Date generale și scopul studiului: Diabetul zaharat prin definiție este o alterare a
metabolismului glucidic. Cu toate acestea, alterările metabolice din cadrul diabetului zaharat
nu sunt limitate doar la metabolismul glucidic, ci se extind si la cel lipidic, proteic și chiar la
cel energetic. Respirometria de înaltă rezoluție este o metodă de evaluare de ultimă
generație a respirației mitocondriale atât în situații fiziologice cât și în patologie. Scopul
studiului a fost să evalueze literatura pentru a determina efectele respirației mitocondriale
asupra controlului glicemic.
Material și metodă: S-a efectuat o căutare în cadrul bazei de date Pubmed folosind
următoarele cuvinte cheie: mitocondrie, respirometrie de înaltă rezoluție, HbA1c, diabet.
Lucrările care au fost găsite în urma căutării au fost citite, procesate și discutate pentru a
analiza modificările respirației mitocondriale care duc la alterarea controlului glicemic și cum
această alterare poate influența bioenergetica celulară.
Rezultate: Există o asociere între reducerea valorilor parametrilor respirației mitocondriale
(fosforilarea oxidativă și capacitatea de transport de electroni) în cadrul subiecților cu diabet
zaharat (atât pentru oameni cât și animale). Aceste modificări se pot observa într-o varietate
de țesuturi examinate, cum ar fi: muscular (scheletic și cardiac), hepatic, renal, nervos și
chiar în cadrul celulelor sanguine.
Discuții: Se ridică întrebarea dacă alterarea respirației mitocondriale induce diabetul sau vice
versa. Cele două par a fi interconectate. Celulele beta pancreatice sunt dependente de buna
funcționare a mitocondriilor, iar o alterare a acesteia va duce la o reducere a secreției de

insulină. Pe de altă parte, controlul glicemic precar, evidențiat prin HbA1c crescută, se
asociază cu o reducere a parametrilor respirației mitocondriale. Studiile efectuate pe
animale în care diabetul este indus prin expunere la streptozotocină sau cele efectuate la
pacienți cu diabet zaharat tip 1, ar putea evidenția mai mult efectele controlului glicemic
precar asupra respirației mitocondriale deoarece disfuncția mitocondrială nu este o cauză
directă a debutului acestor tipuri de diabet.
Concluzii: Respirația mitocondrială influențează și este influențată de controlul glicemic și
deci este o țintă validă pentru tratamentul diabetul zaharat.

can be seen in a variety of examined tissues: muscle (skeletal and heart), liver, kidney,
neurons and even blood cells.
Discussions: There is the question if the reduction in mitochondrial respiration induced the
diabetic state or vice versa. The two seem to be intertwined. Pancreatic beta cells are
dependent on proper mitochondrial function therefor a loss of this function will lead to a
decrease in insulin secretion. On the other hand, poor glycemic control as evidenced by high
HbA1c, has been linked to a reduction in mitochondrial respiration parameters. Animal
studies using streptozotocin-induced diabetes or patients with type 1 diabetes, could shed
more light on the effects of poor glycemic control on mitochondrial respiration as
mitochondrial dysfunction isn’t a direct cause of the debut of this type of diabetes.
Conclusions: Mitochondrial respiration influences and is influenced by glycemic control and
therefor is a valid target for treatment of diabetes mellitus.

rezultatelor obținute au fost formulate soluții de gestionare a greutății corporale sub formă
de proceduri clare atât pentru persoanele cu diabet zaharat tip 2 cât și pentru persoane care
asociază supragreutate în absența diabetului zaharat de tip 2, pentru a crea premizele
aplicabilității practice la aceste categorii de pacienți. Exemple în acest sens sunt:
CounterWeight®-Plus și CounterWeight®-Plus – Pro800 Weight Loss Plan.
Sumarul rezultatelor obținute în urma studiilor care au testat eficiența și siguranța
intervenției CounterWeight®-Plus: 40% dintre participanți au reușit să scadă în greutate 15 kg
pe parcursul celor 12 luni. În studiul DiRECT 24% dintre participanți au obținut o scădere mai
mare de 15 kg în 12 luni, peste 50% au menținut o pierdere în greutate de peste 10 kg. Gradul
de remisie a diabetului este direct proporțională cu gradul de scădere în greutate: dacă la o
scădere de 5-10 kg, gradul de remisie este de 34%, la o scădere de peste 15 kg, gradul de
remisie poate ajunge până la 86%.
DESIGN: Identificarea, selectarea și obținerea acordul participanților.
Programul este structurat în 3 etape:
Etapa 1 - Etapa lichidiană: utilizarea unor înlocuitori de mese sub formă lichidiană (supe sau
shake-uri) pentru 12 săptămâni. Formulele nutriționale conțin cantități complete de macro și
micro nutrienți și adaos de fibre, pentru a oferi în medie 800 kcal/zi. Aportul suplimentar de
fibre este recomandat prin suplimente adăugate în supe și shake-uri, individualizat de către
dietetician.
Etapa 2 - Etapa de reintroducere a alimentelor solide: În această etapă cu durată de 12
săptămâni are loc reintroducerea treptată a meselor echilibrate nutrițional în porții
controlate.
Etapa 3 - Etapa de menținere a greutății: Programul de menținere a greutății obținute include
posibilitatea reintroducerii în cicluri repetitive a formulelor lichidiene utilizate în etapa 1,
atunci când dieteticianul consideră necesar. Activitatea fizică moderată 30-60 de minute pe
zi este de asemenea încurajată.
OBIECTIVE: Obținerea remisiei diabetului zaharat tip 2 dupa 6 luni de intervenție medicală
nutrițională.
MATERIAL ȘI METODĂ: Programul de intervenție medicală nutrițională desfășurat în cadrul
clinicii Consultmed în perioada ianuarie-iunie 2021 a inclus 6 pacienți cu diabet zaharat tip 2
nou diagnosticați, cu valori ale hemoglobinei glicozilate între 6,5 - 7,8% și IMC-ul cuprins între
34,2 - 54,8 kg/mp. Programul a urmat structura studiului DiRECT, iar la finalul celor 6 luni
pacienții au pierdut între 13-35 kg, ajungând la o hemoglobină glicozilată între 5,4-6%.

CONTEXT: Evaluation of the effectiveness and safety of nutritional medical intervention for
Type 2 Diabetes remission.
Diabetes remission is defined as achieving normoglycemia in the absence of active
pharmacological (antihyperglycemic drugs, immunosuppressive drugs) or surgical therapies
(ongoing procedures, such as repeated replacement of endoluminal devices) for at least 1
year.
The DiRECT study (Diabetes Remission Clinical Trial) confirmed the hypothesis that an
intensive program of structured nutritional intervention for weight loss, can bring major
benefits to patients with type 2 diabetes and excess weight. Following the results obtained,
body weight management solutions were formulated in the form of clear procedures, both
for people with type 2 diabetes and those associating excess weight in the absence of type 2
diabetes, in order to create the premises for practical applicability to these categories of
patients. Examples of this are: CounterWeight®-Plus and CounterWeight®-Plus – Pro800
Weight Loss Plan.
Summary of the results obtained from the studies that tested the effectiveness and safety of
the CounterWeight®-Plus intervention: 40% of the participants were able obtain a weightloss
of 15 kg during the 12 months. In the DiRECT study 24% of participants lost more than 15 kg
over 12 months and more than 50% maintained a weight loss of more than 10 kg. The degree
of diabetes remission is directly proportional to the degree of weight loss: if for a decrease of
5-10 kg, the degree of remission is 34%, with a decrease of more than 15 kg, the degree of
remission can reach up to 86% .
DESIGN: Identification, selection and obtaining the participants informed consent.
The program is structured in 3 stages:
Stage 1 - Liquid stage: using meal replacements in liquid form (soups or shakes) for 12 weeks.
Nutritional formulas contain complete amounts of macro- and micronutrients and added
fiber to provide an average of 800 kcal/day. Additional fiber intake is recommended through
supplements added to soups and shakes, individualized by the dietician.
Stage 2 - Solid food reintroduction stage: In this 12-week stage, the gradual reintroduction of
nutritionally balanced meals in controlled portions takes place.
Stage 3 - Weight maintenance stage: The weight maintenance program includes the
possibility of re-introducing in repetitive cycles the liquid formulas used in stage 1, when
deemed necessary by the dietician. Moderate physical activity 30-60 minutes a day is also
encouraged.
OBJECTIVE: Achieving remission of type 2 diabetes after 6 months of nutritional medical
intervention.
MATERIAL AND METHOD: The nutritional medical intervention program carried out in the
Consultmed clinic between January and June 2021 included 6 patients with newly diagnosed
type 2 diabetes, with A1c hemoglobin values between 6.5 - 7.8% and BMI between 34, 2 -
54.8 kg/m2. The program followed the structure of the DiRECT study protocol, and at the end

of the 6 months the patients lost between 13-35 kg, reaching a glycosylated hemoglobin
between 5.4-6%.
CONCLUSIONS: The participants who joined the nutritional intervention program for 6
months benefited from a significant reduction in HbA1c.

VAFV la pacienții cu T2D tratați timp de șase luni cu dapagliflozin. Valorile TyG au cele mai
bune performanțe de predicție pentru reducerea EFV, având cea mai mare sumă de
sensibilitate și specificitate la nivelul pragului de 0,904. AIP, CRR, VAI, indicele de conicitate,
VAFV, volumul atriului stâng și volumul ventriculului drept sunt biomarkeri valizi pentru
scăderea EFV după tratamentul cu dapagliflozin la pacienții cu diabet zaharat.

Background and aims: Dapagliflozin treatment proved to reduce the epicardial fat volume
(EFV) and total body weight in patients with type 2 diabetes (T2D). EVF and visceral abdominal
fat (VAFV) are not routinely evaluated in T2D. This study aims to identify biomarkers that
predict EFV and VAFV reduction after dapagliflozin treatment in patients with T2D.
Material and methods: In a prospective, observational, consecutive-case enrollment
scenario, 52 patients with T2D were initiated on dapagliflozin 10 mg q.d. as part of the
standard of care. At enrollment and after six months of dapagliflozin treatment, patients were
evaluated using cardiac ultrasonography, native computer tomography, transient liver
elastography, and metabolic lab tests. In addition, atherogenic index of plasma (AIP),
atherogenic coefficient (AC), triglyceride glucose index (TyG), cardiac risk ratio (CRR), conicity
index, and visceral abdominal index (VAI) were calculated.
Results and discussions: Higher AIP (r= 0.28; p= 0.04), CRR (r= 0.28; p= 0.04), and TyG (r= 0.32;
p= 0.01) are associated with more important reductions of the EFV. Higher reductions of the
EFV are associated with a lower conicity index (β= -0.29; p= 0.03), VAFV (β = -0.32; p= 0.02),
left atrium volume (β= -3.08; p= 0.003) and right ventricle diameter (β = -2.13; p= 0.04),
respectively, higher decrease in VAFV is associated with a higher conicity index (r= 0.31; p=
0.02) after six months of dapagliflozin treatment. Valid performance for predicting clinically
relevant decreases of EFV after dapagliflozin treatment was observed for AIP (AUROC= 0.903;
Youden= 0.732; p< 0.001), CRR (AUROC= 0.772; Youden= 0.595; p= 0.004), TyG (AUROC=
0.957; Youden= 0.904; p< 0.001) and VAI (AUROC= 0.898; Youden= 0.712; p< 0.001). Also, a
conicity index > 1.42 (AUROC= 0.702, Youden= 0.383; p< 0.001) and baseline VAFV> 54 cm3
(AUROC= 0.849, Youden= 0.695, p< 0.0001) had a valid performance for predicting clinically
relevant decrease of VAFV.
Conclusion: Higher initial EFV values are associated with more important reductions of EFV;
Respectively, higher initial VAFV values are associated with more important reductions of
VAFV in patients with T2D treated for six months with dapagliflozin. TyG values have the best
prediction performances for the reduction of EFV, having the highest sum of sensitivity and
specificity at the 0.904 threshold level. AIP, CRR, VAI, conicity index, VAFV, left atrium volume

and right ventricle volume are valid biomarkers for EFV decrease after dapagliflozin treatment
in patients with diabetes.

renal function, discontinuing SGLT2 inhibitors during periods of acute infections and when
using medications with nephrotoxic potential, adjusting other associated diuretic
medications, and educating the patient on a proper oral hydration regimen, to reduce the risk
of AKI.

Background: Diabetes mellitus (DM) is a major public health issue. In 2040, is expected reach
a 10.4% of world population, translating to 642 million patients with DM. Type 2 DM (T2DM)
is accounting for 90-95% of all cases. Chronic exposure to hyperglycaemia affects the
microvasculature, leading to complications such as diabetic kidneys disease, retinopathy or
diabetic neuropathy, with high impact on the quality of life.
Objective: The aim of this study is to assess the epidemiological data and risk factors for
microvascular complications (retinopathy, diabetic kidney disease and neuropathy) in
patients with T2DM.
Material and Methods: A retrospective study was conducted on patients admitted for acute
hyperglycaemia consecutively in NIDNMBD “N.C.Paulescu” from Bucharest between march
2021 and march 2022.
Results and Discussions: 733 patients with T2DM, with mean age of 64,1(DS=+/-10,9) years,
36,9% females and 63,1% males, preponderant from urban area (71,7%) were included. The
average duration of DM was 11,9 (DS=+/-2,4) years and the mean glycated haemoglobin was
9,8(DS=+/-2,5)%. The microvascular complications distribution was 68% for diabetic
peripheral neuropathy; 5,8% for autonomous diabetic neuropathy; 22,6% for retinopathy;
35,7% for diabetic kidney disease; the bundle - peripheral neuropathy, retinopathy and kidney
disease- was encountered in almost 11 % of T2DM patients.
Conclusions: The prevalence of microvascular complications in T2DM increases
proportionally with the duration of the pathology evolution. The association of a long
duration of T2DM alongside with high glycaemic level, high blood pressure and dyslipidemia,
risk factors for microvascular complications, are increasing the risk for their appearance.

