Grade 10 grades

Result of the assessment and interview

Grade Requirements
ENGLISH, MATH AND SCIENCE must be 86.00 and above
General Average must be 85.00 and above
No grade below 82.00 in other subjects

Accomplished Application Slip (to be downloaded from this page
after the assessment of your online registration)
Two (2) pieces of 2x2 inch. recent picture with white background and
applicants’ name tag at the chest
Original:
• Form 138 (Grade 10)
• PSA Birth Certificate
• NCAE
• Certificate of Good Moral Character
Photocopy:
• Certificate of Completion
• Baptismal Certificate
• Grade 10 School ID

 Place in clean, long brown envelope with Surname, Given Name
and Middle Initial written at the upper left hand corner of the
envelope.
 LRN (Learner’s Reference Number) must be indicated in the
Report card
 ESC (Educational Service Contracting) or QVR (Qualified
Voucher Recipient) is strictly required to applicants coming
from private schools

alternative for the pants unless the student can
provide a letter of approval coming from the office
of senior high school. And also, girls should always
wear their necktie.
