I. PARTEA GENERALĂ
1. OBEZITATEA – boală metabolică populaţională
1.1. INTRODUCERE. Obezitatea este încadrată în grupa bolilor metabolico – nutriţionale
populaţionale. Astăzi ele recunosc o „explozie” mondială ca frecvenţă în populaţia generală. Complexa
patologie metabolică se caracterizează printr-un risc crescut de morbiditate şi mortalitate, în special prin
boli cardiovasculare. Impactul individual şi general al bolilor metabolice se concretizează în cel puţin trei
aspecte: biologic, epidemiologic şi socio – economic. Prevenţia şi managementul clinic integrat, cu
consecinţe pozitive prin reducerea semnificativă a riscului cardiovascular sunt obiective principale ale
asistenţei medicale în bolile populaţionale. Se estimează că în mileniul III această patologie va constitui o
problemă majoră de sănătate publică, cu mare extindere şi grave afectări la nivel de individ şi de
societate.
În cadrul bolilor metabolice populaţionale, pe lângă obezitate se numără dislipidemiile şi diabetul
zaharat tip 2, toate caracterizate prin multiple elemente comune: mecanisme patogenetice, manifestare
clinică în prezenţa complicaţiilor deja instalate, asocieri morbide cu important rol de potenţare reciprocă,
cronicizare acompaniată de risc cardiovascular progresiv în evoluţie.
1.2. DEFINIŢIE. În definirea modernă şi complexă a termenului de „obezitate” se apelează la o
multitudine de abordări. Abordarea fiziologică vizează creşterea peste normal a procentului de masă
grasă. Abordarea fiziopatologică, mai actuală, presupune definirea unui sindrom poligenic heterogen,
rezultat din pozitivarea balanţei energetice. Abordarea medicală subliniază prezenţa comorbidităţilor şi
complicaţiilor bolii şi include în diagnostic şi tratament cele mai noi achiziţii ale ştiinţelor medicale.
Abordarea statistică priveşte obezitatea la nivel populaţional, şi o caracterizează prin abaterile pozitive de
la o greutate considerată normală (greutatea corespunzătoare celei mai lungi durate de viaţă). Abordarea
psiho-socială se bazează pe criterii estetice, atingerea lor devenind un obiectiv important şi suficient
uneori pentru a motiva obezul să-şi modifice stilul de viaţă.
În absenţa metodelor fiabile de măsurare directă a masei de ţesut adipos, în practica clinică s-a
recurs la o estimare indirectă, antropometrică, numită „indice de masă corporală”. Acesta se defineşte ca
raportul dintre greutatea actuală a persoanei, exprimată în kg, şi pătratul înalţimii, exprimat în m2. Această
formulă de calcul a fost propusă în 1869 de către Quetelet, dar a devenit uzuală în clinică doar în 1972,
graţie adoptării ei de către Keys.
1.3. EPIDEMIOLOGIE. Caracteristica epidemiologiei obezităţii este prevalenţa ei crescută atât în
ţările dezvoltate industrial, cât şi în zonele defavorizate, prin subdezvoltare, ale lumii. Se poate chiar
discuta despre o „pandemie” de obezitate. Realitatea contemporană recunoaşte „globezitatea” (1) drept cea
mai frecventă boală de nutriţie, cu tendinţa de a înlocui treptat patologia dominantă a secolului trecut –
bolile infecto-contagioase şi malnutriţia. Obezitatea este inclusă între primele zece cauze preventibile de
mortalitate, fiind una din cele mai vizibile, dar neglijate, probleme de sănătate publică (OMS, 2002) (2).
Dincolo de dimensiunea estetică cauzatoare de dizarmonie afectivă, obezitatea se impune pe plan medical
prin asocierea cu numeroase afecţiuni cronice: boli metabolice şi cardiovasculare, suferinţe endocrine şi
articulare, anumite tipuri de neoplazii (3).
1.4. ETIOPATOGENIA OBEZITĂŢII
1.4.1. Factorii genetici
1.4.1.1. Evaluarea eredităţii în obezitate. Genetica obezităţii recunoaşte interacţiunea a multiple
gene care individual ar avea un efect relativ scăzut (gene de susceptibilitate), dar care, interacţionând cu
alte gene şi diverşi factori de mediu se vor exprima clinic. În cercetarea genelor implicate în obezitatea
comună au fost utilizate două abordari: studiul „genelor candidate” şi studiul genomului uman.