Hipoglicemia reprezintă o adevărată provocare, atât pentru pacient, cât și pentru clinician,
prin diagnosticul și tratamentul acestuia. Deseori, valorile scăzute ale glicemiei pot ascunde
diverse defecte metabolice, uneori greu de evidențiat și tratat.
Una dintre acestea este reprezentată de sindromul Hirata, afecțiune extrem de rară, care
cunoaște însă o rată de diagnosticare în creștere, datorită evoluției investigațiilor clinice și
paraclinice actuale. Autoimunitatea este principalul factor etiopatogenic, ce se obiectivează
clinic prin prezența hipoglicemiilor severe și paraclinic, markerul specific este reprezentat de
anticorpi anti-insulină.
Vom prezenta un caz clinic, un pacient în vârstă de 60 de ani, fără antecedente patologice
personale semnificative, ce s-a prezentat în Spitalul de Urgență Constanța pentru repetate
căderi de la același nivel, cu un puternic impact asupra calității vieții, ce l-au îndrumat pe
acesta către serviciul de urgențe. A fost obiectivată hipoglicemia severă (28 mg/dl), motiv
pentru care a fost admis în Clinica de Diabet, Nutriție și Boli Metabolice, în vederea
evidențierii etiologiei episoadelor hipoglicemice și aplicarea sancțiunilor terapeutice
corespunzătoare.
Hypoglycemia represents a real challenge, both for the patient and for the clinician, through
its diagnosis and treatment. Often, low blood sugar values can hide various metabolic defects,
sometimes difficult to highlight and treat.
One of these is represented by Hirata syndrome, an extremely rare condition, which,
however, has an increasing diagnosis rate, due to the evolution of current clinical and
paraclinical investigations.
Autoimmunity is the main etiopathogenic factor, which is clinically objectified by the presence
of severe hypoglycemia and paraclinical ,the specific marker is represented by anti-insulin
antibodies.

Diabetes mellitus (DM) is a chronic, noncommunicable disease with its prevalence and
incidence continually growing worldwide. The acutization of an associated comorbidity can
negatively influence the prognosis of patients with DM.
We have reviewed the current international guidelines that focus on the glycemic targets of
the inpatients with DM admitted for an acute event such as stroke, acute coronary syndrome
(ACS), or acutization of the chronic kidney disease (CKD). There are no established
standardized recommendations.
The 2022 Standards of Medical Care in Diabetes (American Diabetes Association) recommend
glucose concentrations between 140-180 mg/dL for inpatients, with the individualization of
the glycemic targets. More stringent goals (110–140 mg/dL) should be recommended for
selected patients such as critically ill postsurgical patients or patients with cardiac surgery, as
long as they can achieve this target without significant hypoglycemia. For those where
frequent glucose monitoring or close nursing supervision is not feasible, a glucose range
between 180-250 mg/dL is accepted. Glycemic levels above 250 mg/dL are recommended for
terminally ill patients with a short life expectancy.
The Kidney Disease Improving Global Outcomes (KDIGO) 2020 Clinical Practice Guideline
proposed a range between 6.5-8% of the value of HbA1c with the individualization according
to the severity of CKD.
The management of hyperglycemia in acute stroke should evolve toward less rigorous
glycemic control. The European Stroke Organisation, as well as the American Stroke
Association, propose glycemic targets of 140-180 mg/dL in the first 24 hours.
According to the American College of Cardiology and the American Heart Association patients
with DM and ACS, hospitalized in the non-Intensive Care Unite setting, efforts should be
directed in maintaining plasma glucose levels <180 mg/dL. It is essential to point out that first-
line treatment for those with ST-elevation Myocardial Infarction is the reperfusion therapy
(percutaneous coronary intervention or fibrinolysis therapy).
In conclusion, the glycemic targets proposed by the current international guidelines are quite
similar, with slight differences depending on the disease. However, the glucose concentration
range must be individualized concordantly with the associated comorbidities as well as with
the socio-economic, cultural status, and the level of adherence to the treatment, without
significant risk of hypoglycemia or hyperglycemia.

Introducere: Agoniștii receptorilor de glucagon-like peptide-1 (AR GLP-1) și inhibitorii
cotransportorului-2 sodiu-glucoză (iSGLT2) și-au dovedit efectele benefice pleiotrope în
numeroase studii de outcome cardiovascular și de viață reală. Ca o consecință, ghidurile
actuale recomandă aceste două clase de antidiabetice precoce în managementul pacienților
cu diabet zaharat tip 2 (DZ2), în special în prezența altor comorbidități precum boala
cardiovasculară aterosclerotică (BCVATS), insuficiența cardiacă sau boala cronică de rinichi.
Totuși, deși acești agenți farmacologici sunt ușor disponibili în România, prescripția lor
rămâne scăzută. În acest context, am inițiat o colaborare interdisciplinară între
departamentele de diabetologie și cardiologie din Timișoara cu scopul de a îmbunătăți
managementul pacienților cu BCVATS și DZ2.
Material și metodă: În acest studiu transversal, non-intervențional, am înrolat 76 pacienți
diagnosticați anterior cu DZ2 și BCVATS care au fost îndrumați către Centrul de Diabet din
cadrul Spitalului Clinic Județean de Urgență „Pius Brînzeu” Timișoara, România de la Institutul
de Boli Cardiovasculare Timișoara (IBCV) între noiembrie 2020 și iunie 2022. Informații privind
vârsta, indicele de masă corporală (IMC), HbA1c, rata filtrării glomerulare estimate (RFGe),
comorbiditățile cardiovasculare și agenții antihiperglicemianți prescriși de diabetologul
pacienților anterior internării în IBCV au fost colectate din fișele acestora, alături de noile
recomandări de tratament care au fost individualizate conform caracteristicilor pacienților.
Rezultate și discuții: Conform așteptărilor, cel mai frecvent utilizat antidiabetic în lotul studiat
a fost metforminul, într-o proporție de 67,1% (n=51), urmat de derivații de sulfoniluree cu
31,5% (n=24), insulină cu 22,3% (n=17), inhibitorii DPP-4 și iSGLT2 cu 9,2% (n=7) și apoi AR
GLP-1 cu 6,5% (n=5). Cu toate acestea, proporția pacienților care au primit o recomandare de
agenți antidiabetici cu efecte de cardioprotecție a crescut în cadrul colaborării
interdisciplinare la 31,5% (n=24) pentru iSGLT2 și 22,3% (n=17) pentru AR GLP-1.
Concluzii: O colaborare interdisciplinară între departamentele de cardiologie și diabetologie
ar putea contribui la creșterea prescripției noilor agenți antidiabetici precum AR GLP-1 și
iSGLT2 care și-au dovedit efectele benefice privind mortalitatea, morbiditatea și calitatea
vieții pacienților cu DZ2 și BCVATS.

Introduction: Glucagon-like peptide-1 receptor agonists (GLP-1 RA) and sodium-glucose co-
transporter-2 inhibitors (SGLT2i) have proven their pleiotropic beneficial effects in a
multitude of cardiovascular outcome trials and real-life studies. As a consequence, current
guidelines are recommending these two classes of antidiabetic drugs early in the
management of patients with type 2 diabetes mellitus (T2DM), especially in the presence of
other comorbidities such as atherosclerotic cardiovascular disease (ASCVD), heart failure or
chronic kidney disease. However, although these pharmacological agents are widely available
in Romania, their prescription remains low. In this context, we have initiated an
interdisciplinary collaboration between the diabetology and cardiology departments in
Timișoara with the aim of improving the management of patients with ASCVD and T2DM.
Materials and methods: In this cross-sectional, non-interventional study, we enrolled 76
patients previously diagnosed with T2DM and ASCVD that have been referred to the Center
for Diabetes Treatment of the Emergency Hospital Timișoara, Romania from the Institute of
Cardiovascular Diseases Timișoara (IBCV) between November 2020 and June 2022. Data
regarding the patients’ age, body mass index (BMI), HbA1c, estimated glomerular filtration
rate (eGFR), cardiovascular comorbidities and the antihyperglycemic agents prescribed by
their diabetologists before the admittance to IBCV were collected from the patients’ medical
records alongside with the new recommendations of treatment that have been individualized
according to the patients’ characteristics.
Results and discussion: As expected, the most frequently used antidiabetic drug in the study
group was metformin with a proportion of 67.1% (n=51), followed by sulphonylureas with
31.5% (n=24), insulin with 22.3% (n=17), DPP-4 inhibitors and SGLT2i with 9.2% (n=7) and,
lastly, GLP-1 RA with 6.5% (n=5). However, the proportion of patients that had a
recommendation of cardioprotective antidiabetic drugs has increased during the
interdisciplinary collaboration, to a proportion of 31.5% (n=24) in SGLT2i and of 22.3% (n=17)
in GLP-1 RA.
Conclusion: An interdisciplinary collaboration between cardiology and diabetology
departments could contribute to increasing the prescription of novel antidiabetic drugs such
as GLP-1 RA and SGLT-2i that have proven to have beneficial effects regarding the mortality,
morbidity and quality of life of patients with T2DM and ASCVD.

Introducere: Prevalența obezității în rândul copiilor este mai mare ca oricând, după pandemia
SARS-COV2 [1]. Este nevoie de o evaluare mai completă a riscului cardiometabolic al acestor
copii [2]. Grosimea intimă-medie carotidiană (CIMT) este un marker al disfuncției endoteliale
și un predictor al progresiei procesului aterosclerotic [3,4]. Deoarece se corelează cu
majoritatea parametrilor clinici și paraclinici utilizați în managementul copiilor obezi, CIMT
poate completa evaluarea curentă a acestor copii cu risc crescut [5].
Obiectiv: De a arăta că CIMT poate fi utilizată în evaluarea aterosclerozei subclinice în cazul
copiilor obezi.
Materiale și metode: Au fost analizați 73 de pacienți cu vârste cuprinse între 6 și 18 ani. S-a
măsurat CIMT utilizând ecograful Aixplorer MACH 30 – software de măsurare automată a
CIMT automatic (SuperSonic Imagine, Aix-en-Provence, France). Subiecții au fost împărțiți în
3 grupuri, în funcție de severitatea excesului ponderal, apreciat prin IMC: obezi,
supraponderali și normoponderali. Studiul s-a concentrat pe impactul excesului adipos asupra
CIMT și de felul în care CIMT se corelează cu IMC, circumferința abdominală, stadiile de
dezvoltare pubertară Tanner și tensiunea arterială ca parametrii clinici, și de markeri sanguini
uzuali: profilul lipidic, trigliceridele, glicemia a jeun.
Rezultate: Excesul ponderal și adipozitatea abdominală în cazul copiilor se asociază cu
creșterea CIMT. Mai mult, circumferința abdominală și raportul trigliceride/HDL-colesterol
sunt predictori semnificativi ai valorilor CIMT. Valori crescute ale CIMT au fost detectate și în
cazul adolescenților cu dezvoltare Tanner IV și V (p=0.032). Subiecții care au prezentat valori
tensionale > percentila 95, au prezentat și valori mai mari ale CIMT, indiferent de valoarea
IMC-ului. CIMT a prezentat corelații semnificative cu întreg panelul lipidic. Deși HDL-c, LDL-c,
colesterolul total și trigliceridele s-au corelat cu CIMT, nu a fost și cazul glicemiei a jeun. Mai
mult, puterea corelațiilor dintre CIMT și non-HDL-c, raportul trigliceride/HDL-c și raportul
colesterol total/HDL-c a crescut direct proporțional cu valoarea IMC.
Concluzii: În cazul copiilor, severitatea excesului ponderal duce la creșterea valorilor CIMT.
Adipozitatea abdominală a copiilor obezi se corelează cu valorile CIMT. Adipozitatea
abdominală și raportul trigliceride/HDL-colesterol reprezintă predictori semnificativi ai
valorilor CIMT.
Creșterea valorilor tensionale se corelează cu creșterea CIMT, indiferent de IMC-ul
pacientului.
Toți parametrii sanguini evaluați au arătat grade variabile de corelație cu CIMT, cu excepția
glicemiei a jeun.

Abdominal adiposity of obese children is very reliably correlated to CIMT values. Waist
circumference and TG/HDL-c are significant predictors of CIMT.
High blood pressure is correlated to higher CIMT values, regardless of the patients’ BMI.
All evaluated blood parameters showed correlations to CIMT, except for fasting glucose.

ASCVD au asociat mai multe comorbidități, deși acestea sunt afecțiuni independente care
contribuie la includerea în categoria de risc foarte ridicat.
Concluzii: Prezența ASCVD poate crește gradul de conștientizare atât a pacienților, cât și a
medicului pentru un control cardio-metabolic mai bun, deși ținte adecvate ar trebui obținute
în timp util în ambele grupuri, deoarece prevenția primară este la fel de importantă.

VERY HIGH RISK IN TYPE 2 DIABETES MELLITUS PATIENTS: WHO SHOULD DO MORE?

Aim of the study: To characterize patients with Type 2 Diabetes Mellitus (T2DM) at very high
cardiovascular risk as regarding demographic parameters, metabolic control achievement,
presence of comorbidities, use of novel antidiabetic drugs.
Material and Method: In this cross-sectional study, we included consecutively admitted
patients with T2DM from one Department of a third-care center for Diabetes Mellitus during
Jan2021-March2022. Patients with ASCVD (atherosclerotic cardiovascular disease) and those
with risk factors considered very high risk according to 2019 ESC Guidelines on Diabetes, Pre-
Diabetes and Cardiovascular Diseases were selected and then divided into two groups. Clinical
and biological profiles were compared.
Results: From 549 T2DM patients with complete data, 436 (79.41%) were very high risk
patients of which 238 (54.58%) had documented ASCVD. Demographic characteristics, DM
characteristics, biologic profile, comorbidities and the use of novel antidiabetic drugs such as
glucagon-like peptide 1 receptor agonists and sodium-glucose cotransporter-2 inhibitors
were analyzed between the 2 groups and are synthetized in Table 1. Patients with ASCVD had
lower LDL-cholesterol (p<0.0001) and non-HDL-cholesterol (p<0.0001), triglycerides (p=0.02),
A1c hemoglobin (p=0.0056), and were fewer active smokers (p=0.028). However, they had a
higher age (p<0.0001) and associated more microvascular complications such as diabetic
neuropathy (p=0.024) and renal impairment(lower eGFR- p<0.001, higher levels of

albuminuria-p=0.0185). Heart failure was more frequent in the ASCVD group as well
(p<0.0001). 28 % of patients benefited from novel antidiabetic drugs in the ASCVD group vs
26.3 in the non-ASCVD group.
Discussions: Although far from targets, better lipid control was obtained for patients with
ASCVD, as well as lower A1C hemoglobin levels. However, patients with ASCVD associated
more comorbidities, although these are independent conditions that contribute to inclusion
in the very high risk category.
Conclusions: Presence of ASCVD may raise awareness from both the patients and the doctor
for a better cardio-metabolic control, although adequate targets should be obtained timely
in both groups, as primary prevention is equally important.

