On April 25, 2007
• At 8:00 a.m., WIB MT. Maulana departs from the Pier to the Port of Dumai to load 1,600 Kl premium. • At 10:00 WIB, the crew prepared to carry out the tank cleaning process for the next loading, because the type was different from the loading unloaded. • At 10.58 West Indonesia Time, one of the crew saw smoke coming out of the electrical terminal which was below the middle manifold, and then was extinguished using a portable extinguisher by Engineer II and Oil Officer. • At 11.00 WIB, when approaching to the smoky electricity terminal an explosion occurred in the tank loading 3 parts left and right. Finally the captain took action to land the ship. Then the captain ordered the crew to use a life jacket and then leave the ship. Blackouts were continued from the side of the river assisted by residents, POLAIR and TNI AL-Perawang. • At 1:30 a.m., the captain ordered the crew to cool the deck and evacuate. Causal Factor From the results of the analysis it was concluded that the cause of the MT. Maulana explosion caused by leakage of air pipes adjacent to the electricity terminal. Electrical terminals and cables in porous pipes have decreased insulation ability, causing heat and burning of cable insulation wrapped in porous pipes. The fire then triggered an explosion through a gas leak in the air pipe adjacent to the electricity terminal. Recommendation DIRECTORATE GENERAL OF SEA TRANSPORTATION • Carrying out an SMS (ISM-Code) audit or authorizing an audit must be as detailed as possible, so that the SMS really becomes a guide in carrying out tasks on board • Supervise ISM-code audits conducted by non-governmental institutions that are given authority (PT. BKI) MANAGEMENT / OPERATOR OF TANKER SHIP • Do screening of the pipes installed on the deck when the ship performs docking to find out that the pipes are not porous. • If one round of operation is planned strictly, one ship may not transport different cargo values, and if forced to carry cargo of different grades, prepare sufficient time to carry out gas freeing and tank cleaning.