1.4.1.2. Gene implicate în apariţia şi dezvoltarea obezităţii. Din punct de vedere genetic, în cea
mai mare parte a cazurilor, apariţia obezităţii este datorată efectului sumativ şi interactiv al unor gene
individuale. În controlul aportului alimentar şi al homeostaziei energetice, un rol important îl joacă
numeroase neuropeptide hipotalamice. La om, gena leptinei se află pe braţul lung al cromozomului 7, iar
gena pentru receptorul leptinei pe braţul scurt al cromozomului 1. Proteina tubby, cu rol necunoscut,
probabil implicată în dezvoltarea circuitului neuronal hipotalamic, este codificată de o genă numită tub.
1.4.2. Metabolismul trofinelor calorigene. Trofinele calorigene sunt substanţe organice donatoare
de energie pentru organism. Ele provin din alimente şi sunt reprezentate prin glucide, lipide şi proteine.
Prin metabolism se înţelege totalitatea transformărilor biochimice care au loc în organismul uman în
scopul menţinerii funcţiilor sale în limite fiziologice.

1.4.3. Consumul de alcool. În cadrul unui stil de viaţă sănătos se acceptă ( a nu se confunda cu „se
recomandă”) un consum moderat, ponderat de alcool. Acesta reprezintă echivalentul zilnic a: un pahar (50
ml) de „tărie” (băutură alcoolică concentrată) (ţuică, whisky, coniac etc.) sau un pahar (200 ml) de vin sau
o sticlă (500 ml) de bere.
1.4.4. Balanţa energetică. Specia umană supravieţuieşte graţie energiei obţinute prin
metabolizarea alimentelor. Doar plantele pot încorpora energie solară prin intermediul procesului de
fotosinteză. Omul nu poate încorpora energie radiantă, deci singura sursă disponibilă este aportul exogen
prin alimente (63). Balanţa energetică reprezintă aportul energetic necesar menţinerii greutăţii corporale
constante. În cadrul alimentaţiei sănătoase balanţa energetică trebuie să fie echilibrată. Practic, este vorba
despre un echilibru între aporturile şi cheltuielile de energie (62). Când aporturile energetice depăşesc
cheltuielile apare supraponderea şi obezitatea.
1.4.5. Reglarea neuro – endocrină a obezităţii. Balanţa energetică recunoaşte o reglare pe
termen scurt, care se referă mai ales la aportul alimentar şi efectele imediate şi directe ale ingestiei, şi o
reglare pe termen lung, care urmăreşte menţinerea unei cantităţi normale de depozite nutritive în organism
(70). Centrii nervoşi implicaţi în reglarea balanţei aport/ cheltuieli energetice sunt situaţi la nivelul
hipotalamusului. Ei sunt reprezentaţi prin centrul foamei, situat în nuclei hipotalamici laterali şi centrul
saţietăţii, în nuclei hipotalamusului ventro – median.

1.4.6. Fiziopatologia obezităţii. Balanţa energetică este una dinamică. Dacă ecuaţia Energie
ingerată – Energie consumată = Energie stocată/ economisită este reală în condiţii staţionare, ea nu ţine
cont de faptul că energia stocată antrenează o contrareglare care duce la creşterea pierderilor energetice.
Este necesar un dezechilibru cronic, exprimat prin ecuaţia dinamică: Schimbări în energia ingerată –
Schimbări în energia pierdută = Schimbări în energia stocată/ economisită.
1.5. DIAGNOSTICUL ŞI CLASIFICAREA OBEZITĂŢII
1.5.1. Depistarea şi evaluarea iniţială. Depistarea obezităţii se recomandă a se realiza la toate
controalele periodice de sănătate, consult medical în orice specialitate, situaţii speciale sau prin screening
dirijat ţintit.
1.5.2. Clasificarea obezităţii
1.5.3. Diagnosticul şi evaluarea finală a obezităţii.
Diagnosticul obezităţii va preciza: tipul abdominal/ gluteo–femural, clasa clinică de risc şi
condiţii morbide asociate: hipertensiune arterială, diabet zaharat, dislipidemie, precum şi orice altă
comorbiditate.
1.6. COMPLICAŢII ŞI ASOCIERI MORBIDE
1.6.1. Complicaţii metabolice (insulinorezistenţa, diabetul zaharat, dislipidemiile)
1.6.2. Sindromul metabolic. Sindromul metabolic este recunoscut în prezent ca o entitate clinică
ce generează un important risc cardiovascular prin factorii cumulaţi.
1.6.3. Afecţiuni cardiovasculare. Complicaţiile cardiovasculare în obezitate sunt cele mai
frecvente şi cu cel mai mare impact asupra stării de sănătate.
1.6.4. Obezitatea şi afecţiunile respiratorii
1.6.5. Complicaţii hepatice. În 1980, Ludwig a introdus şi descris termenul de steatohepatită
nonalcoolică (NASH-nonacoholic steatohepatitis) pentru a denumi un sindrom clinico-patologic găsit
frecvent la populaţia obeză şi/ sau diabetică neconsumatoare de alcool, detaliindu-l ulterior (147).