Introducere: Diabetul gestațional este una din patologiile care complică sarcina, având
consecințe imediate, dar și consecințe pe termen lung, atât asupra mamei cât și a fătului.
Prevalența diabetului gestațional este în creștere la nivel mondial, printre factorii de risc
numărându-se excesul ponderal al mamei, istoricul familial de diabet zaharat tip 2 la rudele
de gradul I, macrosomia la o sarcină anterioară , vârsta mamei de peste 35 de ani.
Obiective: Caracterizarea unui lot de paciente cu diabet gestațional urmărind existența unor
particularități față de datale din literatură.
Metode: Am realizat un studiu retrospectiv pe pacientele cu diabet gestațional care s-au
prezentat pentru evaluare în Ambulatoriu de Diabet zaharat la Spitalul Clinic Nicolae Malaxa
în perioada 2021-2022. Diagnosticul de diabet gestațional a fost pus folosind criteriile
American Diabetes Association (ADA) după utilizarea testului de toleranță la glucoză efectuat
cu 75g glucoza și măsurarea glicemiei a jeun și apoi la o oră și la două ore după ingestia
glucozei.
Rezultate: Lotul a cuprins 48 de paciente provenind majoritar din mediul urban (79%), cu
vârste cuprinse între 21 și 41 de ani, media vârstei fiind 31 de ani, 71 % din gravide au fost în
grupa de vârstă 25-35 de ani și doar 17% au avut vârsta peste 35 de ani. Din punct de vedere
al asocierii obezității sau diabetului zaharat pe linie maternă, doar 31% (15) din femei
prezentau această caracteristică, anterior sarcinii 54% (26) de gravide au avut un IMC>24.99
kg/mp, 37,5% prezentând un IMC normal, iar la momentul primei evaluări pentru diabet
gestațional creșterea excesivă în greutate s-a regăsit în procent de 18,75% în lotul de studiu,
iar din punct de vedere al IMC-ului 71% din gravide prezentau exces ponderal. Din cele 11
paciente multipare incluse în studiu niciuna nu avea istoric de diabet gestațional la sarcinile
anterioare, dar aproximativ jumătate din ele (5) au prezentat creștere ponderală excesivă la
o sarcină anterioară.
Concluzii: În studiul nostru majoritatea pacientelor au fost în grupa de vârstă 25-34 de ani și
au provenit din mediul urban. Doar o treime din paciente au prezentat antecedente
heredocolaterale de diabet zaharat tip 2 sau obezitate pe linie maternă, iar creșterea excesivă
în greutate în timpul sarcinii s-a regăsit la 18,75% din participante. Pe de altă parte, 54% din
femeile incluse în studiu aveau un IMC>24,99 anterior sarcinii. Sunt necesare studii

populaționale extinse pentru a caracteriza mai bine femeile cu diabet gestațional din țara
noastră și a descoperi particularități în ceea ce privește factorii de risc.

Introduction: Gestational diabetes is one of the pathologies that complicate pregnancy, with
immediate consequences, but also long-term consequences for both mother and fetus. The
prevalence of gestational diabetes is increasing worldwide, among the risk factors being the
mother's overweight, family history of type 2 diabetes in first-degree relatives, macrosomia
in a previous pregnancy, the mother's age over 35 years. Objectives: characterization of a
group of patients with gestational diabetes following the existence of particularities
compared to data in the literature. Methods: we conducted a retrospective study on patients
with gestational diabetes who presented for evaluation in the Diabetes Outpatient Clinic of
the Nicolae Malaxa Clinical Hospital in the period 2021-2022. The diagnosis of gestational
diabetes was made using the criteria of the American Diabetes Association (ADA) after using
the glucose tolerance test performed with 75 g of glucose and measuring fasting blood
glucose and then one and two hours after glucose ingestion. Results: The group included 48
patients, mostly from urban areas (79%), aged between 21 and 41 years, the average age
being 31 years, 71% of pregnant women were in the age group of 25-35 years and only 17%
were over 35 years old. From the point of view of the association of obesity or maternal
diabetes, only 31% (15) of women had this characteristic, before pregnancy 54% (26) of
pregnant women had a BMI> 24.99 kg / sqm, 37.5% presenting a normal BMI, and at the time
of the first evaluation for gestational diabetes, excessive weight gain was found in a
percentage of 18.75 in the study group, and in terms of BMI 71% of pregnant women had a
weight. Of the 11 multiparous patients included in the study, none had a history of gestational
diabetes in previous pregnancies, but half of them (5) had excessive weight gain in a previous
pregnancy.
Conclusions: In our study, most patients were in the 25-34 age group and came from urban
areas. Only one-third of patients had a hereditary history of type 2 diabetes or maternal
obesity, and excessive weight gain during pregnancy was found in 18. 75% of participants. On
the other hand, 54% of the women included in the study had a BMI> 24.99 before pregnancy.
The glucose tolerance test should be performed in all pregnant women regardless of the
presence or absence of risk factors for gestational diabetes. Extensive population studies are
needed to better characterize women with gestational diabetes in our country and to discover
particularities in terms of risk factors.

Aim of the study: The aim of this study is to assess the use of glucagon-like peptide 1 receptor
agonists (GLP1-Ra) in treatment of patients with type 2 diabetes mellitus (T2DM) and
atherosclerotic cardiovascular disease (ASCVD).
Material and methods: A retrospective study included 1562 consecutively admited patients
with T2DM in Paulescu Institute during 2020-2021 after their informed consent.

Results: From 1279 patients with full records, 1028 had T2DM, 567 patients of each had
ASCVD diagnosis (group 1) and 461 without ASCVD diagnosis (group 2). Demographic
characteristics, DM characteristics, DM complications, biologic profile, comorbidities and the
use of antihypertensive and novel antidiabetic drugs were analyzed between the 2 groups
and are synthetize in Table 1. Significant differences between the two groups were
encountered for age (p<0.001), DM duration (p<0.001), gender (p=0.05), smoking (p=0.007),
high blood pressure (p<0.001), atrial fibrilation (p<0.001), diabetic peripheral neuropathy
(p<0.001), retinopathy (p<0.001), chronic kidney disease (p<0.001), HDL-Cholesterol
(p=0.035), total cholesterol (p<0.001), eGFR (p<0.001), HbA1c (p=0.015) and UAC ratio
(p=0.002).
Discusions: Patients with T2DM and ASCVD reach harder metabolic control and,
subsequently, develop more DM complications and comorbidities, so GLP1-Ra administration
is favorable not only for treatment of ASCVD but, also, for targeting metabolic control (HbA1c,
weight or cardiovascular protection).

Dietele de excludere cu evitarea anumitor alimente cu conținut crescut de fibre (nuci întregi,
semințe, fructe/legume nepreparate termic) și/sau lactoză (produse lactate) sunt frecvent
preferate de pacienții cu BII pe fondul asocierii alimentelor respective cu apariția sau
agravarea simptomatologiei și a temerii pacienților de a nu declanșa activarea bolii, deși există
date limitate în literatura de specialitate care să susțină aceste recomandări.
Este important să se facă diferența între dietele care pot induce sau menține remisiunea
bolilor inflamatorii intestinale și dietele care ameliorează doar simptomele pacienților cu boli
inflamatorii intestinale.
Managementul pacienților cu BII, în contextul în care sunt disponibile date limitate sau
neconcludente privind intervențiile nutriționale, necesită o abordare multidisciplinară.
Optimizarea nutriției ar trebui să devină parte a măsurilor strategice de îngrijire de calitate
pentru pacienții cu BII.

Aim of the study: Diabetes Mellitus (DM) is one of the cardiorenal-metabolic disease and
represents a risk factor for Atherosclerotic Cardiovascular Disease (ASCVD). Both DM and
ASCVD are two growing chronic non-transmitable pandemic diseases worldwide. We aimed
to evaluate the ASCVD characteristics in patients with newly or previous diagnose of DM.
Material and Method: We included consecutively admitted inpatients with type 1 and type 2
diabetes mellitus in a Diabetes Mellitus Department from Paulescu National Institute for
Diabetes Mellitus during 2020-2021. Results: From 1562 patients, 1279 have a complete
clinical and paraclinical profile and 632 have ASCVD – 56 with newly diagnosed DM (group 1)
and, respectively, 576 with previously diagnosed DM (group 2). Demographic characteristics,
DM characteristics, DM complications, biologic profile, comorbidities and the use of new
antidiabetic drugs such as glucagon-like peptide 1 receptor agonists and sodium-glucose loop
2 inhibitors were analyzed between the 2 groups and are synthetize in Table 1. Discusions:
Significant differences between the two groups were encountered for diabetic peripheral
disease (p<0.001), vegetative neuropathy (p=0.032), retinopathy (p<0.001), chronic kidney
disease (p=0.003), high blood pressure (p=0.049), LDL-C (p=0.003), total cholesterol
(p=0.034), HDL-C (p=0.002), HbA1c (p<0.001) and for GLP1-Ra (p=0.002). Conclusions: ASCVD
is a great part of the cardiorenal-metabolic disease and is important to be emphasized and
actively screened in patients with DM.

trombocite și concentrația mare de AFP au fost factori de risc semnificativi ai HCC . Regresia
multivariată a hazardului proporțional Cox a arătat că obezitatea a fost un factor de risc
independent de HCC, cu un hazard ratio de 1,795 (95% CI:1,074-3,000; p=0,0260) când
25<IMC <30 și 3,210 (95%CI: 1,469-7,016, p=0,0035) când IMC <30 în comparație cu pacienții
cu IMC <18,5.
CONCLUZII: Obezitatea este un factor de risc independent de hepatocarcinogeneză în rândul
pacienților cu hepatită cronică C.

THE IMPACT OF CARDIOVASCULAR RISK FACTORS IN PATIENTS WITH DIABETES AND
COVID 19

Perspectives: High blood presure, obesity, dyslipidemia, ischemic heart disease, hystory of
acute myocardial infarction or stroke are listed as rick factors for bad outcomes of patients
with COVID 19 and diabetes. The aim of the study is to identify the prevalence of
cardiovascular risk factorts in patients with diabetes diagnosed with COVID 19, as well as their
impact on the viral infection evolution.
Methods: we retrospectively analyzed 260 COVID19 patients hospitalized between octomber
2020 and march 2021, divided according to diabetes presence. In the resulting two subgroups
there was compared the presence of cardiovascular risk factors. Parameters related to
inflammatory syndrome and prothrombotic risk, as well as the length of stay in hospital were
also monitored. The collected data were analyzed through IBM SPSS statistics software for
Windows.
Results: out of the 260 patients included in the study, 45 % (n=117) had assosiated diabetes.
Among Covid 19 and diabetes patients, the prevalence of cardiovascular risk factors were the
following: 81,20% (n=95) hypertension, 35,04% (n=92) obesity, 64,10 % (n=75) dyslipidemia,
11,97% (n=14) history of myocardial infarction and 35,90 % (n=42) ischemic heart disease,
compared to control subgroup to which the following values were objectified: 64,34 % (n=92),
16,08% (n=23), 31,46% (n=45), 6,29% (n=9), respectively 21,67% (n=31). The average number
of hospitalization days of those with diabetes was 14,18 ± 6,5 days, statistically significant
higher (p=0.008) than 11,52 ± 6,17 days for patients withoutout diabetes. Moreover, in
diabetic patients a corelation between time of hospitalization and a previous stroke was
highlighed (p=0.036). Regarding hositalization, dyslipidemia, hypertension, hystory of acute
myocardial infarction, stroke or obesity did not impact the viral infection evolution. Also, the
inflamatory syndrome was not significantly influenced by yhe presence of cardiovascular risk
factors, except obesity. We identified statistically significant correlations between D-dimers
(p=0.028), ESR (p=0,019), fibrinogen (p=0,004) and body ass iindex ≥ 30kg/dl. Ovesity was also
correlated with elevated feritin values (p=0.047).
Conclussions: the results suggest that the prevalence of cardiovascular risk factors is higher
in COVID 19 diagnosed patients who also associated diabetes. Although the statistical analysis
did not reveal any significant differences between cardiovascular risk factors and viral

infection evolution, the results must be interpreted in the context in which data on
cardiovascular pathologies treatment were limited.

cardiovasculare care sunt frecvent asociate cu DZ2 și am demonstrat că steatoza hepatică (p
< 0,001), CVD (p = 0,004) și IC (p < 0,001) au fost semnificativ statistic mai frecvente la subiecții
cu Scorul STOP-BANG ≥ 5, în timp ce nu a existat o semnificație statistică în ceea ce privește
frecvența fibrilației atriale (p = 0,733) și a dislipidemiei (p = 0,485). În plus, folosind regresia
logistică, am demonstrat că numai CA (OR [95% CI] 0,871 [0,824; 0,920]; p<0,001) și ASCVD
(OR [95% CI] 0,191 [0,068; 0,534]; p = 0,002 ) au fost factori de risc predictivi pentru riscul de
OSA moderat/severă.
Concluzii: Pacienții cu DZ2 prezintă un risc mai mare de a dezvolta OSA. Obezitatea
abdominală și prezența ASCVD sunt factori de risc importanți pentru OSA, fiind asociate cu
riscul de OSA moderată/severă.

Results: With regard to the studied anthropometrical parameters, we observed statistically
significant higher values of BMI, WC, WHtR, NC and NHR (p < 0.001) in the subjects with STOP-
BANG score ≥5 for both genders, the only parameter that did not reach a statistically
significant difference between the 2 categories being WHR. When we studied the association
of other metabolic and cardiovascular disorders which are frequently associated with T2DM
and proved that liver steatosis (p < 0.001), CVD (p = 0.004) and HF (p < 0.001) were statistically
significant more frequent in the subjects with STOP-BANG score ≥ 5, while there was no
statistically significance regarding the frequency of atrial fibrilation (p = 0.733) and
dyslipidemia (p = 0.485). Furthermore, using logistic regression, we demonstrated that only
waist circumference ( OR [95% CI] 0.871 [0.824; 0.920]; p<0.001) and established ASCVD (OR
[95% CI] 0.191 [0.068; 0.534]; p=0.002) were risk factors predictive for the risk of moderate-
to-severe OSA.
Conclusions: Patients with T2DM are at a higher risk of developing OSA. Abdominal obesity
and the presence of established ASCVD are important risk factors for OSA, being associated
with the risk of moderate-to severe OSA.

