Factori predispozanţi
- a) sexul
După majoritatea statisticilor se poate aprecia că până la 15 ani ,
frecvenţa reumatismului articular acut este de două ori mai mare la fete
în raport cu băieţii , fără ca la adulţi să se manifeste o preponderenţă
semnificativă în ceea ce privește sexul . Numeroși reumatologi recunosc
totuși o creștere a incidenţei coreei și a reumatismului articular acut mai
pronunţată la femei , condiţiile favorabile de apariţie și agravare a bolii
fiind date de pubertate ,graviditate și tulburări de climă .
- b) vârsta
Reumatismul articular acut poate fi considerat o boală caracteristică a
copilului și adultului tânăr . Frecvenţa mare a bolii o găsim între 5 și a15
ani (asemănător cu infecţia streptococică ) fiind rară între 2 și 5 ani și
excepţională sub 2 ani . Tendinţa de apariţie a bolii chiar la copiii sub 4
ani sau la sugari a creat noţiunea de iniţierea reumatismului.
Coborârea vârstei medii de incidenţă a reumatismului articular acut
poate fi pusă în corelaţie cu factorii epidemiologici ai colectivităţilor
preșcolare , cămine și grădiniţe . Unle manifestări endocrine , debutând
cu perioada pubertăţii creează un nou vârf care se manifestă între 13 și
20 ani și deosebit de rar , la individul trecut de 30 de ani ca prim atac.
- c) ereditatea
Rolul predispoziţiei familiale și al eredităţii reprezintă subtilitatea . Wilson
și Schweitzer consideră factorul familial legat de o genă recesiză
autozomală . C.Ch. Dimitriu crede că o contribuţie importantă o au și
condiţiile identice de mediu reprezentând factori predispozanţi .
Factori favorizanţi
- a) mediul geografic
Este cunoscut faptul că frecvenţa reumatismului articular acut este
practic scăzută sau aproape absentă la tropice și în ţările calde , în
general , ca și în regiunile polare . Păunescu Podeanu și colaboratorii
confirmă faptul că schimbările meteorologice au o influenţă deosebită
asupra atacurilor acute reumatismale , în sensul creșterii puseelor odată
cu creșterea presiunii maxime .
- b) contagiozitatea
Reumatismul articulara acut se manifestă în aparenţă cu caracter de
contagiozitate deoarece în anumite colectivităţi există o adevărată
explozie de atacuri de reumatism articular acut

Infecţia streptococică faringiană cu streptococ beta hemolitic reprezintă
factorul declanșator în mai mult de 50 % dintre cazuri . În cazurile în
care nu se notează angine exprimate clinic ci doar seratura imunologică
a unei determinări streptococice ,evidenţierea acestui fenomen se face
prin intermediul titrului
În dinamica morfologică a fenomenelor tisulare din reumatismul articular
act se deosebesc patru faze distincte :

Predisposing factors
- a) sex
According to most statistics, it can be appreciated that up to 15 years, the
frequency of acute articular rheumatism is twice as high in girls as compared to
boys, without a significant preponderance of sex in adults. However, many
rheumatologists recognize an increase in the incidence of chorea and more
acute joint rheumatism.

aggravation of the disease being given by puberty, pregnancy and climate
disorders.
- b) age
Acute joint rheumatism can be considered a disease characteristic of the child
and young adult. The high frequency of the disease is found between 5 and 15

exceptional under 2 years. The tendency to develop the disease even in
children under 4 years old or in infants created the notion of initiating
rheumatism.
The lowering of the average age of incidence of acute articular rheumatism can
be correlated with the epidemiological factors of the preschools, homes and
kindergartens. Some endocrine manifestations, beginning with the period of
puberty, create a new peak that manifests between 13 and 20 years and very
rarely, in the individual past 30 years as the first attack.
- c) heredity
The role of family predisposition and heredity represents subtlety. Wilson and
Schweitzer consider the family factor linked to an autosomal recessive gene.
C.Ch. Dimitriu believes that an important contribution has the same
environmental conditions representing predisposing factors.
Facilitating factors
- a) the geographical environment
It is known that the frequency of acute articular rheumatism is practically low
or almost absent in the tropics and in warm countries, in general, as in polar
regions. Păunescu Podeanu and his colleagues confirm that the meteorological
changes have a special influence on the acute rheumatic attacks, in the sense
of the increase of the shoots with the increase of the maximum pressure.
- b) contagiousness
Acute articular rheumatism is apparently contagious because in some
communities there is a real explosion of attacks of acute articular rheumatism

Hygiene-dietary treatment
- a). rest
To prevent possible cardiac complications or to avoid aggravation of the
existing ones, it is necessary to rest in bed lasting 7-10 days. The return to the
previous activities is made progressively;
- b). diet
This will be evaluated according to the patient's weight and age. In children it is
recommended to increase the intake of fats and proteins, especially the
increase of egg consumption. Fluid intake should exceed urinary excretion. In
the acute set of acute joint rheumatism is recommended completely
deodorized regimen.
Vitamins A, B and C are also given.
Pharmaceutical treatment
- a). antibiotic (antistreptococcal)
Crystalline Penicillin G is recommended 1,000,000 - 2,000,000 U.I. daily, for ten
days to two weeks, injected intramuscularly four times a day. Benzantin
Penicillin (Moldamin) 1,000,000 can also be used. I.U. in single dose for adults
and 500,000 U.I. for the children.
The convalescence of acute articular rheumatism is long lasting, the period
conducive to the prophylaxis of acute articular rheumatism.