1.6.6. Afecţiuni osteo – articulare. Obezitatea se asociază cu o patologie osteo – articulară
complexă, care afectează un procent important din populaţia de vârstă medie şi avansată, fiind considerate
una din cele mai costisitoare şi invalidante boli.
1.6.7. Obezitatea şi patologia endocrină

1.6.8. Obezitatea şi cancerul. Obezitatea determină un risc crescut de dezvoltare a unor tumori
maligne: la bărbaţi cancerul de prostată şi de colon, iar la femei cancerul de sân, uter şi ovar.
2. MANAGEMENTUL CLINIC AL OBEZITĂŢII
2.1. PRINCIPII GENERALE. Managementul greutăţii presupune totalitatea acţiunilor prin care
se realizează obiectivele terapeutice impuse de situaţia clinico-biologică, socială şi psihologică a
pacientului (159, 160). Principalele obiective propuse sunt: scădere ponderală, menţinerea noii greutăţi,
profilaxia obezitaăţii, alături de controlul complicaţiilor şi al condiţiilor asociate. Abordarea modernă în
managementul clinic al obezităţii recunoaşte o strategie care se bazează pe cicluri „scădere ponderală –
menţinere”.
2.2. OBIECTIVE. Obiectivele managementului clinic al obezităţii se cer a fi stabilite împreună
cu pacientul, în funcţie de clasa clinică de risc, asocierile morbide şi complicaţiile diagnosticate. Se
stabilesc obiective pe termen scurt (primul ciclu de „scădere – menţinere ponderală”) şi pe termen lung.
2.3. PROGRAM TERAPEUTIC. Metodele managementului clinic al obezităţii sunt: optimizarea
stilului de viaţă, medicaţia specifică, intervenţii chirurgicale şi intervenţii specifice.
2.3.1. Optimizarea stilului de viaţă. Optimizarea stilului de viaţă presupune dietele hipocalorice,
exerciţiu fizic, terapie comportamentală şi abandonarea fumatului şi consumului de alcool.
2.3.2. Medicaţia specifică. Farmacoterapia trebuie luată în considerare în cazul obezităţii cu risc
crescut şi a formelor rezistente.
2.3.3. Intervenţii chirurgicale. Metodele invazive se aplică în situaţii extreme, la pacienţi cu
IMC > 40 kg/ m2, cu acumulare masivă şi handicapantă a ţesutului adipos cu deformări consecutive
majore sau la cei cu risc clinic foarte crescut, dacă acesta, prin patologia specifică, permite intervenţia
chirurgicală.
2.3.4. Metode adjuvante
2.4. EDUCAŢIA TERAPEUTICĂ
2.4.1. Definiţie, componente. În obezitate, educaţia terapeutică este specifică şi reprezintă
totalitatea activităţilor efectuate de medic sau asistenta medicală cu scopul de a instrui pacientul asupra
bolii, riscului pe care îl presupune şi modalităţilor de terapie.

4.3.3. Istoria familială şi personală patologică a celor două loturi (tabelele 4.22 şi 4.23). Istoria
familială în cadrul lotului cu educaţie specifică se găseşte în tabelul 4.22. Se observă că obezitatea este
prezentă la rudele de gradul I în proporţie de 49%, confirmând componenta genetică a obezităţii în grad
asemănător cu cel citat în literatura de specialitate. Diabetul zaharat este reprezentat în proporţie de 30%
la rudele de gradul I. În istoria familială apar dislipidemiile şi bolile cardiovasculare în procente
apropiate: 47 şi 52.
4.3.4. Indicatori antropometrici. Gradul excesului ponderal, iniţial şi final, în cazul celor două
loturi se găseşte în tabelul 4.25. Analizând rezultatele, se constată o reducere semnificativă a obezităţii
gradul III ( de la 14% la 6%), o scădere importantă a cazurilor cu obezitate gradul I (de la 47% la 25,5%),
scădere explicabilă prin trecerea în clasa supraponderii, prin reducerea semnificativă a greutăţii. Logic şi
consecutiv, procentul pacienţilor diagnosticaţi cu suprapondere la evaluarea finală a crescut de la 26,5%
iniţial la 53%. Valoarea medie a tuturor indicatorilor antropometrici, indiferent că a fost vorba despre
obezitate singulară sau asociată cu diabet zaharat tip 2 sau glicemie bazală modificată, a scăzut
semnificativ. Cea mai importantă variaţie au înregistrat-o valorile medii ale greutăţii (-8,31 kg), IMC-ului
(-3,06 kg/m²) şi circumferinţei abdominale (-7,63 cm) la pacienţii cu obezitate simplă, probabil de aport.
