REZUMAT
n ultimii ani, utilizarea unor metode complementare i alternative n pediatrie a crescut considerabil, n
special pentru afeciunile cronice precum artrita juvenil idiopatic, n care durerea poate fi o problem
semnificativ. Interesul crescut n abordrile medicinei complementare i alternative pentru simptomele
durerii la copii i adolesceni, precum i la populaia adult, a concentrat atenia asupra problemelor de
siguran i eficacitate ale acestor metode. Urmrirea unor intervenii sigure, eficiente i rentabile pentru
durerea cronic la copiii care sufer de artrit juvenil, folosite n locul sau mpreun cu abordrile medicale
convenionale, este un obiectiv valoros, care ar trebui s fie abordat cu cea mai mare rigoare tiinific.
Trebuie s gndim creativ i diferit n scopul de a educa cadrele medicale, precum i publicul larg, pentru a
le schimba atitudinea.

INTRODUCERE
Metodele complementare i alternative de terapie au prezentat n ultimele decenii o adevrat
renatere la nivel mondial i naional, fiind deja
recunoscute de Organizaia Mondial a Sntii.
Aceste practici completeaz armonios arsenalul terapeutic actual cu noi valene i posibiliti, care,
mpreun cu procedeele din medicina alopat, ntregesc medicina modern a zilelor noastre (1).
n ultimii ani se constat o cretere permanent
a numrului de persoane care apeleaz pentru copiii
lor la mijloace de terapie complementar sau alternativ pentru a le mbunti starea de sntate,
ceea ce a dus la revizuirea, actualizarea i dezvoltarea
multor metode i practici de terapii tradiionale.

subliniaz c, n mod obinuit, majoritatea populaiei
mondiale apeleaz la practici de medicin tradiional, cum ar fi medicina tradiional chinez

Terapia cu ultrasunete este o form de tratament care genereaz o cldur profund creat de
undele sonore (20). Pe msur ce undele strbat n
profunzime esutul afectat, frecvene diferite sunt
absorbite de piele, esutul celular subcutanat,
muchi sau oase.
Ultrasunetele aplicate dup un protocol special,
urmate de un masaj profund i de stretching, sunt
folositoare n tratamentul de reabilitare la copilul i
adolescentul cu sechele locomotorii reumatismale.
Efectele termice sunt datorate absorbiei undelor
sonore; efectele fizice netermice sunt de cavitaie,
micro-streaming i acustic-streaming.
Rezultatul efectelor de cavitaie la nivelul esutului sunt provocate prin bule de aer microscopice,
care transmit vibraiile ntr-un mod special i stimuleaz direct membranele celulare.
Tehnicile cognitiv comportamentale de reabilitare a pacienilor cu durere cronic persistent
sunt considerate a fi printre cele mai valoroase
(21).
Biofeedback-ul se bazeaz pe folosirea instrumentaiei de reflexie n procesele psiho-sociale fa
de care individul nu este contient n mod normal i
care poate fi adus sub control voluntar. Prin biofeedback, pacientului i se furnizeaz informaii cu
privire la propria activitate fiziologic n relaie cu
rspunsuri cognitive, emoionale i comportamentale, permind astfel corectarea proceselor fiziologice, care de obicei nu sunt supuse controlului voluntar. Avantajele metodei sunt bazate pe efectele
psihologice de ameliorare sau de diminuare a simptomelor clinice precum: durerea, anxietatea, strile
de fric, insomnia, fobiile, depresia etc. (22).
Stimularea electric transcutanat este utilizat ca o form de terapie de sine stttoare sau n
combinaie cu alte procedee pentru combaterea
durerii (23).
Durerea din bolile reumatice de orice fel, beneficiaz n cel mai nalt grad de terapia prin metoda
acupuncturii. Copiii, dei rspund foarte bine la
aceast metod de tratament, n cele mai multe
cazuri au reticen i chiar fric la vederea acelor
(3).
Durerea sau disfuncia dintr-o zon a corpului
nostru sau a unui organ trebuie privite mergnd
napoi spre celulele care le constituie.
Terapia cu laser de mic putere (Low Level
Laser Therapy LLLT) se bazeaz pe principiul
modificrilor selective ale metabolismului celular,
n funcie de frecvena, doza i lungimea de und a
radiaiei laser (20).
n anul 1960, Theodore Maiman a produs prima
scnteie de lumin laser la Laboratoarele Huges din

