Rezumat
Summary
The seventh edition of TNM classification of lung cancer is due to be published this year.
It is based by studies on a database which contains almost 68000 patients. The changes
will involve a reassessment of treatment algorithms and would improve the alignment of
TNM stage with prognosis. This article wishes to present some controversial aspects of
the surgical treatment of lung cancer, in order to be able to offer to our patients the best
management of this dreadful disease.

CRITERIILE T, N, M REDEFINITE
Tx – tumora primitivă nu poate fi evaluată sau este doar dovedită doar prin prezenţa
celulelor maligne în spută sau lavajul bronhoalveolar, fără evidenţierea ei imagistică sau
bronhoscopică.
T0 – nu se evidenţiază tumora primitivă.
Tis – tumoră in situ
T1 – tumoră mai mică sau egală cu 3 cm în cel mai mare diametru, înconjurată de ţesut
pulmonar sau pleură viscerală, fără aspecte bronhoscopice proximal de bronşia lobară (nu
în bronşia primitivă), cu excepţia tumorilor are invadează strict peretele bronşic, fără
extindere în parenchimul pulmonar adiacent, care poate fi localizată la orice nivel.
T1 a fost subîmpărţit în T1a (mai mică de 2 cm) şi T1b (între 2 şi 3 cm), dar nu
există semnificaţie statistică evidentă de supravieţuire mai mare pentru T1a.
T2 – tumoră cu dimensiuni cuprinse între 3 şi 7 cm sau care are una dintre următoarele
caracteristici:
Implică bronşia primitivă, dar maxim până la 2 cm distal de carena traheală
Invadează pleura vicerală
Asociată cu atelectazie sau pneumonită ostructivă care nu implică întregul plamân
T2 a fost subîmpărţit în T2a (între 3 şi 5 cm) şi T2b (între 5 şi 7 cm),
Fosta T2 care are dimensiuni mai mari de 7 cm a fost restadializată T3
T3 – tumoră cu dimensiuni mai mari de 7 cm sau tumoră care invadează direct una dintre
următoarele structuri: peretele toracic (inclusiv tumorile de apex), diafragmul, nervul
frenic, pleura mediastinală, pericardul; sau tumoră în bronşia primitivă la mai puţin de 2
cm de carenă dar fără implicarea acesteia; sau asociată cu atelectazie sau pneumonită
ostructivă care implică întregul plamân; sau nodul tumoral satelit în acelaşi lob cu tumora
primitivă (restadializare din T4)
T4 – tumoră de orice dimensiune care invadează una dintre următoarele structuri:
mediastin, cord, vase mari, trahee, nervul laringeu recurent, esofagul, corpii vertebrali,
carena sau nodul tumoral satelit în alt lob al aceluiaşi plămân decât tumora primitivă
(restadializare din M1)
Nx – nodulii limfatici nu pot fi evaluaţi
N0 – fără determinări tumorale în limfoganglioni
N1 – noduli intrapulmonari, peribronşici şi hilari ipsilaterali invadaţi tumoral, incusiv
prin extensie directă
N2 – invazie tumorală în nodulii limfatici mediastinali ipsilaterali şi sau subcarenari
N3 – invazie tumorală în nodulii limfatici mediastinali controlaterali, scalenici sau
supraclaviculari;
alţi noduli limfatici: axilari, inghinali etc sunt clasificaţi M1
Mx – nu poate fi evaluată prezenţa metastazelor la distanţă
M0 – fără metastaze la distanţă
M1 - metastaze la distanţă
M1a – nodul tumoral într-un lob controlateral sau citologie pozitivă în lichidul
pleural sau pericardic (restadializare din T4)
M1b – alte determinări secundare la distanţă