4.3.5. Parametri biochimici. Valoarea medie a glicemiei lotului cu educaţie specifică a scăzut
semnificativ (p < 0,001), de la 151,47 ± 74,57 mg/ dl până la 111,08 ± 22,31 mg/ dl. Lotul martor,
dimpotrivă, recunoaşte o creştere a valorii medii a glicemiei, de la 107,16 ± 23,36 mg/ dl până la 108,16 ±
21,59 mg/ dl la finele studiului. Variabilitatea glicemiei în funcţie de valoarea circumferinţei abdominale
se află iconografiată în figura 4.75. 3,6% din variabilitatea glicemiei se explică prin variaţia
circumferinţei abdominale. Valoarea medie a hemoglobinei glicate, dozată la pacienţii cu diabet zaharat
tip 2, a scăzut semnificativ statistic (p = 0,001), de la 8,37 ± 1,8% până la 6,65 ± 1,12 %. Variabilitatea
hemoglobinei glicate în funcţie de valoarea IMC se poate urmări în graficul 4.76. 3,4% din variabilitatea
hemoglobinei glicate este explicată prin variaţia valorii medii a IMC-ului. Valoarea medie a colesterolului
seric total a scăzut semnificativ statistic (p < 0,001) la pacienţii lotului cu educaţie terapeutică specifică,
de la 244,18 ± 80,10 mg/ dl până la 192,03 ± 39,72 mg/ dl la evaluarea finală. În cazul lotului martor
aceeaşi variaţie a fost nesemnificativă statistic. 3,9% din variaţia colesterolului total este explicată prin
reducerea greutăţii. Liderii de opinie preocupaţi de problema obezităţii şi a riscului cardiovascular discută
azi despre ceea ce se cunoaşte deja sub numele de „talie hipertrigliceridemică”. Asocierea
hipertrigliceridemiei (357 mg/dl) la o circumferinta abdominala crescuta, in cazul lotului nostru de 109
cm defineste „talia hipertrigliceridemică”. 3% din variabilitatea trigliceridelor este explicată de variaţia
circumferinţei abdominale (figura 4.85.).
4.3.6. Tensiunea arterială. Dacă în momentul evaluării iniţiale valoarea medie a tensiunii
arteriale sistolice se situa în zona de hipertensiune (150,93 ± 24,34, mmHg) la finele studiului ea se
normaliza (133,35 ± 18,08 mmHg), în condiţii de educaţie terapeutică continuă şi farmacoterapie în doze
decise de obiectivele clinice. Variabilitatea tensiunii arteriale diastolice în funcţie de IMC este chiar mai
puternică statistic, 10,7% din această variabilitate fiind explicată prin variaţia valorilor medii ale IMC-
ului (figura 4.96).
4.3.7. Riscul cardiovascular. 16,3% din variabilitatea riscului cardiovascular se explică prin
variaţia IMC-ului, deci a reducerii greutăţii, ţinând cont de faptul că inalţimea, în cadrul formulei de
calcul a IMC, este variabilă nemodificabilă (figura 4.100).
4.3.8. Clasa clinică de risc. Modificarea distribuţiei clasei clinice de risc după intervenţie prin
educaţie terapeutică se găseşte iconografiată în figura 4.103. Distribuţia clasei clinice de risc a obezităţii
la evaluarea iniţială şi finală reproduce modificarea factorilor de risc cardiovascular asupra cărora s-a
acţionat, diferenţiat, în timpul perioadei de intervenţie şi monitorizare.
4.3.9. Aprecierea calităţii vieţii. S-au evaluat pacienţii în ceea ce priveşte calitatea vieţii în bolile
metabolice populaţionale printr-un chestionar pe care aceştia l-au completat la includerea în studiu şi la
finele lui. Pacienţii lotului martor au completat acelaşi chestionar de evaluare a calităţii vieţii, dar aceştia
nu au fost incluşi în programul de educaţie terapeutică. Iniţial, 22% dintre pacienţi au apreciat că au o
calitate scăzută a vieţii, 44% moderată şi 34% au recunoscut calitatea crescută a vieţii. La finele studiului,
doar 12% dintre subiecţi aveau calitate scăzută a vieţii, un beneficiu indubitabil al optimizării stilului de
viaţă. Modificările procentuale de autoevaluare a calităţii vieţii subiecţilor din lotul martor au fost
minime, de maximum 2%, în condiţiile în care ei nu au beneficiat de educaţie terapeutică specifică.

IV. CONCLUZII
Educaţia terapeutică specifică în managementul clinic al obezităţii, sistematică şi continuă, este
parte integrantă a tratamentului.
