Enterococii au fost clasificaţi iniţial drept coci Gram pozitivi aparţinând genului
Streptococcus1.
Numiţi ulterior “streptococi de provenienţă fecală” sau “enterococi”, au fost
consideraţi pentru mult timp o subdiviziune a genului Streptococcus (streptococci de
grup D), diferenţiindu-se de aceştia prin rezistenţa crescută la agenţi chimici şi fizici 2.
La începutul anilor 1930, enterococii au fost clasificaţi ca streptococi de grup D
şi au fost diferenţiaţi de grupul D de streptococi non-enterococi prin teste biochimice
auxiliare2.
În anii următori, Sherman a recomandat ca termenul “enterococi” să fie utilizat
în mod specific pentru streptococii care se dezvoltă atât la 10°C cât şi la 45°C, la un
pH de 9.6, în prezenţa NaCl 6.5%, supravieţuiesc la 60°C timp de 30 de minute şi
hidrolizează esculina3.
Diferenţierea evidentă a S.faecalis și S.faecium de ceilalţi membrii ai genului a
făcut posibilă includerea acestor specii într-un gen bacterian nou, ale cărui
caracteristici au fost foarte bine studiate prin tehnici de biologie moleculară2.
Metodele ce stabilesc criteriile de includere a bacteriilor în genul Enterococcus
cuprind recombinarea ADN-ADN, secvenţierea genică a subunităţii 16S ARNr, analiza
proteică şi testele fenotipice obişnuite4, 5.
Bacteriile din genul Enterococcus au fost catalogate ca bacterii cu un grad scăzut
de patogenitate, dar in anii 1990 au devenit importanţi agenţi etiologici ai infecțiilor
nosocomiale6.
La pacienţii spitalizaţi, enterococii pot fi evidenţiati la nivelul leziunilor
ţesuturilor moi, ulcerelor și tractului gastro-intestinal7.
Sindroame infecţioase care recunosc enterococi în etiologie pot fi reprezentate
de infecții ale tractului urinar la pacienţii cu uropatii obstructive, infecții mixte ale
plăgilor mai ales la pacienţii aflaţi sub tratament antimicrobian 8, 9.
Pot sa apară bacteriemii la vârstnici și pacienţii imunocompromiși 10.
Studii efectuate în Statele Unite au demonstrat că majoritatea pacienţilor
infectaţi cu tulpini deosebit de rezistente la antibiotic provin din unităţile de terapie
intensivă11.
Factorii de risc ce pot fi adăugaţi celor deja menţionaţi regăsim: durata crescută
a spitalizării, colonizarea gastro-intestinală cu VRE, expunerea la echipamente
medicale contaminate, transplantul medular, afecţiunile hematologice, tratamentul cu
cefalosporine de generatia a 3-a și antibiotice cu acţiune asupra bacteriilor anaerobe16.

CONTRIBUŢIA PERSONALĂ
Obiective
Enterococii, deși fac parte dintre comensalii florei intestinale pot deveni
patogeni nosocomiali.
În ultimele 3 decenii s-a constatat o creștere constantă a incidenței acestor
infecții, fenomen cauzat probabil de creșterea populației care prezintă factorii de risc
(inflaţii cronice, intervenţii chirurgicale, neoplazii intestinale)dar și de apariția
tulpinilor de enterococi cu rezistență multiplă la antibiotice, în special la Vancomicină.
În această lucrare am urmărit evaluarea implicării tulpinilor de Enterococcus
spp. în etiologia infecțiilor și sensibilitatea la antibiotice a acestor microorganisme.
În acest scop, au fost luate în studiu atât tulpinile izolate din infecții de pe
secţiile spitalului, cât și tulpini izolate de la pacienţi din ambulator.
În al doilea rând, am urmărit evidențierea fenotipurilor de rezistență ale

Material şi metodă
Studiul a fost realizat prospectiv, urmărindu-se tulpinile de Enterococcus spp.
implicate în etiologia infecțiilor atât la pacienții spitalizați cât și la pacienții din
ambulator.
În acest scop, au fost luate în studiu infecțiile enterococice diagnosticate la
pacienţii internaţi pe secţiile institutului, precum și infecțiile diagnosticate în
ambulator în perioada ianuarie 2010- decembrie 2012.
În cadrul spitalului, s-au utilizat date ale Compartimentului de Microbiologie
al Laboratorului Clinic. Pentru colectarea datelor din spital am folosit fișa tip de
supraveghere, prezentată în anexa Ordinului Ministerului Sănătăţii 10/2005.
Pentru fiecare an studiat (2010, 2011 şi 2012) s-au cules date atât despre
tulpinile de enterococi care s-au dovedit a fi agent etiologic unic cât și despre tulpinile
de enterococi izolate în asociere cu alte microorganisme.
În spital, toate tulpinile de enterococi izolate au fost identificate cu sistemul
automat Vitek 2 Compact (bioMérieux).
Speciile de Enterococcus identificate de VITEK 2 GP: E.avium, E. casseliflavus,
E.cecorum, E.columbae, E.durans, E.faecalis, E.faecium, E.gallinarum, E.hirae,
E. raffinosus, E. saccharolyticus.
În ambulator, bacteriile din genul Enterococcus au fost identificate rapid pe baza
prezenței antigenului de grup D, cu truse PASTOREXR STREP (BioRad) şi al galeriilor API 20
STREP (bioMérieux).
Metodologia statistică
Datele obţinute în urma cercetării au fost introduse şi stocate în baze de date tip
Microsoft Acces 2010 (format *.mdb) şi au fost exportate spre prelucrare în fişiere de
tip worksheet Excel 2010 (*.xls).
Prelucrarea statistică a fost efectuată cu ajutorul pachetelor software Microsoft
Excel 2010, EPI-INFO versiunea 6.04 şi IBM SPSS versiunea 10, rezultatele prelucrării
fiind exportate în toate cele trei cazuri în fişiere de tip worksheet Excel 2010.
Variabilele de tip dihotomic, caracterizate prin procentaj, au fost comparate prin
aplicarea tabelelor de contigență, cu calcularea testului χ2 și utilizarea, la nevoie, a
corecției Fisher18.
Toate testele statistice au fost calculate cu 2 extremități iar valoarea semnificației
statistice p s-a luat în considerare la valori ≤ 0,01.

