a limited checklist of foods and beverages with a frequency response section for subjects to report

how often each item was consumed over a specified period of time.

24 hours recall-

 A 24-hour diet recall is a dietary assessment tool that consists of a


structured interview in which participants are asked to recall all food
and drink they have consumed in the previous 24 hours. It may be
self-administered.

Physical Assessment

 MAC Mid arm circumference -MUAC is the circumference of the left


upper arm and is measured at the mid-point between the tips of the
shoulder and elbow. To measure: Bend the left arm, find and mark
with a pen the olecranon process and acromium. Mark the mid-
point between these two marks.
 MUAC is the circumference of the left upper arm and is measured
at the mid-point between the tips of the shoulder and elbow. To
measure:
 Bend the left arm, find and mark with a pen the olecranon process

 Mark the mid-point between these two marks.
 With the arm hanging straight down, wrap a MUAC tape around the
arm at the midpoint mark.
 Measure to the nearest 1 mm.
 It is easy to measure MUAC even on very thin arms.
 MUAC of between 125mm (12.5cm) and 135mm (13.5cm), YELLOW
COLOUR, indicates that the child is at risk for acute malnutrition and
should be counselled and followed-up for Growth Promotion and
Monitoring (GPM). MUAC over 135mm (13.5cm), GREEN COLOUR,
indicates that the child is well nourished.
 Malnutrition refers to deficiencies, excesses, or imbalances in a
person's intake of energy and/or nutrients. The
term malnutrition addresses 3 broad groups of
conditions: undernutrition, which includes wasting (low weight-for-
height), stunting (low height-for-age) and underweight (low weight-
for-age);

 The thickness of these folds is a measure of the fat under the skin,
also called subcutaneous adipose tissue. Skinfold thickness results
rely on formulas that convert these numbers into an estimate of a
person's percentage of body fat according to a person's age and
gender.

MUST screening tool (Malnutrition
Universal Screening Tool)
 MUST' is a five-step screening tool to identify adults, who are
malnourished, at risk of malnutrition (undernutrition), or obese. It also
includes management guidelines which can be used to develop a
care plan. It is for use in hospitals, community and other care
settings and can be used by all care workers.
Subjective Global Assessment tool
(SGA)
 SGA is a simple bedside method used to diagnose malnutrition and
identify those who would benefit from nutrition care.
The assessment includes taking a history of recent intake, weight
change, gastrointestinal symptoms and a
clinical evaluation. SGA has been validated in a variety of patient
populations.

Mini Nutrition Assessment tool
(MNA)
 The MNA® is a validated nutrition screening and assessment tool
that can identify geriatric patients age 65 and above who are
malnourished or at risk of malnutrition. The MNA® was developed
nearly 20 years ago and is the most well validated nutrition
screening tool for the elderly.

Geriatric Nutritional Risk Index

 GNRI is not an index of malnutrition, but it is a “nutrition- related” risk


index because GNRI scores are correlated to a severity score that
takes into account nutritional status-related complications (bedsores
and infections).