Evaluarea eficienţei educaţiei specifice în obezitate este dificilă prin cuantificare intrinsecă. De
aceea, beneficiile se apreciază prin evaluarea obiectivelor terapeutice.
Atingerea obiectivelor terapeutice presupune aplicarea unui program educaţional individualizat,
bine structurat şi continuu, adaptat condiţiilor biologice şi socio-familiale ale persoanei, cu
implicarea acesteia în propriul tratament.
Condiţiile comune celor 3 studii sunt obezitatea şi educaţia terapeutică specifică. Adiacent, în
studiul A s-a urmărit în mod particular variaţia factorilor de risc cardiovascular şi a
insulinorezistenţei. În cazul subiecţilor incluşi în studiul B s-a realizat şi analiza aportului
nutriţional, a structurii raţiei, precum şi cea a frecvenţei consumului diferitelor alimente în funcţie
de obiceiuri şi preferinţe alimentare. Pacienţii incluşi în studiul C au beneficiat, suplimentar, de
aprecierea calităţii vieţii.
Obiectivele terapeutice propuse au fost scăderea ponderală, menţinerea noii greutăţi, controlul
condiţiilor asociate şi al complicaţiilor obezităţii.
Asocierile dismetabolice prezente la pacienţii incluşi în cele 3 studii efectuate conturează
diagnosticul de sindrom metabolic.
Scăderea ponderală, a IMC-ului şi circumferinţei abdominale, precum şi ameliorarea valorilor
profilului lipidic se desfăşoară în paralel cu reducerea riscului cardiovascular.
Beneficiile pe termen lung pot fi puse în evidenţă doar prin studii cu întindere mare în timp, interval în
care optimizarea stilului de viaţă să devină obisnuinţă.
Eficienţa abordării moderne în terapia nonfarmacologică a obezităţii presupune cunostinţe
complexe şi echipă multidisciplinară.

I. GENERAL PART
1. 1. OBESITY – a metabolic populational disease
1.1 INTRODUCTION. Obesity belongs to the populational metabolic nutritional diseases.
Nowadays they recognize a worldwide ”explosion” in frequency in the general population. The complex
metabolic pathology is characterised by a high morbidity and mortality, especially in cardiovascular
diseases. The individual and general impact of metabolic diseases materialises itself in at least three
aspects: biological, epidemiologic and socio – economic. Clinical integrated prevention and management,
with positive consequences through the significant decrease of the cardiovalscular risk are main
objectives of medical assistance in the populational diseases. We can esteem that in the third millennium
this pathology will turn into a major public health problem, with great extension and serious
consequences at an individual and society level.
Within the group of metabolic populational diseases, besides obesity there are hyperlypidemia and
diabetes mellitus type 2, all characterized by multiple common elements: pathogenetic mechanisms,
clinical manifestation in the presence of already existing complications, morbid associations with an
important mutual reinforcement function, chronic evolution accompanied by a progressive cardiovascular
risk in evolution.
1.2. DEFINITION. For a modern and complex definition of the term “obesity” we rely on a
multitude of approaches. The physiological approach refers to an overincrease of the fat intake
percentage. The physiopathological, more actual approach, requires the definition of a poligene
heterogenous sindrome, resulted from the positive value of the energetic balance. The medical approach
underlines the presence of comorbidities and of disease complications and it includes the latest findings of
the medical sciences in the diagnosis and treatment. The statistic approach deals with obesity at a
populational level, and characterizes it by positive changes from a normal weight (the weight
corresponding to the longest life period). The psycho – social approach is based on aesthetic criteria, and
reaching them, sometimes, becomes an important and sufficient objective to motivate the obese to change
his/ her lifestyle.
In the absence of reliable methods for a direct measurement of the fat tissue, an indirect,
anthropometric value, known as body mass index was used in clinical practice. This value is defined as the
relation between the actual weight of the person, expressed in kg, and the height square, expressed in m2 .
This counting formula was proposed in 1869 by Quetelet, but, in the clinic, it became customary only in
1972, thanks to Keys who adopted it.
1.3. EPIDEMIOLOGY. The characteristic of the obesity epidemiology is its high prevalence both
in industrially developped countries and in underprivileged areas of the world, due to underdevelopment. It
is even possible to discuss about a “pandemy” of obesity. The contemporary reality recognizes “globesity”
(1) as the most frequent nutritional disease, with a tendency to replace, gradually, the dominant pathology
of the past century – the infective–contageous diseases and malnutrition. Obesity is included in the first ten
preventive causes of mortality, being one of the most visible, but neglected, public health problems (OMS,
2002) (2). Besides the aesthetic dimension causing emotional disharmony, obesity imposes itself at a
medical level in its association with various chronic diseases: metabolic and cardiovascular diseases,
endocrine and articular suffering, certain types of cancer (3).
