INFARTO DE MIOCARDIO
• En pacientes con ACV isquémico o AIT en el curso de IM agudo
complicado con formación de trombo mural o anormalidad en el mov.
de pared apical con una FE <40%, intolerantes a VKA debido a eventos
adversos no hemorrágicos, el Tx con LMWH, dabigatran, rivaroxaban o
apixaban por 3 meses podría ser considerado como alternativa a VKA
para prevenir stroke rrecurentes o TIA.
IM
• Tratamiento con AVK (INR 2,0 -3,0 ) por 3 meses, en pacientes con
ACV isquémico o AIT, en el ajuste sd. Coronario agudo anterior sin
demostración de formación de trombo mural de Ventr. Izq. pero con
acinesia o discinesia apical identificados mediante la ecocardiografía u
otra modalidad de imagen (Clase IIb, Nivel de evidencia C).
MIOCARDIOPATIA
• En caso ritmo sinusal con trombos de aurícula o v. izquierdos
demostrado por ecocardio u otra imagen, tx con VKA es recomendado
por > de 3 meses.
• LVAD mecánico el tx con VKA (INR 2-3) es razonable en ausencia de
otra contraindicación. (LVAD :dispositivo de asistencia ventricular izquierda)
ENFERMEDAD VALVULAR
CARDIACA
• ACV O TIA con valvulopatia mitral y FA, tx con AVK a largo plazo con INR
2-3 es recomendado.
• VM sin FA u otra causa probable de sus síntomas el tx con AVK a largo
plazo con INR 2-3 se puede considerar en lugar de tx antiplaquetario.
• Enfermedad de válvula mitral no reumática o de válvula aortica sin FA u
otra indicación para anti coagular, la terapia antiplaquetaria es
recomendada.
• Calcificación del anillo mitral sin FA tx antiplaquetario es recomendado.
• Prolapso de válvula mitral tx antiplquetario es recomendado.
