CHARACTERISTICS OF INDUSTRY:
 Most of the hospitals have on-site laboratories to perform blood transfusion
 Small Inventory of blood was kept on site

HEALTH CARE TRENDS

No. of trends in the industry made the CBB’s centralized testing Strategy more attractive

Hospitals facing pressure for cost reductions from 3rd party payers ,patients & Corporations

An Acute shortage of qualified lab Technicians & physical space constraints

Complexity in transfusion medicine is growing hence increased the value of expertise

CENTRAL BLOOD BANK

Total operation Transfusion services

Non-profit They distribute more
included staff of 400 are small part of
organization serving than 400000 blood
people, having 161000 CBB’s product and
32 hospitals donation units products per year
services

Initially all the hospitals They kept on-site They Also Provided CBB is having Van
were connected via facility equipped and “Runner services” which Services for
walkways and within 5- staffed for STAT testing had dedicated staff to for
10 min walking in some hospitals who transportation of blood Shipment and
distance were little far from CBB between hospital & CBB delivery of blood

THE PROPOSAL

CBB would centralize the testing for St. Elizabeth’s hospital into CBB’s facility
• What should Paul do to get St. Elizabeth's and other
PROBLEM STATEMENT hospitals to accept his proposal along with
addressing concerns of stakeholders?

Proposal by CCB to hospital

• Centralize the testing for St. Elizabeth's hospital into CBB’s facility
• Offers employment to hospital lab technicians and purchase of its equipment at reasonable
value
• Kept an on-site facility equipped and staffed for STAT requests and provided runner service to
drop off crossmatched blood
• Savings in 20% of direct costs per year
CONCERNS OF HOSPITAL
Jackson Duke from St. Elizabeth was concerned about aggravation that might be
caused due to multiple people being involved in testing which might lead to risk of life

CONCERNS OF STAKEHOLDERS

The lab technicians felt their work would be dehumanised and would lose link with
the patient if the test’s were done by separate technicians as per the new system

The lab assistants were upset with their new assigned tasks as “runners” to deliver
blood from the on-site facilities to hospital floors

With centralised system, drivers would have to pick up samples and deliver more
frequently, sometimes within three-hour turnaround
DECISION MAKING UNIT OF HOSPITALS
Ecologists
Administrators Who performs transfusion in
Look for turnaround time and patient room, they look for
availability of cost cuttings. availability and expert
consultation

Surgeons Critical care units

Availability during emergency
They offer tests before a day,
situations .CBB offered STAT with
so they don’t concern about
1hour turnaround time
availability and cost.

Anesthesiologists Pathologists
They are support arms to surgeons, These people are in charge of
they want faster Reponses on transfusion labs, They don’t want to
special requests and quick turn lose control over major part of their
around time responsibilities
Clerical Staff
Pick up and delivery to and
from testing Site .CBB offered
“runner” service to carry blood
samples
ALTERNATIVES
1. Type/screen and cross matching be performed by two separate technicians at separate workstations as
it will reduce the turnaround time.
2. The job to be done by single person simultaneously in workstation. This will increase the turnaround time.

1. Real time information tracking system to keep check on every single testing and delivery of blood
products.
2. Increase in Van and drivers due to more frequent pick up and delivery of blood products to meet time
frames.

1. Use data to check the highest demanded type of blood type and maintain its inventory at the hospital
24*7 for emergency situations along with as emergency lab.

1. Pooling Medical Expertise into single laboratory. Technologists skilled in Blood Groups are hard to find.It
takes 20 years to become a veteran
AUTOMATION TESTING

Automation of Blood Group Testing is Simplified testing of

donor/patient blood samples on automated equipment

Automation is essential for Blood centers ,it provides higher safety and reliability for blood
grouping and antibody screening

Automation eliminates manual errors, reduces manpower requirement and has uniform
performance

Improves Blood stock management .Improves blood unit identification and distribution

Information can be reached immediately without compromising the quality. Reduces time
and improves efficiency