1.4. THE ETHIOPATHOGENY OF OBESITY
1.4.1. Genetic factors
1.4.1.1. Heredity evaluation in obesity. The genetics of obesity recognizes the interaction of
multiple genes that individually would have a relatively insignificant effect (susceptible genes), but which,
interacting with other genes and other environmental factors will be clinically expressed. Within the
investigation of the genes involved in common obesity, two approaches were used: the study of the
“candidate genes” and the study of the human genome.
1.4.1.2. Genes involved in the outbreak and development of obesity. From a genetic point of
view, in the most part of the cases, the outbreak of obesity is due to the additional and interactive effect of
certain individual genes. The numerous hypothalamic neuropeptides play an important role in the control
of food intake and of energetic homeostasia. With humans, the leptine gene is situated on the long arm of
chromosome 7, and the gene for the leptine receptor on the short arm of chromosome 1. The tubby protein,
with an unknown role, maybe involved in the development of the neural hypothalamic circuit, is encoded
by a gene named tub.

1.4.2. The metabolism of the energy – producing trophines. The energy – producing trophines
are organic substances, producing energy for the organism. They come from food and are represented in
the carbohydrates, lypids and proteins. Metabolism means the totality of biochemical transformations that
take place in the human body in order to keep its functions within physiological limits.
1.4.2.1. The metabolism of the carbohydrates

1.4.3. Alcohol consumption. In a healthy lifestyle it is accepted (not to be confused with “it is
recomended”) a moderate, considered alcohol intake. This represents the daily equivalent of: a glass (50
ml) of “liquor”, (alcoholic concentrated drink) (tuica, whisky, cognac) or a glass (200 ml) of wine or a
bottle (500 ml) of beer.
1.4.4. The energetic balance. The humans survive thanks to the energy resulted from food
metabolisation. Only plants can acquire solar energy by means of the photosynthesis process. The humans
cannot acquire radiant energy, therefore their only available source is food intake (63). The energetic
balance represents the energetic intake necessary to maintain a constant body weight. For a healthy food
the energetic balance must be equilibrated. Practically, there is a balance between intake and consumption
of energy (62). When the energetic intake exceeds consumption, overweight and obesity appear.
1.4.5. The neuro – endocrine adjustment of obesity. The energetic balance recognizes a short
term adjustment, that refers especially to the food intake and the immediate and direct effects of the
ingestion, and a long term regulation, that has the puropose to maintain a normal quantity of nutritional
deposits in the organism (70). The nervous centres involved in the regulation of the intake/ energetic
consumption balance, are situated at the level of the hypothalamus. They are represented by the hunger
centre, situated in the hypothalamic lateral nuclei and the sate centre, in the nuclei of the ventro – median
hypothalamus.

1.4.6. The physiopathology of obesity. The energetic balance is dynamic. If the equation intaken
Energy – consumpted Energy = stocked/ saved Energy is real in stationary conditions, it doesn’t take into
account the fact that the stocked energy activates a counterregulation that produces an increase of the
energetic loss. A chronic debalance, expressed by the dynamic equation: Modifications in the intaken
energy – Modifications in the lost energy = Modifications in the stocked/ saved energy, is necessary.
1.5. DIAGNOSIS AND CLASSIFICATION OF OBESITY
1.5.1. Diagnosis and initial evaluation. The diagnosis of obesity is recommended to be made in
all periodic health controls, medical consult in any speciality, special situations or by means of a directed,
aimed screening.
1.5.2. Classification of obesity
1.5.3. Diagnosis and final evaluation of obesity.
The diagnosis of obesity will precise: the abdominal/ gynecoid type, the clinical risk class and the
morbid associated conditions : hypertension, diabetes mellitus type 2, dyslypidemia, as well as any other
comorbidity.
1.6. COMPLICATIONS AND MORBID ASSOCIATIONS
1.6.1. Metabolic complications (insulineresistance, diabetes mellitus type 2, dyslypidemia)
1.6.2. The metabolic sindrome. At present, he metabolic sindrome is recognized as being a
clinical entity that generates an important cardiovascular risk by means of the accumulated factors.
1.6.3. Cardiovascular diseases. The cardiovascular complications in obesity are the most
frequent ant with the greatest impact on the state of health.
1.6.4. Obesity and the respiratory diseases
1.6.5. Hepatic complications. In 1980, Ludwig introduced, described and later detailed the term
of nonalcoholic steatohepatitis (NASH) to name a clinical pathological sindrome frequently found in the
obese and/ or diabetic non alcohol consuming population (147).

1.6.6. Osseous – articular diseases. Obesity is associated with a compex osseous – articular
pathology, tha affects an important percentage of the middle – aged and old population, being considered as one
of the most expensive and invalidating diseases.