Concluzii generale
Tulpinile de Enterococcus spp. izolate din spital au reprezentat 15,6% din
totalul germenilor implicați în etiologia unor infecții, în timp ce în ambulator 7,05%
din infecțiile diagnosticate au fost determinate de enterococi.
Enterococii deși nu sunt printre cei mai frecvent izolați agenți patogeni,
prin rezistența lor naturală la cefalosporine, fac parte din flora implicată în etiologia
unor infecții grave la pacienți spitalizați.
Stabilirea fenotipurilor de rezistență circulante în spital dar și în
ambulator este importantă în elaborarea terapiei antiinfecțioase în regim de urgență.
Diferențele semnificative statistic privind rezistența la chimioterapicele
antiinfecțioase în favoarea spitalului comparativ cu ambulatorul, sunt legate de
administrarea mai frecventă a antibioticelor la pacienții spitalizați, dar și de prezența
altor factori de risc privind transmiterea tulpinilor multirezistente.
Enterococii dețin numeroase mecanisme de rezistență capabile să le
asigure supraviețuirea în prezența chimioterapicelor antiinfecțioase, ceea ce impune
identificarea unor posibile noi ținte de acțiune pentru antibiotice.
Controlul emergenței și răspândirii antibiorezistenței reprezintă o
prioritate în sănătatea publică, la nivel mondial.

PERSONAL CONTRIBUTION
1. Assumption of work/Objectives
Enterococci, although they are part of the intestinal flora commensals, may
become nosocomial pathogens.
In the past three decades, a constant growth in the number of infections
caused by species of the genus Enterococcus has been reported, a phenomenon
probably caused by a growth in the population that shows risk factors (chronic
inflammations, surgery, intestinal neoplasia), but also by the emergence of enterococci
strains with multiple resistance to antibiotics, especially Vancomycin 20.
The present paper is concerned with the assessment of the involvement of
Enterococcus spp. strains in the aetiology of infections, and these microorganisms’
sensitivity to antibiotics.

To this end, both the strains isolated from hospital ward infections as well as
the strains isolated from outpatients were taken into study.
Secondly, the present paper attempts to highlight the circulating resistance
phenotypes of the enterococci strains isolated from the hospital wards as well as from
outpatients, which phenotypes play a role in the improvement of the empirical
antibiotherapy of the infections caused by these strains.
Statistical methodology
The data obtained as a result of research was entered into and stored in
Microsoft Access 2010-type (*.mdb format) databases and was exported for processing
to Excel 2010 (*.xls) files.
Statistic processing was carried out with the aid of Microsoft Excel 2010, EPI-
INFO version 6.04 and IBM SPSS version 10 packages, and the results of this processing
were exported in all three cases to Excel 2010 worksheet files.
The dichotomous variables, characterized by percentage, were compared by
applying contingency tables and calculating the χ2 test, and using the Fisher correction
where necessary 22.
All these statistical tests were calculated with two extremities, and the
statistical significance value p was taken into account in the case of values ≤ 0.01.
General conclusions
Although they are not among the most frequently isolated pathogenic
agents, enterococci, through their natural resistance to cephalosporins, are part of the
flora involved in the aetiology of certain serious infections in hospitalized patients.
The establishment of the circulating resistance phenotypes in hospital
but also in outpatients is important in the urgent development of the anti-infective
therapy.
The statistically-significant differences regarding the resistance to
antibiotics in favour of the hospital, as opposed to outpatients, are connected to the
more frequent administration of antibiotics in the case of hospitalized patients, but
also to the presence of other risk factors regarding the transmission of multiresistant
strains.
Enterococci hold numerous resistance mechanisms capable of ensuring
their survival in the presence of antibiotics, which calls for the identification of certain
possible new action targets for antibiotics.
The control of the emergence and spread of antibioresistance is a public
health priority, worldwide.
by different medical and outpatient services, represents an all-time first for university
hospitals and private laboratories in Cluj-Napoca and also nationwide.
Specifying the frequency and aetiology of different types of enterococcal
infections (urinary infections, post-operative infections, respiratory infections,
bacteraemias/septicaemias, gynaecological infections, peritonitis, cholecystitis, CVC
infections) is genuinely useful in the prevention and control of these infections,
especially those caused by VRE strains.
The highlighting of the VRE strains as well as of the multiple resistance
to antibiotics of these microorganisms isolated from hospitalized patients is an
important personal contribution that has outstanding practical repercussions, as it is
an “indicative guide” for laboratory doctors and clinicians.
Specifying the resistance phenotypes by antibiotic class (beta-lactams,
aminoglycosides, glycopeptides, MLSB), as well as the circulating phenotypes of the
Enterococcus spp. strains is really helpful, because it can point the way for clinicians to
a highly efficient antimicrobial therapy.
