The operation at the time of the incident was rigging
down the top drive (TDS). The IP was on the
manrider on the off driller's side. The retaining pin
(TDS to torque tube) had been removed. The IP
signalled the driller to lower the block. The guide
beam became unexpectedly free, allowing the TDS
assembly to rotate. The IP's right hand became
crushed between the TDS carrier and a derrick
beam.

p 7 casing float collar was being cleaned/dried up on
the rig floor using an air hose disconnected from the
pipe spinner. The air hose ball valve was half open.
pfter cleaning the collar the Derrick man (IP) who
was holding the hose asked the floor man to close
the air valve, but the floor man instead of closing,
opened the valve to full, causing the hose to swing
back & hit the IP's nose.

p crew was installing crash barrier posts using a
small Pile Driving machine. The post had been
driven to a depth of 1 meter when the Charge-hand
noticed that it was slightly of vertical. e wanted to
correct this, took a fist-hammer and forcefully started
hitting the post. The hammer was in poor condition
and the head detached from the handle at the
second blow.

 1/2" tubing was being picked up to the V-door.
Once the Tubing was at the V-door level, the
elevator was latched into the tubing and the winch
line removed and left at the rig floor. The Driller
proceeded to lift the block and assistant driller (IP)
was holding the tubing. When the block was picked
up, the block caught the winch line and moved it
toward the tubing.

During pipe connection in mouse hole, the Floorman
(IP) held the break out tong pull line (that was
already latched on to single pipe) with his right hand,
while the kelly spinner was operating to make up the
connection. ps the pipe rotated clockwise in the
mouse hole while the threads were being engaged,
the tong rotated causing the line to jerk under
tension.

the IP was doing isolation of an electrical panel for
the Beam Pump. e isolated the mains from a 15 V
feeder by turning the isolator knob to ³off´ position,
opened the feeder panel and removed three
fuses. While attempting to clean the upstream
connection of the isolator using a bristle brush, an
electrical flashover occurred. The metallic part of the
brush came in contact with the two phase terminals,
resulting (IP).

jammed line in a Stabbing Board sheave from the
loose end. The action resulted in sudden movement
of the line through the sheave due to gravity of
counter weight on the other side. The IP¶s hand got
trapped between the sheave and the line.

and on the little finger of the left hand.

installing the Wireline lubricator, when he heard

The Weatherford UBD crew were moving temporary
paperwork. p Field Operator (IP) and his colleague
were carrying a 2-ft long Teclock pup joint (35 kg wt)
to store it on the rack. ps they were walking towards
the rack, the colleague's glove slipped from the load,
the joint dropped, subsequently hitting the IPs right
foot. The IP sustained trauma on his right foot

trailer, using a crane. The Chief lectrician (IP) was
standing on the trailer, assisting the crane operator
with the loading operation, when the hook part of the
assembly suddenly swung, hit the IP on his right leg,

scaffolding that was to be used to inspect a heat
exchanger on the station. The IP slipped and fell on

pfter picking up and running in hole .1/2 inch liner,
stands of drill pipes were being run from the mast.
The Derrickman (IP), was assisting the floorman
(Derrickman reliever) to release the chain which was
securing the drill pipes stacked on the monkey
board. The IP partially sat near the stacked stands of
drill collars and in the movement path of the Kelly
hose. Whilst trying to release the chain, the Topdrive
was moved downwards for running in hole and the
Kelly hose rubbed the IPs helmet causing the helmet
to turn, which then got caught with the downwards
moving clamp of the Kelly hose squeezing the
helmet onto the jaw.

pn pssistant Driller (IP) and a derrick man were
assembling the sucker rod BOP on the ground.
While trying to re-align the extension flange type to
the bottom of the assembly, the sucker rod assembly
stripped off from the rod table thread connection,
trapping the IPs right hand middle finger between the
flange and the assembly.

p Mechanic (IP) was replacing the casing running
tool (CT) dies & packer cup from 9 5/8 inch to 7
inch. While unscrewing the packer cup screw,
trapped pressure was released. This caused the
casing segments part to be pushed upwards,

While working on the construction of a Steam
Injection Manifold, the crew used hydraulic jacks
mounted on timber to elevate approximately 80 m of
8" flowline, to allow surface preparation for welding
of pipe shoes. While the Fitters elper (IP) was
engaged in the activity of positioning the last of 5
jacks, the flow line slipped, hitting him across both
arms and his right thigh

p 50 T crane was lifting a pressure equipment skid
from the wash bay area to the wellhead equipment
area. The crane operator was getting ready to
position down the load to the ground when suddenly
the load started to drop down in an uncontrolled
manner. This caused the crane to topple on to its left
side, hitting the roof of a workshop and the wall of
the wellhead equipment area.

While working on road asphalting project, borrow pit
location, a elper (IP) was trying to start a
dewatering pump of the brackish tanker (pulley type
starter) for pumping the water to a borrow pit. is
right hand slipped while pulling the starter and his
right middle finger got trapped in the rotating shaft of
the pump.

  
£ The IP sustained crush injury to his right middle
finger tip with partial amputation and fracture.

While preparing for No Destructive Test (X-ray
welding) activities on a flange joint of 30¶¶ water
outlet line from T-809 at NPS, there was a flash
fire that resulted in burn injuries to three members of
the crew

  
£ Two of the crew members suffered from
superficial burns.
£ The third crew member sustained 2nd degree
burns of the face & hands and superficial burns
of the legs.
