HOT SPOON BATHING

Objective of presentation

 At the end of the presentation, students would be


able to define
 1.Hot fomentation and hot spoon bathing
 2.State the indications/benefit of hot fomentation
and hot spoon bathing
 3.Conditions for hot fomentation and hot spoon
bathing
 4. Contraindications for hot fomentation and hot
spoon bathing
 5.Know the requirement needed for the procedure
 6 .A demonstration of how the procedure is done
 Fomentation is derived from the Latin word
“Fomentum” which mean soothing application or
poultices
 Its any warm application that delivers heat to the
body for healing purposes.
HOT FORMENTATION

 Moist application of heat over an area (eye) by


means of double thickness of flannel or other soft
material wring out from hot water.
 Hot formentation can be done at the clinical setting
or patient can be educated to perform procedure at
home.
INDICATIONS

 To clean the eye


 To relieve pain
 To increase circulation
 To reduce inflammation or swelling
 To unclog swollen eyelid eg in stye and infected cyst
it helps to aid rupture.
 Add moisture: It can ease the gritty sensation that
comes with dry eyes
 Muscle spasm: The wet warmth can relax the eye
CONDITIONS FOR HOT FOMENTATION

 Stye
 Chalazion
 Dry eyes
 Blepharitis (swollen of the eyelid)
 Swollen eye as in eye injury, orbital cellulitis, grave
disease
CONTRAINDICATIONS

 Numbness around the eye eg. If there is impaired


sensation around the eye.
 Visible wound of the eye. Application of hot
fomentation around an open eye wound will cause
more pain to patient
REQUIREMENTS NEEDED FOR HOT
FOMENTATION

 A tray containing the following


 Two receivers,
one for used swabs
the other for disposable gloves, face towel ,Vaseline
 Gallipot containing cotton wool soaked in normal saline
 Two jugs, one for hot water and the other one for cold
water
 A bowl to mix the hot and cold water
 Jaconet cape (waterproof cape) and towel
 Bath thermometer (if available)
 Table and chair
 Patient folder
PROCEDURE

 Confirm patient identity by mentioning his or her name


 Explain procedure and reassure to patient to allay any fear
and anxiety
 Provide privacy by screening the place or closing windows
 Aid patient to sit comfortably by providing patient with a seat
of appropriate height
 Send tray to patient bed side
 Protect patient dress with jaconet cape and towel
 Wash and dry hands
 Put on disposable gloves
 Ask patient to close eyes and apply vaseline around affected
eye to prevent burns and scalds
 Mix hot and cold water
PROCEDURE CONTINUES

 Check the temperature of the water using bath thermometer.


In the absence of bath thermometer, use the back of palm or
the elbow to check the hotness of water.
 Dip face towel into the water and squeeze off excess
 Instruct patient to cover eye and apply the warm towel on the
eye for 15-30 seconds
 Repeat procedure for several time
 When done, thank patient for his or her cooperation
 Instruct patient to continue with the procedure at home
 Decontaminate reusable items and dispose usable items
 Wash and dry hands
 Document procedure in patient folder
HOT SPOON BATHING

 It is the application of hot steam on the eye using a


padded spoon or spatula dipped in hot water.
INDICATIONS

 To clean the eye


 To relieve pain
 To increase circulation
 To reduce inflammation or swelling
 To unclog swollen eyelid eg in stye and infected cyst
it helps to aid rupture.

REQUIREMENTS

 A clean tray containing the following


 Two receivers one for used swabs and the other
containing a piece of clean material(gauze bandage,
disposable gloves)
 A jug containing hot water
 A bowl for the hot water
 Jaconet cape and towel
 Two gallipot, one containing cotton wool soaked in
normal saline and the other containing vaseline
 Patient folder
PROCEDURE

 Explain procedure to patient and inform him/her of its benefit


 Reassure the patient to allay any fear and anxiety
 Provide privacy
 Send prepared tray to patient bed side
 Provide a comfortable position for patient with a table placed
infront of patient
 Wash and dry hand
 Cover patient with jaconet cape and towel
 Gloves are worn and vaseline applied around the affected eye
 Wrap a pieces of clean material around the end of a wooden
spoon to form a pad
 Place a bowl in front of the patient and pour hot water into it
PROCEDURE CONTINUES

 Ask the patient to bend over the bowl of hot water. Dip the
pad into the water and bring it up close to the affected eye
which must be kept shut
 Allow steam to rise
 To avoid scalding, do not let the pad touch the patient eye
 When the pad cools, dip it again
 Keep the water hot by adding fresh water as necessary
 Repeat procedure for a period of 15 minutes
 Ask patient to open the eye after procedure and remove
jaconet cape and towel
 Thank patient for his/her cooperation
 Decontaminate reusable items and dispose off used items
 The procedure and findings are documented in patient folder