<30 mg/g creatinina; 37% ACR: 30-300 mg/g creatinina; 6% ACR> 300 mg/g creatinina. În ceea
ce privește controlul glicemic optim, am stabilit următoarele intervale: HbA1c<6%, HbA1c:6-
7% si HbA1c>7%, 6% dintre pacienți aveau HbA1c <6%, 27% dintre aceștia aveau HbA1C 6-7%
si 67% aveau HbA1c>7%. Din cei 532 de pacienți cu boală cronică de rinichi, doar 30 se aflau
sub tratament cu inhibitor de SGLT2. În ceea ce privește profilul cardiovascular, 146 de
pacienți au prezentat insuficiență cardiacă de diferite grade de severitate. În ceea ce privește
distribuția pe vârstă și sex: majoritatea pacienților 34% au fost în intervalul 60-69 de ani.
Durata lungă a diabetului zaharat se corelează cu riscul de complicații micro și
macrovasculare. În acest sens, 49% dintre pacienții cu insuficiență cardiacă aveau o durată a
diabetului zaharat între 16 și 30 de ani, urmați de 33% cu o durată între 5-15 ani, cel mai mic
procent fiind înregistrat în grupurile extreme cu o durată mai mică de 5 ani (9%) și peste 30
de ani (9%). Istoricul evenimentelor cardiovasculare majore, anume infarctul miocardic acut
și accidentul vascular cerebral, a fost găsit la 31% respectiv 18% dintre pacienții cu insuficiență
cardiacă. Majoritatea pacienților (68%) prezentau insuficiență cardiacă cu fracție de ejecție
păstrată. Tratamentul cu inhibitor de SGLT2 a fost găsit la momentul prezentării la 15 din 146
(10%) pacienți cu insuficiență cardiacă.
Concluzii: Majoritatea pacienților cu diabet zaharat de tip 2 nu au reușit să atingă obiectivele
terapeutice specifice. Combaterea inerției terapeutice urmată de introducerea inhibitorului
de SGLT2 cu protecție cardio-renala dovedită în rândul pacienților cu diabet zaharat
reprezinta parte integranta a managementului centrat pe pacient.

Epidemiology: There are currently 425 million people living with diabetes worldwide, and it
is estimated that by 2045 there will be 629 million people with diabetes. Nearly half of all
hyperglycemic deaths occur before the age of 70. SGLT2 inhibitor is a therapeutic option that
has proven cardiorenal protection by reducing mortality and morbidity, features documented
in a series of studies such as DECLARE-TIMI, CANVAS, CREDENCE and EMPA-REG OUTCOME.
The present study included 975 patients evaluated at the National Institute of Diabetes,
Nutrition and Metabolic Diseases “N.C. Paulescu” Bucharest, Romania, between March 1st
2021 and March 1st 2022. Out of 975 patients included in the study 532 had chronic kidney
disease. Regarding the glomerular filtration of the patients, the data are as follows: 50% of
patients had GFR> 90 ml/min/1.73m2; 30.8% had GFR 60-89 ml/min/1.73 m2;11.7% had GFR

45-59 ml/min/1.73m2; 4.5% had GFR 30-44 ml/min/1.72m2 and 3% GFR had 15-29
ml/min/1.73m2; In terms of albumin/creatinine ratio: 57% ACR <30 mg/g creatinine; 37%
ACR: 30-300 mg/g creatinine; 6% ACR> 300 mg/g creatinine. In terms of optimum glycemic
control, we set the intervals as:HbA1c: <6%, HbA1c: 6-7% and HbA1c> 7%. 6% of patients had
HbA1c <6%, 27% of them had HbA1c 6-7% and 67% had HbA1c>7%. Of the 532 patients with
chronic kidney disease, only 30 had a SGLT2-inhibitor in the treatment regimen. Regarding
the cardiovascular profile 146 patients had heart failure of varying degrees of severity. In
terms of age and sex distribution: most patients 34% are in the range of 60-69 years.The long
duration of diabetes correlates with the risk of micro and macrovascular complications.
Regarding this, 49% of patients with heart failure have a duration of diabetes between 16 and
30 years, followed by 33% with a duration between 5-15 years, the lowest percentage being
registration in extreme groups lasting less than 5 years (9%) and over 30 years (9%). The
history of major cardiovascular events, namely acute myocardial infarction and stroke was
found in 31% and 18% of patients with heart failure. Most of the patients 68% had heart
failure with preserved ejection fraction. SGLT2-inhibitor treatment was found at the time of
presentation in 15 of 146 (10%) patients with heart failure.
Conclusions: Most patients with type 2 diabetes failed to achieve specific therapeutic goals.
Thanks to the new therapeutic classes, such as SGLT2 inhibitors, these targets can be achieved
much more easily. Even if the results obtained in this study do not fully reflect the reality of
outpatient care services, combating therapeutic inertia followed by the introduction of the
SGLT2-inhibitor with proven cardiorenal protection among patients with diabetes and
significant cardiorenal impairment is the main goal of patient-centered treatment.

Materiale și metode: Studiul a inclus 152 de subiecți adulți de etnie romă, cu obezitate și
diabet zaharat de tip 1 sau tip 2; dintre aceștia, majoritatea au fost femei (n=79; 52%).
Datele necesare au fost obținute din foile de observație ale pacienților internați pe Secția de
Diabet, Nutriție și Boli Metabolice a Spitalului Clinic „Nicolae Malaxa” din București. Toți
participanții au semnat consimțământul informat. Factorii de risc cardiovascular analizați au
fost hipertensiunea arterială, obezitatea de tip central, fumatul și consumul de alcool.
Rezultate: Majoritatea pacienților incluși au avut DZ tip 2, doar 2 pacienți având DZ tip 1.
Caracteristicile generale ale subiecților au inclus: vârsta medie a subiecților de 56,02 ani,
IMC mediu de 36,93 (kg/m2), valoarea medie a tensiunii arteriale sistolice de 150,83 mmHg,
durata medie de evoluție a diabetului zaharat de 6,88 ani și valoarea medie a HbA1c de
9,59%. Valoarea medie a circumferinței abdominale a fost de 120,14 cm, ambele sexe
prezentând obezitate de tip central. Din antecedentele personale patologice, mai puțin de
10% dintre pacienți (n=15; 9,86%) au prezentat accident vascular cerebral, mai puțin de 15%
sindrom coronarian acut (n=22; 14,47%), în timp ce boala arterială periferică a fost prezentă
la 16 pacienți (10,52%). Aproximativ 80% dintre pacienți au fost hipertensivi (n=121;
79,62%). Făcând referire la datele demografice și comportamentele nocive, majoritatea
pacienților incluși locuiesc în mediul urban (n=89; 58,55%), doar o treime dintre pacienți
sunt consumatori cronici de etanol (n=51; 33,55%), în timp ce aproximativ 43% (n=65;
42,76%) sunt fumători. În ceea ce privește strategia terapeutică, 124 de pacienți au avut
metformin inclus în schema terapeutică, doar 5 au beneficiat de tratament cu iSGLT2, 12
subiecți au beneficiat de agoniști de receptor de GLP-1, 39 subiecți aveau incluse
sulfonilureicele în tratament, în timp ce 88 pacienți au asociat insulinoterapie.
Concluzii: Luând în considerare cele de mai sus, factorii de risc cardiovascular sunt frecvent
întâlniți în rândul populației de etnie romă cu diabet zaharat, prevalența hipertensiunii
arteriale, a obezității centrale, a fumatului și a consumului de alcool fiind destul de
semnificativă în studiul nostru, ce ar putea fi explicată prin factorii genetici,
comportamentele legate de sănătate, discriminare sau accesul limitat la serviciile de
sănătate sau educație. Din aceste motive, sunt necesare cercetări suplimentare în rândul
acestei populații pentru a crea o imagine de ansamblu asupra factorilor de risc
cardiovascular.

Background and objectives: Obesity in diabetic patients has shown increasing rates over the
last decades, leading to an enhanced risk of cardiovascular disease, which calls attention to
the need for improvement of these factors. However, it is of great interest the extent at which
these factors affect the Roma population. Accordingly, the aim of our study is to assess the
prevalence of cardiovascular risk factors in a Roma population.
Methods: The study included 152 Roma adult subjects with obesity and type 1 or type 2
diabetes mellitus, of which the majority were women (n=79; 52%). Data were obtained from
the patients’ observation hospital records in the Diabetes, Nutrition and Metabolic Diseases
Department from “Nicolae Malaxa” Clinical Hospital in Bucharest, Romania. All participants
signed the informed consent. The cardiovascular risk factors that were analyzed included
hypertension, central obesity, smoking and alcohol consumption.
Results: The majority of patients studied had T2DM, with only 2 patients with T1DM. General
characteristics of the subjects include: average age of 56,02 years, mean BMI of 36,93 (kg/m2),
mean systolic blood pressure of 150,83 mmHg, average duration of diabetes of 6,88 years and
mean A1C value of 9,59%. Mean value of the abdominal circumference was 120.14 cm, both
sexes having abdominal obesity. Less than 10% of the patients (n=15; 9.86%) associated
stroke, less than 15% an acute coronary syndrome (n=22; 14.47%), while peripheral artery
disease was present in 16 patients (10.52%). Approximately 80% of the patients were
hypertensive (n=121; 79.62%). The majority of patients included lived in the urban area (n=89;
58.55%). Only a third of the patients consumed alcohol (n=51; 33.55%), while approximately
43% (n=65; 42.76%) were smokers. Regarding the treatment approach, 124 patients had
Metformin included, only 5 used SGLT2i, 12 subjects benefited from GLP-1 RA, 39 used
sulphonylureas, while 88 had different insulin regimens.
Conclusions: Taking these findings into consideration, cardiovascular risk factors are common
among the Roma population with diabetes, the prevalence of hypertension, central obesity,
smoking and alcohol consumption being quite significant in our study, that might be related
to health behaviors, genetics, discrimination or limited health access or education. For these
reasons, further research needs to be performed.

According to the national report "Evaluation of the level of physical development and state
of health based on the balance medical examinations of children and young people from
school communities in urban and rural areas - school year 2019-2020", obesity due to non-
endocrine causes at the country level remains on the 2nd place in the list of chronic
conditions, and the dynamics of its prevalence are recorded with increasing values, from
preschool to the highschool grades. For the period 2011-2020 in the urban environment the
prevalence of obesity increased from 3.05% to 4.4%, and in the rural environment it is much
more pronounced from 1.18% to 3.4% (2.9 times). The number of new cases of obesity
registered in 2020 in children 0-19 years old is 6731 with an incidence rate of 165,4 %000 and
represents 18.9% of the total cases per country. Diabetes in children 0-19 years has an
increasing trend in the period 2016-2020, so that in 2020 867 new cases were declared,
representing approximately 1.4% of the total new cases in the country, with an incidence rate
of 21.3 %000 place. These data should rase a question mark to all concerned with the health
and normal physical development of children.
The project "Access to health, access to life" took place between April and May 2022 in 6
schools in Iași County, both in rural and urban areas.
The aim of the project was to raise awareness of the effects of excessive consumption of
products with a high amount of sugar, to offer healthier options for the school packs, but also
to assess nutritional status.

The team made up of dietetician, medical assistant and representatives of the association
spoke to the children about diabetes, nutrition and physical activity. Blood glucose levels and
anthropometric measurements were carried out.
The information obtained confirmed the hypothesis supported by specialized literature,
children consuming increased amounts of products rich in sugar (carbonated juices, energy
drinks, croissants, chocolate, etc.).
321 children were evaluated and the results demostrated that overweight predominates
among boys with a percentage of 41.9% compared to 23.9% in girls with a higher incidence in
7-14 years category. A lower weight than recommended for their age and height was found
at 5.29% children.
Blood sugars ranged from 65 to 130 mg/dl, with a mean of 88.5 mg/dl. Higher values ( >100
mg/dl) appeared among children who consumed either sugary drinks (juice, energy drinks) or
products with a high glycemic index (chocolate, corn, chips) the period before testing.

Prevalența Diabetului Zaharat a crescut în ultimele decenii. Acest studiu a fost conceput
pentru a determina retinopatia, neuropatia, nefropatia, hipertensiunea arterială și
hiperlipidemia și interdependența acestora la pacienții diabetici nou diagnosticați. În acest
studiu, 103 de pacienți consecutivi nou diagnosticați au fost evaluați și au fost efectuate teste
de screening pentru retinopatie, neuropatie, nefropatie, hipertensiune arterială și
hiperlipidemie.
Material și metode: Design cross secțional, ce a inclus pacienți diagnosticați cu Diabet Zaharat
tip 2, prezentați în ambulatoriul de specialitate Diabet, Nutriție și Boli metabolice al Spitalului

Clinic Nicolae Malaxa, în perioada Ianuarie 2021 - Decembrie 2021. Fiecare pacient a fost
evaluat pentru prezența complicațiilor cronice-hipertensiune arterială, dislipidemie,
parametrii antropometrici ( înalțime , greutate, circomferințe-
gat/abdominal/peritrohanterian, precum și evaluarea tensiunii arteriale în clinostatism și
ortostatism. A fost notat și istoricul familial de Diabet și Obezitate.
Criterii de includere:
- Adulți de peste 18 ani,
- Prezența tuturor evaluărilor necesare pentru determinarea complicațiilor.
Criterii de exludere:
- Sarcina,
- Tratament cu corticosteroizi,

Au fost evaluați 103 pacienți 50 de bărbați (48.5%) și 53 de femei (51.5%). Media de vârstă a
fost de 57,13 +/- 13. 53 ani; în grupa de vârstă 61-70 ani (31.1%) au fost cei mai mulți.

The prevalence of diabetes has increased in recent decades. This study was designed to

determine retinopathy, neuropathy, nephropathy, hypertension and hyperlipidemia and
their interdependence in newly diagnosed diabetic patients. In this study, 103 consecutive
newly diagnosed patients were evaluated and screening tests were performed for
retinopathy, neuropathy, nephropathy, hypertension and hyperlipidemia.
Material and methods: Cross-sectional design, which included patients diagnosed with Type
2 Diabetes, presented in the Diabetes, Nutrition and Metabolic Diseases Outpatient Clinic of
Nicolae Malaxa Clinical Hospital, between January 2021 - December 2021. Each patient was
evaluated for the presence of chronic complications- hypertension, dyslipidemia,
anthropometric parameters (height, weight, neck / abdominal / peritrohanterian
circumferences, as well as the evaluation of blood pressure in clinostatism and orthostatism.
The family history of Diabetes and Obesity was also noted.
Inclusion criteria:
- Adults over 18 years
- The presence of all the necessary evaluations to determine the complications

Concluzii: Deși există numeroase corelații semnificative statistic între parametrii biochimici
la internare analizați în studiul nostru și severitatea infecției cu SARS-CoV-2, este necesară
continuarea cercetării pentru a stabili markeri de prognostic mai eficienți.