ABSTRACT
In the recent years, the use of complementary and alternative methods in pediatrics has considerably
increased, especially for chronic conditions such as juvenile idiopathic arthritis, in which the pain may be a
significant problem. The increased interest in complementary and alternative medicine approaches for pain
symptoms in pediatric as well as in adult population has focused attention on questions of safety and efficacy.
The pursuit of safe, efficacious and cost-effective interventions for chronic pain in children suffering from
juvenile arthritis that may be used in place of, or in concert with conventional medical approaches is a worthy
goal that should be undertaken with the highest degree of scientific rigor. We need to think creatively and
differently in order to educate the medical profession as well as the general public to change their attitude.

INTRODUCTION
Complementary and alternative methods of therapy
have shown in the last decades a genuine global and
national rebirth, being already recognized by the
World Health Organization. These practices accomplish harmoniously the current therapeutic arsenal
with new meanings and possibilities, which together
with the processes of allopathic medicine, fill out
todays modern medicine (1).
In the recent years there has been a continuous
increase in the number of people asking for their
children complementary or alternative therapies in
order to improve their health, which led to the revision, updating and the development of many
methods and practices of traditional therapies.
Experts from the World Health Organization
points out that most people usually ask for the
global practice of traditional medicine, such as
Traditional Chinese Medicine (TCM), Ayurvedic
and other traditional healing practices (2).
Complementary and alternative methods have
won and are even gaining more land in the
medicine of children and adolescents.
Research on pathology and the frequency of use
of alternative and complementary therapies in
children and adolescents, reported that on the first
place are the chronic conditions, and in order of
frequency are neoplastic diseases, juvenile idiopathic arthritis and the cystic fibrosis of the pancreas,
which charge a percentage located within the
30-70% of cases (3).

Another aspect is that synthetic drugs often produce severe side effects, are becoming more expensive, burdening the budgets and the health insurance funds, and have a limited efficiency.
In recent decades there has been a continuous
increase in the number of children with chronic
arthritis who use alternative or complementary methods to improve their health. The most common
complementary and alternative therapies include
dietary supplements, special diets, stress management, prayer, massage, acupuncture, homeopathy,
healing music, laser biostimulation, natural painkillers applied locally, copper bracelets, etc. (9-13).
Physical and occupational therapy as part of the
management of chronic arthritis, prevents ankylosis,
maintain joint mobility and helps the patient to
adjust to everyday life. While pharmacological
therapy reduces pain and inflammation, only therapeutic exercise can restore full range of motion in
the joint area (14).
Exercise is absolutely essential in the patient
with chronic arthritis, because it helps maintain
joint mobility, muscle strength, regain lost motion
or muscle strength. At the same time, exercise helps
to improve the overall state of fitness and physical
activities such as to cover with clothes easier etc.
The physical therapist must actively participate
in the rehabilitation of disabled patient to help him
carry out these exercises during hospitalization, but
especially at home, where most activities must be
performed daily. Exercise program should be individualized for each patient.
Some water activities such as swimming or
exercises to mobilize the joints and muscles are
much easier, without special solicitation.
Swimming is favorite in children, because they
avoid the impact of blows to the spine or joints prone.
Exercises to maintain and increase joint mobility
include the following activities: passive mobilizations, active-assisted mobilizations, active exercises, isometric exercises, exercises to improve
the position, exercises to improve control, coordination and balance.
Rest, hot or cold compress applications and
exercises are usually recommended to reduce pain
and facilitate the movement.
One essential task of the medical doctor and the
physical therapist is working with the child and
family, on their awareness of the importance of the
physical therapy as part of overall management for
a favorable outcome with long-lasting remission of
the disease (9).
Tai Chi is a meditative exercise or exercises such
as mind-body technique with origins in Chinese martial arts; it uses slow, gentle movements with deep
breathing and relaxation techniques.

CONCLUSIONS
Complementary and alternative therapy methods
have in recent decades a true revival in the whole
world and filled up with new meanings and
possibilities the modern therapeutic arsenal of the
conventional medicine in the overall management
of pain in children with chronic arthritis.
