NOCICEPTION
NOCICEPTION:
= the sensory side of the pain includes:
phenomena related to the transduction of painful stimuli
at the level of peripheral receptors (nociceptors)
transmission of action potentials on the sensitive
ascending path
their cortical projection
the appearance of the perception of the painful stimulus
O NOCICEPŢIA:
= latura senzorială a durerii cuprinde:
O fenomenele legate de transducţia stimulilor dureroşi la nivelul receptorilor
periferici (nociceptori)
O transmiterea potenţialelor de acţiune pe calea ascendentă senzitivă
O proiecţia corticală a acestora
O apariţia percepţiei stimulului dureros
Modulation:
- changing the threshold of receptors and the intensity of sensations,
depending on the state of the nervous system and the body
- it occurs at all levels of the nociceptive conduction pathway, causing
either a reduction or an increase in sensation
Analgesia - failure to perceive a painful stimulus associated with
stress
Hyperalgesia - stimuli that are normally indifferent (e.g.
tactile stimuli) can cause pain
Modularea:
modificarea pragului receptorilor şi a intensităţii senzaţiilor, în funcţie de starea sistemului nervos
şi a organismului
se produce la toate nivelurile căii de conducere nociceptive, determinând fie reducerea, fie
amplificarea senzaţiei
Analgezia – neperceperea unui stimul dureros asociat cu un stres
Hiperalgezia - stimuli care în mod normal sunt indiferenţi (ex.
stimuli tactili) pot produce durere

TRANSDUCTION takes place by depolarizing the neural membrane:
CHARACTERISTICS:
is the local skin reaction to a painful, punctiform
stimulus
it takes place in three stages:
- local erythema
- local edema
- erythema surrounding
phenomenon it does not involve axons, but dendrites of
nociceptive primary neurons

MECHANISM:

REFLEXUL DE AXON
FUNCTIONS OF THE AXON REFLEX
trophic function:
- under normal conditions painless stimuli that reach the C fibers:
does not cause a sensation of pain
produce the release of small amounts of neuropeptides
local vasodilation → trophic role for tissues
a neuropeptide stimulating effect on proliferation fibroblasts and
angiogenesis
peripheral modulation of nociception by released neuropeptides
participates in the onset and maintenance of inflammation, depending on the
extent to which C fibers are stimulated
SECONDARY NEURONS
• pass into the anterior cords on the opposite side the spinothalamic tract:
• neospinothalamic tract
• leads information from fb. Aδ and some from fb. C, at the wide thalamus.
• → starts the thalamocortical bundle → projects to the primary and secondary
cortical somesthetic areas.
• Information from fb. rapid Aδ the immediate onset of pain
• together with the information from the other cutaneous receptors, which are
projected in the same area, contribute to the location and sensory
characterization of the painful stimulus
O NEURONII SECUNDARI
O trec în cordoanele anterioare de partea opusă tractul spinotalamic:
tractul neospinotalamic
O conduce informaţii de la fb. Aδ şi câteva din fb. C, la talamusul lat.
→porneşte fasciculul talamocortical → se proiectează la nivelul ariilor somestezice
corticale primare şi secund.
O Informaţiile ce provin de la fb. rapide Aδ apariţia imediată a senzaţiei de durere
O împreună cu informaţiile de la ceilalţi receptori cutanaţi, ce se proiectează în aceeaşi
zonă, contribuie la localizarea şi caracterizarea senzorială a stimulului dureros

extended cortical areas and towards the struct. subcortical (limbic
system, reticulated substance)
the information transmitted in this way has a reduced sensory
function, but determines the appearance of emotional changes and
vegetative phenomena that accompany nociception, in order to
produce the complex sensation of pain.

determină apariţia modificărilor emoţionale şi a fenomenelor vegetative ce
însoţesc nocicepţia, pentru a produce senzaţia complexă de durere

CORTICAL INTEGRATION
• ability to quantitatively assess pain sensation
(cognitive aspect of pain)
• motivational-affective reaction, achieved through the
connections of the cortex with the renencephalon and
the hypothalamus
• vegetative changes associated with pain (involving the
nuclei of the hypothalamus and the reticulated
substance of the brainstem).
INTEGRAREA CORTICALĂ
− capacitatea de evaluare cantitativă a senzaţiei dureroase (aspectul cognitiv al durerii)
− reacţia motivaţional-afectivă, realizată prin conexiunile cortexului cu rinencefalul şi
hipotalamusul
− modificările vegetative asociate durerii (la care participă nuclei din hipotalamus şi
substanţa reticulată a trunchiului cerebral).