1.6.7. Obesity and the endocrine pathology
1.6.8. Obesity and the cancer. Obesity determines a high risk in the development of malign
tumors: with men prostate and colon cancer, and with women breast, uterus and ovary cancer.
2. THE CLINICAL MANAGEMENT OF OBESITY
2.1. GENERAL PRINCIPLES. The weight management postulates the totality of the actions, by
means of which, the therapeutic objectives imposed by the clinical, biological and psychological
situation of the patient are achieved (159, 160). The main objectives proposed are: weight loss,
maintenance of the newly achieved weight, the profilaxy of obesity, together with the control of the
complications and of the associated conditions. The modern approach in the clinical management of
obesity recognizes a strategy based on “weight loss – maintenance” cycles.
2.2. OBJECTIVES. It is required that the objectives of the clinical management of obesity be set
together with the patient, considering the clinical risk class, the morbid associations and the diagnosed
complications. Short term objectives (the first “weight loss – maintenance” cycle) and long term
objectives are set.
2.3. THERAPEUTIC PROGRAMME. The methods of the clinical management of obesity are:
lifestyle improvement, specific medication, surgery and specific interventions.
2.3.1. Lifestyle improvement. Lifestyle improvement postulates diets, physical exercise,
behaviour therapy and giving up smoking and alcohol.
2.3.2. Specific medication. Pharmacotherapy has to be considered in what concerns the high risk
obesity and the resistant forms.
2.3.3. Surgical interventions. Invasive methods are applied in extreme situations, in patients with
BMI > 40 kg/ m2, with a massive and severe accumulation of fat tissue, with consequent major
modifications, or in those with high increased clinical risk, if this, with specific pathology, permits
surgery.
2.3.4. Adjuvant methods
2.4. THERAPEUTIC EDUCATION
2.4.1. Definitions, components. In obesity, the therapeutic education is specific and represents
the totality of activities carried out by the doctor or by the nurse, in order to instruct the patient on the
disease, the risk that it involves and the procedures for therapy.

4.3. Study C
4.3.1. Characteristics of the groups. The average age of the patients who benefitted from
therapeutic specific education is 50,7 ± 12,63 years, and that of the control group is 59,46 ± 12,26%. The
studied group has the following percentual distribution considering sex: 61,5% are women and 38,5%
men, 83% of them coming from the urban area and 17% from the rural area. The control group has an
almost identical distribution: 79,3% are women and 20,7% men, 66,7% coming from the urban area and
33,3% from the rural area. In what concerns the food behaviour, 66% of the subjects in the experimental
group admit (~2/3) the lack of breakfast from their food habits, and, unfortunately, 21% don’t eat bread.
The eating frequency has a varied distribution in the patients (tables 4.20 and 4.21): almost half of them
(44%) have only two meals a day, 8% don’t have snacks, and 24% of them have more than 4 snacks/ day.
4.3.2. The weight history. In the group with specific intervention through education, 72% of the
subjects admit previous diets, but, unfortunately, only 35% of those who had results could maintain their
weight.
4.3.3. Family and personal pathological history of the two groups (tables 4.22 and 4.23). The
family history in the group with specific education is presented in the table 4.22. We can observe that
obesity is present in first – degree family members in a proportion of 49%, confirming the genetic
component of obesity at the same degree with the one mentioned by the literature of speciality. The
diabetes mellitus type 2 is represented in a proportion of 30% with first – degree family members.
Dyslypidemia and cardiovascular diseases appear in the family history in close percentages: 47 and 52.
4.3.4. Anthopometric indicators. The overweight, initial and final degree, in the case of the two
groups is presented in the table 4.25. Analysing the results, we notice a significant reduction of the III
degree of obesity ( from 14% to 6%), an important reduction of the cases with I degree obesity (from 47%
to 25,5%), a reduction explainable by the passage to the overweight class, through significant weight loss.
Logically and consequently, the percentage of the patients diagnosed overweight at the final evaluation,
increased from 26,5% initially to 53%. The average value of all the anthropometric indicators, regardless
of singular obesity or obesity associated with diabetes mellitus type 2 or modified glycemia was
significantly reduced. The most important variation was registered by the average values of weight, (-8,31
kg), the BMI (-3,06 kg/m²) and the waist circumference (-7,63 cm) in patients with simple, maybe intake
obesity.
4.3.5. Biochemical parametres. The average value of the glycemia in the group with specific
education was significantly reduced (p < 0,001), from 151,47 ± 74,57 mg/ dl to 111,08 ± 22,31 mg/ dl.