Premises and Objectives: In March 2020, WHO declared the pandemic of COVID-19.
Understanding this new pathology is of major interest, so far over 530 million cases and over
6 million deaths being confirmed. Once infected, the prognosis is still uncertain, researchers
all over the world attempting to link certain evolutions of the disease to particular markers or
characteristics pertaining to the group of patients which presented a more unfavorable
outcome. The aim of this study is to assess if specific laboratory investigations might predict
certain evolutions and outcomes of SARS-CoV-2 infection in patients with diabetes mellitus
(DM).
Materials and Methods: We conducted a retrospective study that included 238 patients
infected with SARS-CoV-2 and hospitalised in the “Nicolae Malaxa’’ Clinical Hospital - Clinical
Department of Diabetes, Nutrition and Metabolic Diseases, during November 2020 - March
2022. There were two types of data analysed: data regarding the severity of the infection
(dyspnea as main symptom at admission, number of hospitalisation days, necessity of cortico-
, antiviral-, immunomodulatory- or oxygen (O2) therapy, necessity of mechanical ventilation,
ICU admission, exitus) and data regarding biochemical parameters measured at admission.
For statistical data processing we used the IBM SPSS Statistics program.
Results and Discussions: The group consisted of patients with type 2 DM (n=221) and type 1
(n=16). The sex ratio was approximately equal: 55.1% women and 44.9% men. The average
age was 64.895 +/- 13.7 years, and the average onset of DM was 10.141 +/- 9.29 years ago.
Only 7.6% of hospitalised patients were vaccinated (n=18). The evolution of COVID-19 was
mild in 16% of cases, moderate in 58%, moderate-severe in 6.7% and severe in 18.9%,
respectively. The frequency of exitus in our ward was 8.4% (n=20). At admission, the
frequency of diabetic patients who presented with decreased eGFR was 48.3%, with high AST
42%, high ALT 42.4%, high CK-MB 47.1%, positive troponine 0.8%, high fibrinogen 77.3%, high
D-Dimers 59.2%, high CRP 79%, high ESR 72.7%, high procalcitonin 87.4%. Although we would
have expected an association between mechanical ventilation or ICU admission and necessity
of O2 therapy at discharge, in the current study no statistically significant correlation was
found between them (p=0.958, p=0.061). Likewise, there was no significant association

between the number of hospitalisation days and the severity of COVID-19 (p=0.271). There
were little to no significant correlations regarding complete blood count, electrolyte blood
tests, glycemic profile and the severity parameters used in our study. D-Dimers were
significantly associated with necessity of O2 at discharge only (p=0.01). The parameters that
reached the proper level of signification when associated with the most severity criteria were:
eGFR, AST, lipidic profile, CK, Ferritin, ESR, LDH and Procalcitonine.
Conclusions: Although there are plenty of statistically significant correlations between
biochemical parameters at admission and severity of SARS-CoV-2 infection, more research is
needed in order to establish better markers of prognosis.

Introducere: Sistemul de monitorizare continuă a glicemiei (CGMS) reprezintă una dintre cele
mai mari inovații în domeniul tehnologiei medicale și un imens ajutor în managementul de zi
cu zi al pacienților cu diabet zaharat. Cu toate că volumul de informații provenit din afișarea
nivelului glicemiei la fiecare 5 minute, precum și alarmele aferente variațiilor glicemice poate
fi copleșitor pentru pacienți, vizualizarea evoluției complete a profilului glicemic și
posibilitatea unui control mult mai strict, înclină considerabil balanța de partea monitorizării
continue. Ne propunem să analizăm diferite situații din viața unei paciente cu diabet zaharat
care prezintă variabilitate glicemică, precum și soluțiile către care ne-am orientat cu ajutorul
senzorului de glicemie.
Caz clinic: Am evaluat prin CGMS o pacientă în vârstă de 26 de ani, diagnosticată cu diabet
zaharat tip 1 de 11 ani, în tratament intensiv modern prin pompa de insulină (insulina aspart),
fără complicații cronice asociate, care se prezintă pentru variabilitate glicemică importantă,
simptome de poliurie, polidipsie alternând cu senzație intensă de foame însoțită de
transpirații reci. Afirmativ, în ultimele 3 luni nu au fost efectuate posturi alimentare și nu au
fost modificate ratele bazale orare din pompa de insulină. Din antecedentele personale
patologice reținem tiroidita autoimună cu hipotiroidism în tratament cu Euthyrox 25
micrograme, diagnosticată la momentul debutului diabetului zaharat, actual cu funcție
tiroidiană în limite normale.
În urma analizei rapoartelor CGMS am observat o variabilitate glicemică importantă, cu
creșteri ale valorilor glicemice pe perioada dimineții. De asemenea, reținem existența
episoadelor de scăderi marcate ale glicemiei sau de hiperglicemie corespunzătoare meselor

la care nu se respectă raportul dintre carbohidrații cu absorbție rapidă și carbohidrații cu
absorbție lentă sau meselor cu conținut ridicat de grăsimi.
S-au efectuat posturi alimentare pe o perioadă de 2 săptămâni, pe diferite intervale orare, cu
ajustarea ratelor bazale și cu crearea a 2 profile diferite de rată bazală, unul pe timpul
săptămânii și unul pe weekend. Ulterior, corespunzător evoluției profilului glicemic din
perioada post-prandială a fost înlocuit analogul de insulină cu acțiune rapidă cu un analog de
insulină cu acțiune ultrarapidă (lispro). Totodată, a fost ajustată modalitatea de administrare
a bolusului prandial în funcție de proveniența hidraților de carbon, utilizându-se funcția de
bolus dual a pompei de insulină cu experimentarea livrării a diferite procentaje din acest tip
de bolus.
Concluzii: Prin înlocuirea analogului de insulină cu acțiune rapidă cu analog de insulină cu
acțiune ultrarapidă și prin utilizarea bolusului dual a fost obținută o diminuare a variabilității
glicemice, cu mai puține episoade de hipoglicemie apărute imediat post-prandial urmate de
hiperglicemie tardivă.

Introduction: Continuous glucose monitoring systems (CGMS) represent one of the biggest
innovations in the domain of medical technology and a major help in daily management of
patients with diabetes. Although the volume of information from the display of the blood
glucose level every 5 minutes, as well as the alarms related to blood glucose variations can be
overwhelming for patients, the visualization of the complete evolution of the blood glucose
profile and the possibility of a much stricter control, considerably tilts the balance in favor of
continuous monitoring. We propose to analyze different situations in the life of a patient with
diabetes who presents glycemic variability, as well as the solutions we have aimed for with
the help of the glucose sensor.
Clinical case: We evaluated through CGMS a 26-year-old patient, diagnosed with type 1
diabetes for 11 years, in modern intensive treatment with an insulin pump (insulin aspart),
without associated chronic complications, who presented for important glycemic variability,
symptoms of polyuria, polydipsia alternating with intense feeling of hunger accompanied by
cold sweats. Affirmatively, in the last 3 months no fasts were performed and hourly basal
rates in the insulin pump were not changed. From the personal history, we note autoimmune
thyroiditis with hypothyroidism in treatment with levothyroxine 25 micrograms, diagnosed at
the time of the onset of diabetes, currently with thyroid function within normal limits.

Following the analysis of the CGMS reports, we observed important glycemic variability, with
increases in glycemic values during morning. We also observed the existence of episodes of
marked decreases in blood sugar or hyperglycemia corresponding to meals where the ratio
between fast-absorbing carbohydrates and slow-absorbing carbohydrates is not respected or
meals with a high fat content
Food fasts were performed over a period of 2 weeks, at different time intervals, with the
adjustment of basal rates and the creation of 2 different basal rate profiles, one during the
week and one for the weekend. Later, according to the evolution of the glycemic profile in
the postprandial period, the fast-acting insulin analogue was replaced by an ultra-fast-acting
insulin analogue (lispro). At the same time, the method of administering the prandial bolus
was adjusted according to the source of carbohydrates, using the dual bolus function of the
insulin pump and experimenting with the delivery of different percentages of this type of
bolus.
Conclusions: By replacing the fast-acting insulin analog with an ultra-fast-acting insulin analog
and by using the dual bolus, a decrease in glycemic variability was achieved, with fewer
episodes of hypoglycemia occurring immediately post-prandial followed by late
hyperglycemia.
Introducere: Boala diabetică renală afectează aproximativ 30% dintre pacienții cu diabet,
procentul fiind în creștere datorită incidenței globale crescute a diabetului. Acidul uric seric,
produsul final al metabolismului purinic, generat la nivel hepatic sub acțiunea xantin oxidazei,
reprezintă un factor de risc important implicat în patogeneza bolii renale diabetice și,
totodată, unul dintre noile ținte terapeutice.
Obiective: Prezentăm cazul unei paciente în vârstă, având diagnosticul de diabet zaharat tip
2 în evoluție de 8 ani, care asociază patologie cardiovasculară și renală importantă.
Prezentare caz: Pacientă în vârstă de 65 ani, cunoscută cu diabet zaharat tip 2 neglijat
terapeutic, HTA grad III grup de risc foarte înalt, AVC în antecedente, BCR stadiul G3aA2,
dislipidemie aterogenă în tratament, obezitate grad I (IMC= 33.29 kg/m2), s-a prezentat
pentru durere intensă la nivelul halucelui drept, instalată brusc în cursul nopții, însoțită de
eritem și edem local. Din istoricul personal reținem că pacienta a mai avut un episod similar
în urma cu aproximativ 7 luni, pentru care s-a recomandat tratament pe care pacienta nu l-a
continuat la domiciliu. Tabloul clinic este sugestiv pentru un atac de gută recurent, diagnostic
confirmat în urma analizelor de laborator (acid uric= 15 mg/dl, glicemie 350= mg/dl, HbA1c=
14.9%, sindrom inflamator prezent important, leucocitoză, RAC= 245 mg/g, eRFG= 43
ml/min/1.73 m2). Comparând analizele actuale cu cele efectuate anterior, observăm

deteriorarea progresivă a funcției renale și menținerea unui nivel crescut al acidului uric. S-a
inițiat tratament cu Colchicină și insulinoterapie.
Concluzii: Nivelul constant crescut al acidului uric la pacienții cu diabet zaharat tip 2 crește
riscul de apariție a bolii renale cronice diabetice. Excreția acidului uric se relizează
predominant la nivel renal (70%,) în tubul proximal. Evoluția acestor tip de pacienți este
influențată atât de variabile care pot fi controlate therapeutic (glicemie, hipertensiune,
inflamație, dislipidemie), cât și de variabile ce nu pot fi modificate (vârstă, genetică, durata
diabetului, gen).
Introduction: Diabetic kidney disease affects about 30 % of diabetic patients, the absolute
number is growing as a consequence of rising incidence of diabetes worldwide. Serum uric
acid is the final waste product of purine metabolism, generated in the liver by the action of
enzyme xanthine oxidase, is an important risk factor involved in the pathogenesis of diabetic
kidney disease and also one of the new therapeutic targets.
Objectives: We present the case of an elderly patient, diagnosed with type 2 diabetes in 8
years of evolution, who associates important cardiovascular and renal pathology.
Case presentation: 65-year-old patient, known to be therapeutically neglected type 2
diabetes, very high risk group HTA grade III, previous stroke, BCR stage G3aA2, atherogenic
dyslipidemia in treatment, obesity grade I (BMI = 33.29 kg / m2), presented with severe pain
in the right toe, suddenly installed during the night, accompanied by erythema and local
edema. From personal history we note that the patient had a similar episode about 7 months
ago, for which a treatment was recommended for the care of the patient did not continue at
home. The clinical picture is suggestive for a recurrent gout attack, diagnosis confirmed by
laboratory tests (uric acid = 15 mg / dl, blood sugar 350 = mg / dl, HbA1c = 14.9%, significant
inflammatory syndrome present, leukocytosis, RAC = 245 mg / g, eRFG = 43 ml / min / 1.73
m2). Comparing the current tests with the previous ones, we notice the progressive
deterioration of the renal function and the maintenance of an increased level of uric acid.
Colchicine treatment and insulin therapy have been initiated.
Conclusions: Constantly elevated uric acid levels in patients with type 2 diabetes increase the
risk of developing chronic diabetic kidney disease. Uric acid excretion is predominantly renal
(70%) in the proximal tubule. The evolution of these types of patients is influenced both by
variables that can be controlled therapeutically (blood sugar, hypertension, inflammation,
dyslipidemia), and by variables that cannot be changed (age, genetics, duration of diabetes).

Premises and objectives: Type 2 diabetes mellitus(T2DM) is one of the most relevant risk
factors for heart failure(HF), the prevalence of which is increasing worldwide. Excess body
weight, a burgeoning problem worldwide, is a major risk factor for cardiovascular disease
Within this escalating healthcare problem of monumental proportions, obesity-associated
T2DM accounts for 90–95% of all diagnosed diabetes in adults. In fact, diabetes and insulin
resistance are powerful predictors of cardiovascular morbidity and mortality, and each one is
an independent risk factor for death in patients with HF.
Material and Method: The current study is a transversal observational retrospective one, that
included 197 patients admitted to NIDNMD “N.C. Paulescu”, Bucharest, Romania during the
period 01.01.2019-01.03.2022, who presented both T2DM and HF. All the patients were
assessed regarding anthropometric indices, cardiovascular risk factors, blood count,
biochemical parameters and treatament
Results and discussions: Mean age was 66,64±9.4 years, 68% were men. Mean HbA1c was
8.02 ± 2,07%. 118 patients (60%) were diagnosed with obesity . Patients with obesity are 60%
men (a significantly lower percentage than in non obese goup (80%), p=0,002) and have
higher total cholesterol values (169.41 ± 43,96 mg/dl vs 157,8 ± 35,6 mg/dl, p=0.23). Obesity
is also asociated with chronic kidney disease (CKD) (41% vs 24%, p=0,012) and with a more
frequent utilisation of beta blockers (94% vs 84%, p=0,05), angiotensin-converting enzyme
(ACE) inhibitors( 90% vs 79%, p=0,032) and sartans(94% vs 83%, p=0,009)
Conclusion: In our study, the obesity was asociated with CKD, which worsens the prognosis
of the obese patients. It was previousely demonstrated that when HF and CKD are both
present, these two chronic diseases tend to accelerate each other’s progression.
On the other hand, obese patients received more cardiovascular drugs than their non-obese
counterparts. Polypharmacy should be an area of serious concern in patients with T2DM and
HF, since it can lead to low adherence to treatment, as well as to a decreased quality of life.

