DISCHARGING

A PATIENT FROM THE PATIENT CARE

Purpose

 To facilitate prompt discharge of patients from


the patient care area.
Definition of Terms

 Discharge Order
 Instruction from the physician that the patient is to
be released from confinement.

 DAMA (Discharge Against Medical Advice)


 Leaving the hospital at the patient’s or patient’s
representative’s insistence & against the advise
of hospital authorities & attending physician


Responsible Person/Responsibilities

 Physician
 Clears & orders patient discharge
 Nursing Staff
 Carries out documentation, notification and instructs
patient & significant others home health teachings
 Billing Staff
 Facilitates processing of patient’s hospital accounts
 Housekeeping Aide
 Accompanies a patient to transport vehicle
 Cleans vacated room & all equipment & amenities in
the room