The control group, on the contrary, admits an increase of the average value of glycemia, from 107,16 ±
23,36 mg/ dl to 108,16 ± 21,59 mg/ dl at the end of the study. The variability of glycemia, considering the
waist circumference, is presented in the figure 4.75. 3,6% of the variability of glycemia is explained by
the variation of the waist circumference. The average value of the HbA1c, measured in the patients with
diabetes mellitus type 2, was significantly reduced from a statistic point of view (p = 0,001), from 8,37 ±
1,8% up to 6,65 ± 1,12 %. The variability of the HbA1c considering the value of the BMI is shown in the
figure 4.76. 3,4% from the variability of the HbA1c is explained by the variation of the average value of
the BMI. The average value of the seric total cholesterol was significantly reduced from a statistic point
of view (p < 0,001) in the patients of the group with therapeutic specific education, from 244,18 ± 80,10
mg/ dl to 192,03 ± 39,72 mg/ dl at the final evaluation. In the case of the control group the same variation
was insignificant from a statistic point of view. 3,9% of the total cholesterol variation is explained by
weight loss. The specialists preoccupied with the problem of obesity and of the cardiovascular risk
nowadays talk about what it is already known as “hypertrigliceridemic – waist”. The association of
hypertrigliceridemia (357 mg/dl) with an increased waist circumference, in the case of our group of 109
cm, defines the “hypertrigliceridemic – waist”. 3% of the variability of triglycerides is explained by the
variation of the waist circumference (figure 4.85.).
4.3.6. The blood pressure. If in the moment of the initial evaluation the average value of the
sistolic blood pressure was situated in the hypertension area (150,93 ± 24,34 mmHg) at the end of the
study it became normal (133,35 ± 18,08 mmHg), in conditions of continuous therapeutic education and
pharmacotherapy in doses determined by the clinical objectives. The variability of diastolic blood
pressure considering the BMI is even stronger from a statistic point of view, 10,7% of this variability
being explained by the variation of the average values of the BMI (figure 4.96).

4.3.7. The cardiovascular risk. 16,3% of the variability of the cardiovascular risk is explained by
the BMI variation, therefore of the weight loss, taking into account the fact that height, in the BMI
scoring formula, is an unmodifiable variable (figure 4.100).
4.3.8. The clinical risk class. The modification of the distribution of the clinical risk class after
the intervention with therapeutic education is presented in the figure 4.103. The distribution of the clinical
risk class of obesity at the initial and final evaluation reproduces the modification of the cardiovascular
risk factors that were influenced, in different ways, during the period of intervention and monitorization.
4.3.9. Evaluation of the quality of life. Patients were evaluated in what concerns the quality of
life in metabolic populational diseases by means of a questionary that they filled in when they entered the
study and at the end of it. The patients of the control filled in the same form to evaluate the quality of life,
but these were not included in the therapeutic education programme. Initially, 22% of the patients
appreciated that they have a low quality of life, 44% moderate and 34% admitted the high quality of life.
At the end of the study, only 12% of the subjects had a low quality of life, an undoubtful benefit of the
improvement of lifestyle. The percentual changes in the self – evaluation of the quality of life in the
patients of the control group were minimal, of a maximum 2%, considering that they didn’t benefit from
therapeutic specific education.
III. DISCUSSIONS
IV. CONCLUSIONS
The therapeutic specific education in the clinical management of obesity, systematic and
continuous, is integrant part of the treatment.
The evaluation of the efficiency of specific education in obesity through intrinsic quantification is
difficult. Therefore, the benefits are appreciated evaluating the therapeutic objectives.
Reaching the therapeutic objectives means putting into practice an additional educational
programme, individualised, well structured and continuous, adapted to the biological, social and
family conditions of the person, getting him/ her involved in his/ her own treatment.
The common conditions of the three studies are obesity and therapeutic specific education.
Adjacently, in the study A we particularly followed the variation of the factors for cardiovascular
risk and insulineresistance. In the case of the subjects included in the study B we analysed the
nutritional intake, the ratio structure, as well as the frequency of consuming different kinds of food
considering food habits and preferences. The patients included in the study C benefitted, in
addition, from the evaluation of the quality of life.
The therapeutic objectives proposed were weight loss, maintenance of the newly achieved weight,
the control of the associated conditions and of the complications of obesity.
Dismetabolic associations present in the patients included in the 3 studies mark the diagnosis of
metabolic sindrome.
The weight loss, of the BMI and of the waist circumference, as well as the correction of the values
in the lypidic profile are accompanied by a reduction of the cardiovascular risk.
The long term benefits can be marked out only through long time extended studies, an interval when the
improvement of lifestyle would become a habit.
The efficiency of the modern approach in the non – pharmacological therapy of obesity requires
complex knowledge and a multidisciplinary team.