Introducere: Obținerea și menținerea unui control metabolic adecvat reprezintă una dintre
cele mai importante etape ale managementului unei persoane cu diabet zaharat (DZ) tip 1.
Metodele tradiționale de evaluare a controlului glicemic prezintă unele limitări și pot fi
influențate de o serie de factori, de aceea monitorizarea continuă a glicemiei vine în sprijinul
persoanelor cu diabet, contribuind la identificarea și ameliorarea variabilității glicemice.
Caz 1:
Pacient M.G. în vârstă de 35 de ani, normoponderal, diagnosticat cu DZ tip 1 în urmă cu 29 de
ani, actual în tratament intensiv modern cu insulină NovoRapid prin pompă de insulină, cu o
rată bazală de 25,1 UI/24 h și bolusuri prandiale de 1,5 UI pentru 10 g de carbohidrați.
Valoarea medie glicemică din cadrul zilelor analizate a fost de 182 mg/dl, iar valoare HbA1c
(7,7%) fiind peste valoare țintă. Procentul de timp petrecut în intervalul glicemic optim de 70-
180 mg/dl este de doar 29%, cu un procent semnificativ de timp (60%) petrecut în intervalul
glicemic 180-250 mg/dl. De asemenea, există și un procent important (10%) de hiperglicemie
severă în care valorile glicemice au fost peste 250 mg/dl. Valori glicemice între 70-54 mg/dl și
respectiv mai mici de 54 mg/dl s-au înregistrat în mai puțin de 1% din timp.
Pacientul prezintă variabilitate glicemică nesemnificativă, coeficientul de variație având o
valoare de 28,1% (valoarea cut-off de definire a variabilității glicemice fiind CV≥36%). De
asemenea, pe baza valorii MAGE:92,50 putem spune că nu există excursii glicemice
importante în cadrul aceleași zile, și nici variații importante în cadrul a două zile diferite
(MODD:52,99).
Caz 2:
Pacienta R.A. în vârstă de 20 ani, normoponderală, diagnosticată cu DZ tip 1 de 11 ani, aflată
actual în tratament intensiv modern cu insulină Fiasp prin pompă de insulină cu o rată bazală
de 22,8 UI/24 de ore și bolusuri prandiale de 1,4 UI pentru 10 g de hidrați de carbon
consumați.
Valoarea medie glicemică din cadrul zilelor analizate a fost de 162 mg/dl, iar valoare HbA1c
(7,75%) fiind peste valoare țintă. Procentul de timp pe care pacienta l-a petrecut în intervalul
optim glicemic (70-180mg/dl) a fost de 62%. De asemenea, pacienta a avut și valori glicemice
între 70-54 mg/dl în 4% din timp şi valori mai mici de 54 mg/dl în 1% din timp. În privința
hiperglicemiilor, 25% din timp, pacienta a înregistrat valori peste 180mg/dl, dar sub 250 mg/dl
şi un procent de 8% din timp a fost cu excursii glicemice importante de peste 250 mg/dl.
Pacienta prezintă variabilitate glicemică importantă, coeficientul de variație având o valoare
de 37,3%. De asemenea, pe baza valorii MAGE:143,45 putem spune că există excursii
glicemice importante în cadrul aceleași zile, și variații importante în cadrul a două zile diferite
(MODD:57.96).
Concluzii: În cadrul acestui studiu am prezentat faptul că metodele tradiționale de evaluare a
controlului glicemic (automonitorizarea glicemică și HbA1c) nu sunt suficiente pentru a
cuantifica variabilitatea glicemică și pentru a evalua controlul metabolic al pacientului.

Introduction: Obtaining and maintaining an adequate metabolic control is one of the most
important steps in the management of a patient suffering from diabetes mellitus (DM) type
1. Traditional methods of assessing glycemic control have some limitations and can be
influenced by a number of factors, therefore cotinuously monitoring the values can help the
people suffering from diabetes, contributing to the identification and improvement of
glycemic variability.
Case 1
Patient M.G. 35 years old, normal weight, diagnosed with DM type 1 with 29 years ago,
currently in modern intensive treatment with NovoRapid insulin via insulin pump, with a basal
rate of 25.1 IU/24 h and prandial boluses of 1.5 IU per 10 g of carbohydrates. The average
glycemic value during the analyzed days was 182 mg/dl, and the HbA1c value (7.7%) was
above the target value. The percentage of time spent in the optimal glycemic range of 70-180
mg/dl is only 29%, with a significant percentage of time (60%) spent in the glycemic range of
180-250 mg/dl. There is also a significant percentage (10%) of severe hyperglycemia in which
glycemic values were above 250 mg/dl. Glycemic values between 70-54 mg/dl and less than
54 mg/dl occurred less than 1% of the time. The patient shows insignificant glycemic
variability, the coefficient of variation having a value of 28.1% (the cut-off value for defining
glycemic variability is CV≥36%). Also, based on the value of MAGE:92.50 we can say that there
are no important glycemic variations within the same day, nor important variations within
two different days (MODD:52.99).
Case 2

This study shows the fact that the traditional methods of evaluating glycemic control

Introducere: Obezitatea infantilă este definită printr-o percentilă mai mare de 95 a greutății
și înălțimii conforme vârstei, în timp ce supraponderea se încadrează între a 85-a și a 95-a
percentilă. Cazul clinic prezentat are ca scop evidențierea elementelor sindromului metabolic
și asocierea cu diabetul zahrat tip 2 la copil, precum și conduița terapeutică necesară
normalizării parametrilor ce sunt modificați în această patologie pediatrică.
Caz clinic: Pacientul în vârstă de 14 ani s-a prezentat pentru poliurie, polidipsie, polifagie și
scădere în greutate (10kg în 9 luni). Din anamneză s-a evidențiat stilul de viață nesănătos al
copilului, acesta fiind sedentar și având o dietă hipercalorică, bogată în glucide rafinate,
grăsimi saturate și trans, săracă în legume și fructe. La prezentare, s-a constatat un copil cu
suprapondere și obezitate abdominală, fără semne clinice de acidoză metabolică. Din
explorările paraclinice amintim: glicemie la internare=339mg/dL, HbA1c=14,3%, colesterol
total=228mg/dL, HDL-colesterol=23mg/dL, trigliceride=647mg/dL, LDL-colesterol
necalculabil, pH arterial normal, corpi cetonici urinari prezenți și urocultură pozitivă pentru
Escherichia Coli. Pe parcursul internării a fost determinat peptidul C cu valoare de 2,22ng/mL
(valori normale: 0,78-5,19 ng/mL) și anticorpii anti-GAD<5 IU/mL. Astfel, se stabilește
diagnosticul de diabet zaharat tip 2 dezechilibrat insulinonecesitant. Pe parcursul internării,
s-a explicat importanța optimizării stilului de viață, recomandându-se dietă hipoglucidică și
hipolipidică, cu alimente bogate în fibre și antioxidanți și s-a inițiat insulinoterapia, în doze
titrate regresiv, la externare primind recomandare de insulină degludec 6 UI/zi și metformin
2g/zi. Având în vedere vârsta pacientului, pentru dislipidemie, s-a recomandat tratament cu
acizi grași omega 3 3g/zi. După încheierea antibioterapiei pentru infecția urinară, în
ambulator, în funcție de profilul glicemic s-a sistat insulinoterapia. Evoluția parametrilor clinici
și paraclinici la 1 lună, 3 luni și 6 luni de la externare este prezentată în Tabelul 1. Astfel, se
constată scăderea ponderală, echilibrarea diabetului zaharat și ameliorarea tabloului lipidic.

Concluzii: Având în vedere evoluția acestui pacient după optimizarea stilului de viață alături
de tratamentul medicamentos, se poate concluziona că ar fi de un real folos ca societatea
noastră să investească în educația nutrițională și a stilului de viață în special la populația
pediatrică (dar nu numai). Un copil cu obezitate este un adult cu diabet zaharat tip 2 care
reprezintă o mare povară pentru sistemul de sănătate, dar mai ales pentru el însuși. Este de
dorit să conștientizăm această problemă și să ne implicăm activ în soluționarea ei.

Introduction: Childhood obesity is known as greater than the 95th percentile for weight and
height for age, while childhood overweight is between the 85th and 95th percentile. The
presented clinical case aims to highlight the elements of the metabolic syndrome and the
association with type 2 diabetes mellitus in children, as well as the necessary therapeutic
behaviour to normalize the parameters that are modified in this child pathology.
Clinical case: The 14-year-old patient presented with polyuria, polydipsia, polyphagia and
weight loss (10kg in 9 months). The history showed the child's unhealthy lifestyle, being
sedentary and having a high-calorie diet, rich in refined carbohydrates, saturated and trans
fats, poor in vegetables and fruits.
At presentation, the child was found to be overweight and with abdominal obesity, without
clinical signs of metabolic acidosis. From the blood tests that were taken we recall: blood
sugar at admission=339mg/dL, HbA1c=14.3%, total cholesterol=228mg/dL, HDL-
cholesterol=23mg/dL, triglycerides=647mg/dL, incalculable LDL-cholesterol, normal arterial
pH, urinary ketone bodies present and positive urine culture for Escherichia Coli.
During hospitalization, C-peptide was determined with a value of 2.22 ng/mL (normal values:
0.78-5.19 ng/mL) and anti-GAD antibodies<5 IU/mL. Thus, the diagnosis of unbalanced
insulin-requiring type 2 diabetes is established. The importance of lifestyle optimization was
explained, then a hypoglycemic and hypolipidic diet was recommended containing foods rich

in fiber and antioxidants. Insulin therapy was initiated, in regressively titrated doses. At
discharge the patient received a recommendation for insulin degludec 6 IU/day and
metformin 2g/day. Considering the age, the treatment for dyslipidemia was with omega 3
fatty acids 3g/day. After the end of the antibiotic therapy for the urinary infection, in the
outpatient clinic, depending on the glycemic profile, insulin therapy was halted. The evolution
of clinical and paraclinical parameters after 1 month, 3 months and 6 months after discharge
is presented in Table 1. Thus, weight loss, balancing of diabetes and improvement of the lipid
profile are observed.
Conclusions: Considering the evolution of this patient after the optimization of the lifestyle
along with the drug treatment, it can be concluded that it would be of real benefit for our
society to invest in nutritional and lifestyle education, especially in the pediatric population
(but not only). An obese child is an adult with type 2 diabetes who represents a great burden
on the health system, but especially on himself. It is desirable to be aware of this problem and
to be actively involved in its solution.

dacă anumite investigații de laborator se coreleaza cu evoluția infecției cu SARS-CoV-2 la
pacienții cu diabet zaharat (DZ).
Materiale și metode: Am realizat un studiu retrospectiv care a inclus 238 de pacienți
infectați cu SARS-CoV-2 și internați în Spitalul Clinic „Nicolae Malaxa” - Secția Clinică Diabet,
Nutriție și Boli Metabolice, în perioada noiembrie 2020 - martie 2022. Două tipuri de date au
fost analizate: date privind severitatea infecției (dispnee ca simptom principal la internare,
număr de zile de spitalizare, necesitatea administrării de corticoterapie, antivirale,
imunomodulatoare sau oxigenoterapie, necesitatea ventilației mecanice, internare în ATI,
exitus) și date privind terapia antihiper-glicemiantă utilizată înainte de spitalizare. Pentru
prelucrarea datelor statistice am folosit programul IBM SPSS Statistics.
Rezultate și discuții: Lotul a fost format din pacienți cu DZ de tip 2 (n=221), respectiv tip 1
(n=16). Raportul dintre sexe a fost aproximativ egal: 55,1% femei și 44,9% bărbați. Vârsta
medie a fost de 64,895 +/- 13,7 ani, iar debutul mediu al DZ a fost cu 10,141 +/- 9,29 ani în
urmă. Doar 7,6% dintre pacienții internați au fost vaccinați (n=18). Evoluția COVID-19 a fost
ușoară în 16% din cazuri, moderată în 58%, moderat-severă în 6,7% și severă în 18,9%.
Frecvența exitusului în secția noastră a fost de 8,4% (n=20). La internare, frecvența
pacienților diabetici care urmau un tratament cu Biguanide a fost de 53,4%, cu Sulfoniluree
22,7%, iDPP4 8,8%, AR-GLP1 2,5%, iSGLT2 4,6%. 65,5% nu au urmat vreun tratament
insulinic, 0,8% utilizau insulină premixată, 10,9% insulină bazală, 1,7% au urmat un regim
bazal-plus și 19,7% un regim bazal-bolus. În studiul nostru, tipul de DZ a fost corelat statistic
cu severitatea infecției (p=0,049), necesitatea terapiei cu imunomodulatoare- (p=0,027),
oxigenoterapiei (p=0,048) și corticoterapiei (p=0,015), având tendința de a se corela cu
prezența dispneei la internare (p=0,053). Dintre medicația noninsulinică, singurele asocieri
semnificative au inclus Sulfonilureea și severitatea infecției, administrarea de corticoterapie
și dispneea la internare (p=0,049, 0,015, 0,013). Nu au existat corelații între numărul de zile
de spitalizare, necesitatea ventilației mecanice, internarea în ATI, exitus, necesitatea O2- sau
a corticoterapiei la externare și oricare dintre medicația insulinică sau noninsulinică
(p>0,05). În plus, nu s-au decelat corelații semnificative între profilul glicemic și parametrii
de severitate utilizați în studiul nostru (p>0,05).
Concluzii: Nu există suficiente corelații semnificative statistic între utilizarea
medicamentelor antidiabetice înainte de internare și severitatea infecției cu SARS-CoV-2.
Aceste rezultate se adaugă la datele contradictorii din literatură, fiind necesare cercetări
suplimentare.

Premises and Objectives: In March 2020, WHO declared the pandemic of COVID-19.
Understanding this new pathology is of major interest, so far over 530 million cases and over
6 million deaths being confirmed. Once infected, the prognosis is still uncertain, researchers
all over the world attempting to link certain evolutions of the disease to particular markers or
characteristics pertaining to the group of patients which presented a more unfavorable
outcome. The aim of this study is to evaluate the influence of the preadmission antidiabetic
medication on the evolution and outcome of SARS-CoV-2 infection in patients with diabetes
mellitus (DM).
Materials and Methods: We conducted a retrospective study that included 238 patients
infected with SARS-CoV-2 and hospitalised in the “Nicolae Malaxa’’ Clinical Hospital - Clinical
Department of Diabetes, Nutrition and Metabolic Diseases, during November 2020 - March
2022. There were two types of data analysed: data regarding the severity of the infection
(dyspnea as main symptom at admission, number of hospitalisation days, necessity of cortico-
, antiviral-, immunomodulatory- or oxygen therapy, necessity of mechanical ventilation, ICU
admission, exitus) and data regarding the antihyperglycemic agents used before
hospitalisation. For statistical data processing we used the IBM SPSS Statistics program.
Results and Discussions: The group consisted of patients with type 2 DM (n=221) and type 1
(n=16). The sex ratio was approximately equal: 55.1% women and 44.9% men. The average
age was 64.895 +/- 13.7 years, and the average onset of DM was 10.141 +/- 9.29 years ago.
Only 7.6% of hospitalised patients were vaccinated (n=18). The evolution of COVID-19 was
mild in 16% of cases, moderate in 58%, moderate-severe in 6.7% and severe in 18.9%. The
frequency of exitus in our ward was 8.4% (n=20). At admission, the frequency of diabetic
patients who were following a prior treatment with Biguanides were 53.4%, with
Sulfonylureas 22.7%, iDPP4 8.8%, AR-GLP1 2.5%, iSGLT2 4.6%. 65.5% followed none of the
insulin regimens, 0.8% administered premixed insulin, 10.9% basal insulin, 1.7% followed a
basal-plus regimen and 19.7% a basal-bolus regimen. In our study, the type of DM was
statistically correlated with the severity of infection (p=0.049), necessity of
immunomodulatory- (p=0.027), oxygen- (p=0.048) and corticotherapy (p=0.015), while
tending to correlate with dyspnea at admission (p=0.053). Among noninsulinic medication,
the only significant associations included Sulfonylureas and severity of the infection,
corticotherapy administration and dyspnea at admission (p=0.049, 0.015, 0.013). In the
current study, there were no correlations between the number of hospitalisation days,
necessity of mechanical ventilation, ICU admission, exitus, necessity of O2- or corticotherapy
at discharge and any of the insulinic or noninsulinic medication (p>0.05). Furthermore, there
were little to no significant correlations between glycemic profile and the severity parameters
used in our study (p>0.05).
Conclusions: Regarding our study group, there are no sufficient statistically significant
correlations between preadmission use of antidiabetic medications and severity of SARS-CoV-

2 infection. These results are appending to the conflicting data from literature, therefore
requiring further research.

Concluzii: Studiul nostru a demonstrat o frecvență semnificativ statistic mai mare a bolii
cronice de rinichi la pacienții cu risc de OSA, fiind în concordanță cu studiile recente care au
demonstrat o asociere între OSA și nefropatia diabetică și afectarea funcției renale la pacienții
cu DZ2, sugerând că asocierea OSA cu DZ2 este un factor de risc independent pentru boala
cronică de rinichi.

Results: In our study, 312 patients with T2DM (59%) presented a STOP-Bang score ≥5. The
statistical analysis regarding diabetes chronic complications and the risk of moderate-to-
severe OSA is presented in Table 1. Furthermore, we also observed a statistically higher
percentage of patients with both microalbuminuria and macroalbuminuria within the
subjects with STOP-BANG score ≥ 5 (p = 0.005).

are ca scop evidențierea apariției vasculitei la un pacient cu diabet zaharat tip 2 (DZ 2) și
amputații recente degete picior stâng.

Caz clinic: Pacientul în vârstă de 60 de ani, fumător, consumator cronic de alcool, diagnosticat
cu DZ 2 de aproximativ 21 de ani, în tratament ambulator cu Metformin 1g- 2cp/zi și Gliclazida
60mg- 2cp/zi, aflat în stadiul de complicații cronice majore (RD preproliferativa cu aplicații
laser, ADOMI, NDPSM), cu amputație recentă (în urmă cu o lună) de degete III și V membrul
inferior stâng s-a prezentat în Clinica Diabet pentru plaga la nivelul bontului de amputație și
plantar stâng, precum și pentru apariția unei erupții tegumentare însoțită de prurit și durere
la nivelul membrelor superioare și inferioare. La examenul obiectiv, se constată plaga atona
cu necroză profundă picior stâng și erupție tegumentară constituită din vezicule, bule și
leziuni eritemato-descuamative acoperite de cruste hematice groase, aderente, dimensiuni
între 0,5 și 6 cm diametrul, bine delimitate (Figura 1).

Introduction: The link between vasculitis and infection is complex. Infections are the basis of
a varied number of vasculitis, it is assumed that they act as a trigger factor, and the skin is
considered the most frequently affected organ. The clinical case that we propose to present
aims to highlight the occurrence of vasculitis in a patient with type 2 diabetes (T2DM) and
recent left toe amputation.
Case report: The 60-year-old patient, smoker, chronic alcohol user, diagnosed with T2DM for
about 21 years, in treatment with Metformin 1g- 2 cp/day and Gliclazide 60mg- 2 cp/day, in
the stage of major chronic complications (preproliferative RD with laser applications,
peripheral arterial disease, diabetic peripheral neuropathy), with recent amputation (one
month ago) of fingers III and V of the left lower limb presented to the Diabetes Clinic for a
wound at the level of the amputation stump and left plantar ulcer, as well as for the
occurrence of a skin eruption accompanied by itching and pain in the upper and lower limbs.
During the objective examination, it was noticed a wound with deep necrosis of the left foot
and a skin eruption consisting of well delimited vesicles, blisters and erythemato-
desquamative lesions covered by thick, adherent hematic crusts, measuring between 0.5 and
6 cm in diameter (Figure1).
Laboratory exams revealed metabolic imbalance and inflammatory syndrome. X-ray of the
foot highlighted the left V metatarsal osteitis. Considering the presence of skin lesions, serum
complement C3, C4, rheumatoid factor, total proteins, serum protein electrophoresis,
pANCA, cANCA, antinuclear antibodies, extended ANA profile-blot in 18 parameters were
performed, all with negative results. The culture from the wound revealed infection with
methicillin-resistant Staphylococcus aureus (MRSA). Skin biopsy with histopathological
examination highlighted the suggestive aspect for vasculitis. Following the interdisciplinary
consultations, it was decided to treat the patient with corticotherapy, antibiotherapy
according to the antibiogram. The metabolic imbalance required basal-bolus insulin therapy,
and for the associated pathology, treatment with Sulodexide 250 ULS 2 tablets/day,
Pentoxifylline 400mg 2 tablets/day, Thioctic acid 600mg 1 tablet/day, Rosuvastatin 20mg 1
tablet/day, Olmesartan/Amlodipine 40/5mg 1 tablet/day.
Conclusions: Although leukocytoclastic cutaneous vasculitis is rarely identified in the context
of staphylococcal infection in the absence of bacteremia, in this case we find an episode of
vasculitis triggered without evidence of the presence of bacteremia. In the context of an
unstable territory with significant metabolic imbalance and affected response to infections
on this background, the amputation resulted in the persistence of the wound and the

development of the ulcer, followed by superinfection with MRSA and later with the
appearance of the skin rash.

Background and objectives: The prevalence of heart failure is higher in patients with diabetes
with or without coronary heart disease or associated risk factors. Recent studies with iSGLT2
show a positive impact on the risks associated with heart failure. The goal of our study is to
evaluate adherence to treatment and the frequency of adverse reactions.
Methods: The retrospective study included 110 adult patients, who have type 2 diabetes,
treated with iSGLT2. The age of the patients was between 39 and 81 years, most of them were
women (n = 62; 56%). The data were obtained from the patients observation hospital records
in the diabetes department of the Nicolae Malaxa Clinical Hospital, Bucharest, between Dec
2018 and June 2021. We analyzed the frequency of side effects and adherence to treatment.
Results: The majority were treated with empaglifozin (n = 92; 83.6%) compared to
dapaglifozin (n = 18; 16.3%). General characteristics of the subjects: median age 65 years,
with a median age of diabetes of 9 years, median HbA1c at initiation of 7.8%. The median
duration of treatment with iSGLT2 was 1.5 years. Regarding the treatment approach, 91.8%
(n = 101) had metformin, 2.72% (n = 3) GLP1 receptor analog and 3.6% (n = 4) insulin therapy.
As cardiovascular comorbidities 18.2% (n = 20) associated congestive heart failure and 40.1%
(n = 45%) ischemic cardiac disease. 20% (n = 22) of patients discontinued treatment due to
side effects such us: allergy, angioedema, dysuria, recurrent UTI, hemorrhagic cystitis each
with a frequency of 0.9% (n = 1) in the studied group. Only 1.8% (n = 2) had mycotic genital
infection that was treated with antifungal drugs and did not require discontinuation of iSGLT2
therapy. There was a decrease in weight between 1 kg and 15 kg with a median of 5 kg.
Conclusions: iSGLT2 is a therapy with cardioprotective but also with renoprotective effects,
and in the same time with a high level of tolerability and therapeutic safety. Although typical
adverse reactions (mycotic genital infections) were much less common in our study than in
RCTs and did not involve cessation of iSGLT2 therapy, overall, we found a much higher
discontinuation rate (20%) compared to RCTs (<5%).

Concluzii: Doar o treime din pacienți au atins țintele de colesterol și jumătate pe cea de
trigliceride, în ciuda măsurilor terapeutice disponibile. Dislipidemia este frecvent asociată cu
alți factori de risc modificabili precum fumatul, obezitatea, hipertensiunea arterială,
dezechilibru glicemic. Mai multă conștientizare ar trebui adusă asupra țintelor de tratament
în scopul de a obține controlul tuturor factorilor de risc cât mai devreme cu putință.

Abstract: Aim of the study: To assess the proportion in which lipid targets (LDL cholesterol,
non-HDL cholesterol, serum triglycerides), are reached in a real-world Type 2 Diabetes
Mellitus (T2DM) patient cohort and whether some associations can be made.

Material and Method: In this cross-sectional study, we included data from patients with
T2DM admitted in a third-care center for Diabetes Mellitus during Jan 2020-March 2022.
Patients with recent onset of the disease (less than 6 months) were excluded.
Results: From 1011 T2DM patients with complete data, 90 % were very high risk patients and
59.6% had documented ASCVD. 7% of patients had triglycerides >350 mg/dl, hence only non-
HDL was calculated. 29.8% of patients reached the LDL target and 28% reached non-HDL
target. Furthermore, 18.6% required a LDL-reduction with >50%. 50.5% had serum
triglycerides < 150 mg/dl. Subgroup analysis showed that patients who obtained LDL and non-
HDL targets had a higher age (p=0.0001), longer duration of DM (p=0.0001), were non-
smokers (p=0.047 for LDL target and p=0.005 for non-HDL target) and had a lower A1c
(p=0.0041 for LDL-target and p=0.0001 for non-HDL target). Also, there were more insulin-
treated in patients who did not reach non-HDL target (p=0.05). Comparative data are shown
in Table 1. No significant differences were obtained for gender, body mass index, systolic
blood pressure, settlement. Of patients with confirmed ASCVD, a better achievement of LDL
targets had patients with history of stroke (35.9%) vs myocardial infarction (27%) vs lower
extremity arterial disease (23.9%).
Discussions: Patients with increased age and duration of diabetes appeared to reach more
the cholesterol targets, perhaps through a better compliance to lifestyle and treatment
recommendations. Uncontrolled dyslipidemia was also associated with poor glycemic control
and active smoking, once again emphasizing an overall reduced adherence.
Conclusions: Only a third of patients reached cholesterol targets and half the triglycerides
targets, despite available treatment options. Dyslipidemia is often associated with other
modifiable risk factors such as smoking, obesity, hypertension, glycemic imbalance. More
awareness should be raised on treatment goals in order to achieve control of all risk factors
earlier as possible.

Discuții, Concluzii: Evoluția îndelungată a DZ tip 1 nu a determinat apariția bolii renale cronice
diabetice și a complicațiilor macrovasculare în contextul dezechilibrului metabolic cronic, însă
instalarea afecțiunilor musculoscheletale debilitante scade calitatea vieții pacientului, fiind în
general subdiagnosticate.

Results: microvascular complications set in after 15 years of type 1 diabetes, followed by
musculoskeletal and connective tissue disorders 7 years later. Autonomic diabetic
neuropathy has been observed 26 years after the onset of type 1 diabetes.

Discussions, Conclusions: The long evolution of type 1 diabetes did not cause chronic diabetic
kidney disease and macrovascular complications in the context of chronic metabolic
imbalance, but the onset of debilitating musculoskeletal disorders decreases the patient's
quality of life and is generally underdiagnosed.

îmbunătățită a complicațiilor pe termen lung. Obiectivul studiului s-a bazat pe descrierea
pacienților cu diabet zaharat de tip 2 (DZT2) care dezvoltă insuficiență cardiacă (IC) din
perspectiva atitudinii terape-utice adoptate în 2022,dar și de a compara datele cu cele
obținute în 2019 1.
Material și metodă: Studiul actual este unul retrospectiv observațional transversal, care a
inclus 101 pacienți internați la INDNBM „N.C. Paulescu”, București, România în perioada
01.04.2021-1.03.2022, care au prezentat atât T2DM, cât și HF. S-au evaluat indici antropome-
trici, factori de risc cardiovascular, hemograma și parametri biochimici.În vederea analizei obi-
ectivului lucrării,parametrii și datele obținute din lotul curent de pacienti au fost comparate
cu cele ale lotului de pacienti din 20191.
Rezultate și discuții: Vârsta medie a fost de 68,64±98,89 ani,dintre care 67 % au fost bărbați.
HbA1c medie a fost de 8,01 ± 1,8%. Procentul de pacienți tratați cu agoniști ai receptorilor
GLP-1 rămâne relativ similar cu cel din 2019* (18% în 2022 față de 19% în 2019), dar numărul
de pa-cienți tratați cu inhibitori SGLT2 este în creștere (26% față de 6%). În plus, actual sunt
mai mulți pacienți cu injecții multiple zilnice (31% față de 28%) și cu inhibitori DPP4 (10% față
de 7,1%) decât în 2019.
Concluzii: Managementul insuficienței cardiace la pacienții cu diabet zaharat necesită o abor-
dare complexă a diferitelor mecanisme fiziopatologice. Studiul nostru se concentrează pe
impor-tanța noilor ghiduri de tratament și arată că insuficiența cardiacă este un criteriu
important pentru inițierea unor medicamente antidiabetice moderne. Inhibitorii SGLT2 sunt
încă subprescripți, dar modificările SPC-urilor privind raportul de filtrare glomerulară permis
și modificările protocoa-lelor privind modalitatea de prescripție vor rezolva parțial această
problemă.

Premises and objectives: Diabetes mellitus (DM) is a major risk factor for several
cardiovascular outcomes, including heart failure (HF). In the recent years, the therapeutic
approach to the control of hyperglycemia has become much more complex, leading to a
better metabolic control and to an improved prevention of long-term complications. The
objective of the study was to describe the type 2 diabetes patients who develop heart failure

from the perspective of the therapeutically attitude adopted în 2022 and to compare the data
with those obtained in 2019.
Material and Method: The current study is a transversal observational retrospective one, that
included 101 patients admitted to NIDNMD “N.C. Paulescu”, Bucharest, Romania during the
period 01.04.2021-1.03.2022, who presented both T2DM and HF. We assessed
anthropometric indices, cardiovascular risk factors, blood count, biochemical parameters
such as glycosylated hemoglobin, lipid profile, transaminases.
Results and discussions: Mean age was 68,64±98,89 years, 67 % were men. Mean HbA1c was
8.01 ± 1.8%. The percentage of patients treated with GLP1 receptor agonists remain relatively
similar to that of 2019 ( 18% in 2022 vs. 19 % in 2019), but the number of patients being
treated with SGLT2 inhibitors is increasing (26% vs 6%). Thus, now are slightly more patients
with multiple daily injections than in 2019 (31% vs 28%) and with DPP4 inhibitors (10% vs
7.1%).
Conclusions: The management of heart failure in patients with diabetes requires a complex
approach to various pathophysiological mechanisms. Our study focuses on the importance of
new treatment guidelines and reveal that HF is an important criterion for initiating some
particular modern antidiabetic drugs. The SGLT2-inhibitors are still being underprescribed,
but the changes in SPCs regarding the permitted glomerular filtration ratio and the changes
in protocols regarding the modality of prescription will partly resolve this issue.

Pacientă C.R. în vârstă de 3 ani, diagnosticată cu DZ tip 1 în urmă cu doi ani, actual în tratament
intensiv modern cu insulină Humalog prin pompă de insulină cu o rată bazală de 6,175 UI/24
h respectiv bolusuri prandiale de 0,5-0,6 UI pentru 10 g de hidrați de carbon.
Valoarea medie glicemică din cadrul zilelor analizate a fost de 154 mg/dl, iar valoare HbA1c
(6,05%) fiind in țintă. Procentul de timp petrecut în intervalul glicemic optim de 70-180 mg/dl
a fost de 71%, însoțit de valori glicemice sub 70 mg/dl respectiv sub 54 mg/dl în procent mai
mic de 1%. Procentul de timp predominant a fost în intervalul glicemic 180-250 mg/dl (24%
din totalul de timp al înregistrărilor CGMS), în timp ce valorile glicemice mai mari de 250 mg/dl
s-au regăsit în doar 4%. Statistica orară a celor 90 de zile evidențiază un procent mai mare de
timp în care valorile glicemice au fost sub 70 mg/dl în a doua jumătate a zilei (între orele
17:00-19:00), cel mai probabil în contextul efectuării activității fizice, urmat de un procent de
26-29% din timp petrecut în intervalul glicemic 180-250mg/dl și un procent de 4-5% cu valori
glicemice mai mari de 250mg/dl pe parcursul următoarelor 3-4 ore, hiperglicemie apărută
secundar hipoglicemiilor și ingestiei de hidrați de carbon în scopul corectării acesteia.
Caz 2
Pacientul N.A. în vârstă de 18 ani, normoponderal, cu antecedente heredocolaterale de DZ
tip 2, diagnosticat cu DZ tip 1 în urmă cu doi ani, actual în tratament intensiv modern cu
insulină NovoRapid prin pompă de insulină, cu o rată bazală de 9,7 UI/24h și bolusuri prandiale
de 1UI pentru 10 g de hidrați de carbon.
Valoarea medie glicemică din cadrul zilelor analizate a fost de 106 mg/dl, iar valoare HbA1c
(5,4%) fiind în țintă. Procentul de timp pe care pacientul l-a petrecut în intervalul optim
glicemic (70-180mg/dl) a fost de 95%, valori glicemice mai mici de 70 mg/dl regăsindu-se în
doar 2% din timpul total, iar valori mai mici de 54 mg/dl în mai puțin de 1% din timp. Procentul
de timp cu valori glicemice peste 180mg/dl, dar sub 250 mg/dl a fost de numai 2% și mai puțin
de 1% pentru valori glicemice de peste 250 mg/dl.
Pacientul prezintă variabilitate glicemică nesemnificativă, coeficientul de variație având o
valoare de 19,3% (valoarea cut-off de definire a variabilității glicemice fiind CV≥36%). De
asemenea, pe baza valorii MAGE:58,98 putem spune că nu există excursii glicemice
importante în cadrul aceleași zile, și nici variații importante în cadrul a două zile diferite
(MODD:19,86).
Concluzii: În cadrul acestui studiu am prezentat faptul că HbA1c nu este suficientă pentru a
cuantifica controlul glicemic, asigurarea unui control metabolic adecvat bazându-se pe
evaluarea variabilității glicemice pe înregistrările CGMS.

Introduction: Continuous glucose monitoring systems (CGMS) can bring a real benefit to
people suffering of diabetes mellitus (DM) type 1 in terms of ensuring optimal metabolic
control. The development of these new devices can help in a more accurate and detailed
quantification of glycemic variability and contributes to obtaining and maintaining an
adequate metabolic control.
Case 1
Patient C.R. 3 years old, diagnosed one year ago with DM type 1, currently on modern
intensive treatment with Humalog insulin via insulin pump with a basal rate of 6.175 IU/24 h
and prandial boluses of 0.5-0.6 IU per 10 g of carbohydrates. The average glycemic value
during the analyzed days was 154 mg/dl, and the HbA1c value (6.05%) was right on target.
The percentage of time spent in the optimal glycemic range of 70-180 mg/dl was 71%,
accompanied by glycemic values below 70 mg/dl and below 54 mg/dl, in a percentage of
less than 1%. The predominant percentage of time was in the glycemic range 180-250 mg/dl
(24% of the total time of CGMS recordings), while glycemic values higher than 250 mg/dl
were found in only 4%. The hourly statistics of the 90 days show a higher percentage of the
time that glycemic values were below 70 mg/dl in the second half of the day (between
17:00 and 19:00), most likely performing physical activities, followed by a percentage of 26-
29% of the time spent in the glycemic range 180-250mg/dl and a percentage of 4-5% with
glycemic values higher than 250mg/dl during the next 3-4 hours, a hyperglycemic value
shown after many episodes of hypoglycemia and carbohydrate intake in order to correct it.
Case 2
Patient N.A. 18 years old, normal weight, with a hereditary history of type 2 DM, diagnosed
with type 1 DM two years ago, currently on modern intensive treatment with NovoRapid
insulin via insulin pump, with a basal rate of 9.7 IU/ 24h and prandial boluses of 1IU for 10g
of carbohydrates. The average glycemic value during the analyzed days was 106 mg/dl, and
the HbA1c value (5.4%) was on target. The percentage of time the patient spent in the
optimal glycemic range (70-180mg/dl) was 95%, glycemic values lower than 70 mg/dl being
found in only 2% of the total time, and values lower than 54 mg/dl in less than 1% of the
time. The percentage of time with glycemic values above 180mg/dl but below 250mg/dl was
only 2% and less than 1% for glycemic values above 250mg/dl. The patient shows
insignificant glycemic variability, the coefficient of variation having a value of 19.3% (the cut-
off value for defining glycemic variability is CV≥36%). Also, based on the value of
MAGE:58.98 we can say that there are no important glycemic variations within the same
day, nor important variations within two different days (MODD:19.86).
Conclusions: This study shows the fact that HbA1c is insufficient to quantify glycemic
control, ensuring adequate metabolic control based on the assessment of glycemic
variability on CGMS records.

Introduction: The accessibility and adaptability of the medical system in major force
situations influence the prognosis and control of chronic diseases. That is why we conducted
an observational study aimed at evaluating glycemic control, addressability to the
diabetologist, and access to treatment of patients with type 1 diabetes (DZ1) during the Covid-
19 pandemic.
Materials and methods: We used an online questionnaire (Google Forms®) published for one
month on one of the online platforms addressed to DZ1 patients, which includes 40 questions
related to the history of diabetes, the history of hypoglycemia, the presence of anxiety, access
to medication, glycemic self-monitoring and self-care during the pandemic.
Results: Of the 133 patients (75 women), 19.47% were infected with SARS-CoV2. The mean
HbA1c was 7.1±1.11% considering that 16.81% of the subjects reported episodes of
hypoglycemia levels 2 and 3. We also observed an increase in the frequency of self-monitoring
(52.21%). Regarding access to treatment, only 4.42% stated that they had no access to insulin,
2.65% to capillary blood glucose monitoring testing, and 24.78% stated limited access to
continuous glucose monitoring systems or insulin pumps. Regarding the screening of
complications, 48.67% did not consult an ophthalmologist, and 83.19% did not perform a
neurological consultation.
Conclusion: The SARS-CoV2 pandemic negatively influenced access to the Romanian
healthcare system.

The presence of albuminuria has long been recognized as an adverse prognostic feature in
patients with renal disease. Those patients with appreciable albuminuria are much more likely
to develop tubulointerstitial scarring and fibrosis and progress to end-stage renal failure. For
many years it was thought that excess albuminuria was simply a marker or a more severe
renal disease which was more likely to progress as a result of this severity rather than as a
result of the albuminuria itself. This conviction was strengthened by the general assumption
that albumin was a benign or inert moleeule serving primarily to exert oncotic pressure and
act as a carrier within the circulation.
More recently this view has been challenged with the accumulation of evidence suggesting
that albumin is able to influence the function or cells with which it makes contact in the
manner of a signalling molecule.
Obiective: Sindromul metabolic definit printr un grup de factori de risc cardiovascular a atins
in ultimul deceniu proporții epidemice. S-a emis ipoteza conform căreia, tulburările în
metabolismul fosfaților pot reprezenta o caracteristică a sindromului metabolic. Scopul
studiului a fost de a investiga relația dintre nivelurile de fosfat și prezența componentelor
sindromului metabolic, precum și mecanismul presupus pentru reducerea nivelului de fosfat
în sindromul metabolic.

Materiale și metode: Am înrolat 155 de subiecți: 64 cu sindrom metabolic și 91 de control.
Parametrii biochimici ai populației studiate pentru sindromul metabolic au fost comparați cu
populația sănătoasă.
Rezultate: Pacienții cu sindrom metabolic au prezentat niveluri semnificativ mai scăzute de
fosfat (46%) și magneziu în comparație cu martorii (22,7%) (p<0,001). Femeile au prezentat
niveluri serice de fosfat semnificativ mai mari decât bărbații (3,32 mg/dl față de 3,18 mg/dl)
(p<0,03). Nivelurile serice de magneziu nu au evidențiat modificări statistic semnificative
între bărbați și femei. Ratele de excreție fracționată a fosfatului la pacienții cu sindrom
metabolic au fost similare cu martorii (10,1±10,2% vs 13,1±9,9%), precum și excreția
fracționată de magneziu (3,1±1,6% vs 2,8±1,3%).
Concluzii: Pacienții cu sindrom metabolic prezintă concentrații semnificativ mai mici de fosfat
și magneziu în comparație cu martorii. Această reducere este probabil atribuită redistribuirii
interne a fosfatului și este mai evidentă pe măsură ce crește numărul de componente ale
sindromului metabolic.

Background and Aims: Metabolic syndrome represent a cluster of cardiovascular risk factors
and reached epidemic proportions. It was hypothesized that disturbances in phosphate
metabolism may represent a feature of the metabolic syndrome. The aim of the study was to
investigate the relationship between phosphate levels and the presence of metabolic
syndrome components, as well as the putative mechanism for reduced phosphate level in
metabolic syndrome.
Materials and Methods: We enrolled 155 subjects: 64 with metabolic syndrome and 91
controls. Biochemical parameters of the metabolic syndrome study population were
compared with the healthy population. Results: Patients with metabolic syndrome showed
significantly lower phosphate (46%) and magnesium levels compared with controls (22.7%)
(p<0.001). Women showed significantly greater serum phosphate levels than men (3.32
mg/dl versus 3.18 mg/dl) (p<0.03). Serum magnesium levels did no differ significantly
between men and women. Fractional phosphate excretion rates in patients with metabolic
syndrome were similar with controls (10.1±10.2% vs 13.1±9.9%), as well as fractional

magnesium excretion (3.1±1.6% vs 2.8±1.3%). Conclusions: Patients with metabolic syndrome
show significantly lower phosphate and magnesium concentrations compared to controls.
This reduction is likely to be attributed to internal redistribution of phosphate and is more
pronounced as the number of components of metabolic syndrome increases.

INCREASED PREVALENCE OF PERIPHERAL ARTERIAL DISEASE IN HOSPITALIZED
PATIENTS WITH DIABETES

Introduction: It is estimated that in the European Union the prevalence of peripheral arterial
disease (PAD) is 3.4% with 17 million patients with this condition, for the USA the estimated
prevalence is about 5%. Diabetes and smoking are the main risk factors for BAP. Aim: The aim
of the paper is to evaluate the prevalence of BAP in hospitalized patients with diabetes
mellitus (DM), the degree of their metabolic control, the treatment followed, or the presence
of other risk factors.
Materials and methods: We carried out an observational, cross-sectional study where we
included patients admitted to the diabetes departments of the tertiary center "National
Institute of Diabetes, Nutrition, and Metabolic Diseases Prof. Dr. N. Paulescu". The study
period was one year (06.01.2021-05.30.2022). We included patients previously diagnosed
with type 1 or type 2 diabetes for whom ankle-brachial index (ABI) and/or arterial Doppler
ultrasound was performed during hospitalization. Data were collected, stored, and analyzed
in Microsoft Excel 2010 spreadsheet software. For statistical analysis, we used SPPS version
20.
Results and discussion: Out of the total number of 155 patients included in the study, a
number of 104 patients were male (67%). The mean age was 61.5±12.71 years and the
duration of the disease was 17.57±10.24 years. In the study group, 57.41% of patients were
diagnosed with BAP, the prevalence being much higher compared to other populations, a fact
explained by the long duration of diabetes and the poor control of the disease (median HbA1c
8.8±2.8%). Also, cases with multiple and complex pathologies end up being hospitalized in
tertiary centers, which means that the results apply only to this population. When we
analyzed subjects according to the type of diabetes, for type 1 DM HbA1c was significantly
positively correlated with the IGB value (Spearman coefficient=0.518, p=0.008).
Conclusions: BAP has an increased prevalence among DM patients admitted to a tertiary care
center. The IGB value is positively correlated with HbA1c in type 1 DM patients.
